Цель-проанализировать среднесрочные результаты протезирования аортального клапана (АК) биологическим протезом в центре сердечно-сосудистой хирургии. Материал и методы. В ретроспективное исследование включены 528 пациентов с патологией АК [269 мужчин (51%)/259 женщин (49%)]. Критерии включения: больные, которым выполнено протезирование АК биологическим протезом, возраст пациента 18 лет. Критерии исключения: протезирование 2 клапанов. Медиана возраста составила 65 [62-70] лет. Больных с инфекционным эндокардитом было 25 (4,7%), а 61 (11,6%) имели двухстворчатый АК. Медиана периода наблюдения составила 37,8 [2,2-77,4] мес.Результаты. Комбинированные вмешательства были выполнены в 87 (16,5%) случаях. Госпитальная летальность составила 1,1%. В раннем послеоперационном периоде 14 (2,7%) пациентам была показана имплантация постоянного электрокардиостимулятора в связи с нарушениями проводимости. У 2 (0,4%) пациентов развилось периоперационное повреждение миокарда, количество инсультов -8 (1,5%). 10-летняя общая выживаемость, свобода от сердечно-сосудистой смертности, инсульта, свобода от реоперации, свобода от протезного инфекционного эндокардита, свобода от дегенерации составила 58, 84,6, 93,3, 77,7, 95,1 и 87,2% соответственно. К концу периода наблюдения медиана пикового и среднего градиента на биологическом протезе составила 32 (25-40) и 18 (14-25) мм рт.ст., а аортальная регургитация II степени выявлена у 10 (5%) пациентов. Заключение. Отдаленная выживаемость и долговечность биологических протезов Braile Biomedica, SJM Biocor не отличается от показателей других доступных биологических протезов.Финансирование.
