Background: Xanthomas are rare, non-neoplastic lesions which occur due to defect in the LDL receptors leading to their accumulation in tendons and synovium. The prevalence of heterozygous familial hypercholesterolaemia (HFH) is approximately 1 in 500. Aim: To study a rare case of bilateral achilles tendon xanthoma and its functional outcome. Case Presentation: We presented a case of xanthoma over posterior aspect of both lower third legs just above the heel in an 18-year-old female, and diagnosis was confirmed by clinical examination ultrasound and MRI, for which subtotal resection was planned and further followed up. Postoperative treatment consisted of six weeks long leg cast immobilization. After 10 weeks the patient started walking without any difficulties. Conclusion: Subtotal resection of achilles tendon xanthoma has given a good functional outcome. There is no need for reconstruction of tendon in every case.
Back pain is a major issue in our country and its effective management remains a challenge. Prolapsed intervertebral disc is one of the major cause of illness leading to low back ache which is mostly affecting adults in their fourth decade of life. We have selected a study group of 120 people with single and double level disc bulges and prolapses, who underwent MRI for their low backache and managed them with ESI by different approaches, among which 63 patients were given interlaminar approach, 21 transforaminal approach and 36 caudal approach at the end we got fair to good results in our study, by this the time interval for surgical intervention has been increased.
Background: Supracondylar and intercondylar fractures of femur are very often difficult to treat and they are notorious for many complications. Despite recent evolution in the operating techniques and surgical implants, debate continues around the choice of implant for management of distal femur fractures. By this study, we explore the capability of a distal femur locking plate to counter distal femur fractures. Aim: To assess the efficacy of LCP in maintenance of post-operative total functional range of movements over the knee. Methods: Prospective observational study conducted on 30 patients at Mamata General Hospital, Khammam with supracondylar femur fracture. Patients with open 3A, 3B, 3C distal femur fracture, based on Gustilo-Anderson classification were taken into the study. Follow-up at 3 months, 6 months, 12 months and 18 months was carried out and evaluation was done according to the Neer scoring system. Results: In our study, the functional outcome was assessed by Neer's scoring system which revealed a satisfactory result upto a period of 6 months following which the results achieved at the end of 12&18 months showed excellent score where the p-value <0.001 and was considered significant. Conclusion:Overall scores from all the parameters assessed indicated a significant p-value. Choosing an ideal implant for fixation of compound supracondylar femur fracture is crucial. Our study, based on Neer's score shows locking compression plate as one of the ideal implants.
