ZusammenfassungZiel der vorliegenden, prospektiven Studie war es, auf der Basis des Konzeptes der DGEM-Leitlinie anhand der einfachen Parameter BMI, Gewichtsverhalten während der letzten 6 Monate sowie Serumalbumin das Screening einer Fehlernährung in einem deutschen Klinikum durchzuführen. Patienten und Methodik: 219 von insgesamt 310 Neuzugängen, 100 Frauen und 119 Män-ner einer internistischen Abteilung des Klinikums Fulda (66,1 15,8 Jahre), wurden konsekutiv über 10 Wochen in die Studie eingeschlossen. 91 Patienten erfüllten die Einschlusskriterien (Liegedauer < 48 h, physisch und psychisch bedingtes fehlendes Kooperationsvermögen) nicht und nahmen an der Studie nicht teil. Ergebnisse: Eine Fehlernährung, gemessen an Unterernäh-rung mit einem BMI < 18,5 kg/m 2 , lag bei 2,7 % der Patienten vor, eine grenzwertige Unterernährung mit einem BMI von 18,5 ± 19,9 kg/m 2 bei 5,5 % der Patienten. Mangelernährung mit einer unbeabsichtigten Gewichtsabnahme wurde bei 38,4 % der Patienten festgestellt. Eine hohe Krankheitsaktivität, gemessen an Albumin < 35 g/l, bestand bei 23,9 % der Patienten: 11 % hatten eine leichte, 6,7 % eine moderate und 6,2 % hatten eine schwere Hypalbuminämie. Bei Patienten mit Unterernährung bzw. mit grenzwertiger Unterernährung lagen relativ häufiger leichte und schwere Hypalbuminämien vor. Es besteht eine geringe Korrelation (r = 0,209, p £ 0,01) zwischen dem BMI und dem Albumin. Bei Patienten mit grenzwertiger Unterernährung lag die Liegedauer bei 10,9 4,9 Tagen im Vergleich zu Normalgewichtigen mit 8,9 7,1 Tagen.
