ZusammenfassungDer Autor beschreibt seine theoretische Annäherung an die psychosomatische Medizin. Sein Narrativ beginnt mit der grundlegenden psychoanalytischen Theorie der ¹psychosomatischen Krankheitenª. Diese geht davon aus, dass ein bestimmter unbewusster Konflikt eine bestimmte Krankheit verursachen kann. Diese Hypothese, die nicht genügend wissenschaftlich begründ-bar war, wurde später verlassen. An Stelle von ihr wurden weniger reduktionistische und weniger lineare Modelle entwickelt, so wie Multikausalität mit Einbeziehung von psycho-sozialen Faktoren. Andauernder Stress, den ein Individuum nicht auf geeignete Weise bewältigen kann, kann ein wichtiger Vorläufer von Krankheiten sein. Diejenigen, die mit Erfolg Stress bewältigen können, haben oft einen hohen ¹Sense of Coherenceª (Zusammengehörigkeitssinn). Um auf der einen Seite die Komplexität des ¹Krankseinsª zu verstehen und auf der anderen Seite die zahlreichen beitragenden Faktoren zu integrieren, könnte man die System Theorie benutzen: Meistens handelt es sich um offene Systeme, die einander mit Rückkopplungen beeinflussen (z. B. das bio-psycho-soziale Modell). Klinisch kann diese Integration in die Praxis getragen werden, dadurch, dass man sowohl der Krankheitsgeschichte, wie auch der Lebensgeschichte und der gegenwärtigen Lebenssituation des Patienten Aufmerksamkeit schenkt. Eine Balint-Gruppe ist, u. a. ein ¹Behälterª der Narrativen von ¾rzten, die schwierige Beziehungen mit psychosomatischen Patienten in der Gruppe mit Kollegen teilen können. AbstractThe author describes the development of his theoretical approach to psychosomatic medicine. His narrative starts with the basic psychoanalytic theory of ªpsychosomatic diseasesº, that pretended that a specific unconscious conflict may cause a specific internal disease. This hypothesis, being not scientific enough, was left later. Instead of it, less reductionistic and less liniar models were developed, as : multicausality, including psychosocial factors. Lasting stress, with which an individual cannot cope in an appropriate way, may be an important pre curser of diseases. Those who are able to cope successfully with stress, often have a high ªSense of Coherenceº. To understand the complexity of ªbeing illº on one hand, and to try to integrate many of the contributing factors, on the other hand, is possible when one makes use of the theory of systems: Mostly open systems that have mutual feedback mechanisms. (e. g. the bio-psycho-social model). Clinically this integration can be practiced, by paying attention to the illness narrative and the personal life story and life situations of patients. A Balint group is, among others, a container of doctor's narratives where the relationship with difficult psychosomatic patients can be shared.
