Occupational ICD and AD appear to be strongly associated with severity of OHE. AD and severity of OHE were independently associated with prolonged sick leave. Having a food-related occupation appears to be associated with increased risk of loss of job.
Occupational hand eczema (OHE) is the most frequently recognized work-related disease in Denmark and the annual cost to society is high. Understanding of the epidemiology of OHE is essential to be able to give appropriate recommendations for its prevention. The study comprised 758 persons, 490 females and 268 males with recognized OHE in the period October 2001 to November 2002. Data were obtained prospectively from the National Board of Industrial Industry Registry and from a self-administered questionnaire (response rate, 82%). The most frequently recognized diagnosis was irritant contact dermatitis (ICD), mainly caused by wet occupations. The proportion of occupational ICD was equal for males and females, 59.7% and 63.1%, respectively. The estimated rates of OHE were high for bakers, hairdressers and dental surgery assistants, and a high proportion of apprentices were found among hairdressers. The prevalence of atopic dermatitis was low (16.4%) compared to previous studies among hand eczema patients. The prevalence of occupational allergic contact dermatitis in the study population was substantially higher among males than females, and the most frequent causes among males were allergy to chromium (leather exposure), rubber additives (gloves) and nickel due to exposure from work tools and metalworking industry.
Working capacity and dependence on the social welfare system were analyzed in 387 patients 5 years after hip replacement. Significant pre-operative factors were age, etiology of the hip disease and walking ability, and at the follow-up age, walking ability, pain and subjective experience of satisfaction with the operation. Among patients working pre-operatively, 92 per cent kept on working, and 70 per cent of patients on sick-leave went back to work. Only 9 per cent of patients working or sick-listed pre-operatively became invalidity pensioners, but none who were pensioners pre-operatively went back to work.
The risk of accidents and permanent disability is not increased in diabetic individuals. Thus, diabetic individuals should be offered accident insurance at a standard premium without limited coverage.
Totally, 28 consecutive patients with nontraumatic osteonecrosis of the femoral head were included in a prospective study of the clinical and radiographic results of core decompression. One patient (one hip) was lost to follow-up. Preoperatively, the stage according to Ficat (1985) was assessed by plain radiography and scintimetry. After 1-2 years, hip replacement had been performed in 4/11 Stage 1, 7/11 Stage 2, and 4/7 Stage 3 hips, while the remaining 12 hips were judged satisfactory. We conclude that the outcome of core decompression for nontraumatic necrosis of the femoral head is unpredictable.
