There is a growing use of email-based provision of information, development of health-related skills and interventions; however, use of email to assist women and children experiencing abuse after receiving Protection from Abuse (PFA) court order has not been explored. The specific aim of this research was to test the feasibility of an email device called MIVO for use in interacting with women and children after receiving PFA. This qualitative design used a three-step recruitment, screening and email interaction with mothers and their adolescent child after obtaining informed consent and training in the use of an email device. Sample included six pairs of mother and child (n = 12) who have received a PFA within the past 6 months. Demographic data were gathered using the sociodemographic questionnaire. Qualitative data were gathered using email messages from mother and child pairs. Results showed that email interaction is a feasible and acceptable way of providing support and information to survivors of abuse after their PFA. The following themes in their order and rank of appearance were found in the email interaction between the nurse and survivors: (1) safety issues; (2) job-related issues; (3) school-related issues; (4) parenting-related issues; and (5) health-related issues. Themes identified for the children were school work and friends. Privacy, confidentiality and respect for individual rights are paramount in email interactions. Email interaction is useful in education, screening, safety instructions and follow-up care. Technological devices such as MIVO may have usefulness as an email interaction device among women, their child and a nurse to reduce their risk for further interpersonal violence/abuse and to increase disclosure of abuse. Healthcare providers need to identify technological developments, and through evidence-based research examine their feasibility and adaptability for translation into practice specifically, in caring for survivors of abuse.
Especially for the diesel engine drivers, exposure can be regarded as rather high, in particular, when compared to recommended limits by SCOEL and MAK, though the exposures have been in line with the enforced German occupational exposure limits. Whether this exposure situation has caused adverse health effects will be investigated epidemiologically.
The effect of dust exposure on lung function described in older British and American coal miner studies was not confirmed. This can be explained partly by differences in methods (here: longitudinal studies, no prior exposure), but also by lower dust levels. NO (x) exposures showed no relevant influence on lung function-a result confirming findings from British coal mining.
This study does not provide evidence for elevated lung cancer risks. Methodologic limitations, incomplete ascertainment of death causes in particular, impede conclusions about exposure effects.
Zusammenfassung Hintergrund Cyanurchlorid (C3Cl3N3) ist ein weißes kristallines Pulver. Es wird für die Herstellung von Herbiziden benötigt, die in der Landwirtschaft eingesetzt werden. Weitere Verwendung findet es in der Produktion von Textilfarbstoffen und optischen Aufhellern oder der Herstellung von UV-Stabilisatoren für die Plastikindustrie sowie in der Gummiindustrie insbesondere für Automobilanwendungen. Zur Durchführung einer epidemiologischen Längsschnittstudie in drei Cyanurchloridproduktionsbetrieben wurde eine personenbezogene Expositionsermittlung durchgeführt, medizinische Response-Größen und wesentliche Kovariablen ermittelt [30]. In diesem zweiten Publikationsteil wird zur epidemiologischen Zusammenhangsanalyse berichtet. Methoden Um die Unsicherheit in den Belastungen aufgrund der schmalen Messdatenbasis besser abzuschätzen zu können, wurden über multiple Expositionsschätzungen 24 Varianten ermittelt und in Form von Einzelmodellen, einem Multi-Model-Approach und Detailanalysen ausgewertet. In der Längsschnittanalyse wurden „General-estimation-equation“ (GEE)-Modelle auf die wesentlichen Kovariablen adjustiert und für ausgewählte Zielgrößen in Abhängigkeit von kumulierter Exposition sowie in Abhängigkeit von durchschnittlicher Konzentrationshöhe und Expositionsdauer gefittet. Zusätzlich wurde eine Querschnittanalyse über den letzten Jahresdatensätzen der Studienteilnehmer durchgeführt. Hockey-Schläger-Modelle wurden gefittet, um Schwellenwerte zu schätzen. Ergebnisse Lungenerkrankungen hatten wegen zu geringer Fallzahlen keine Relevanz. Es konnte lediglich eine gewisse Sensibilisierung gegenüber Cyanurchlorid ohne Auswirkung auf die Lungenfunktionswerte nachgewiesen werden. Für das Gesamtkollektiv ergab die Einzelmodellbetrachtung gewisse Hinweise auf Lungenfunktionseinbußen nach Langzeitexposition gegenüber Cyanurchlorid. Die Multi-Model-Aanalyse wies keinen klaren Effekt der Cyanurchloridexposition nach, zeigte aber eine mögliche adverse Wirkung auf die Lungenfunktionsparameter VCmax (maximale Vitalkapazität, „maximal vital capacity“) und FEV1 (absolute Einsekundenkapazität, „forced expiratory volume in 1 second“) an. Bei Berücksichtigung von Modellen mit maximalen Lungenfunktionsverlustschätzungen konnte ein Langzeitschwellenwert für die kumulierte Exposition bei etwa 0,2 mg/m3-Jahre identifiziert werden. Orientiert an einem eher repräsentativen Modell, das den geschätzten mittleren Verlust beschrieb, ergab sich ein Schwellenwertbereich von 0,2–0,4 mg/m3-Jahre, wonach sich als beste Schätzung ein Wert von 0,3 mg/m3-Jahre anbietet. Diskussion Unter Berücksichtigung des geschätzten Langzeitschwellenwerts von 0,3 mg/m3-Jahre, einer durchschnittlichen Expositionsdauer von 11,2 Jahren und einer tolerablen zusätzlichen Lungenfunktionseinbuße von 10 % des altersüblichen Verlusts sowie einer Umrechung von Langzeitwerten auf Schichtwerte mit einem Faktor 2 ergibt sich eine Empfehlung für einen Schichtgrenzwert (TWA, „time-weighted average“, 8 h) von 0,06 mg/m3. Eine zukünftige Verbreiterung der Messdatenbasis und ein Follow-up der aktiven Studienpopulation erscheinen sinnvoll.
