Secondary osteoporosis is a frequent complication of rheumatoid arthritis (RA) and the result of an imbalance of catabolic and anabolic mechanisms of bone metabolism. The effects of serial low-dose radon and hyperthermia (LDRnHT) exposure in a therapeutic adit (12 applications in 3 weeks) on the serum levels of the cytokines osteoprotegerin (OPG), receptor activator of NF kappa-B ligand (RANKL), tumor necrosis factor-α (TNF-α), and also on the RANKL/OPG ratio were investigated in 25 RA patients and an age-matched control of 24 patients with osteoarthritis (OA). Cytokine measurements were performed at baseline and after completion of LDRnHT. Anti-CCP antibodies (ACPA) were measured in RA patients in parallel. Medication in both groups was limited to non-steroidal anti-inflammatory drugs, and low-dose prednisolone (16 of 24 RA patients) as needed. RA and OA patients showed a significant decrease of TNF-α levels (p < 0.001). Both groups showed significantly decreased levels of RANKL (RA: p < 0.001, OA: p < 0.01). Only the RA patients presented a significant increase of OPG (p < 0.01) and decrease of the RANKL/OPG ratio (p < 0.01), and the ACPA levels (p < 0.001). LDRnHT results in a reduction of osteocatabolic and an increase of osteoanabolic cytokines, which represents the molecular basis for inhibiting osteoclastic activity in secondary osteoporosis and explains in part the effect of LDRnHT this physical therapy modality in a key inflammatory disease. Although reduced ACPA levels were observed under the therapy and although this could potentially contribute to an osteoprotective effect, in this case, it is rather uncertain as the reduction was only minor in magnitude.
Zusammenfassung Hintergrund: Im Rahmen entz?ndlich-rheumatischer Systemerkrankungen kommt es geh?uft zu einer Abnahme der Knochendichte mit Entwicklung einer sekund?ren Osteoporose, die durch Ver?nderungen im Gleichgewicht der ?osteoanabolen und -katabolen Stoffwechselprozesse hervorgerufen wird. Hierf?r sind unterschiedliche Effekte auf den Knochenmetabolismus verantwortlich, die jedoch durch Physikotherapeutika wirkungsvoll beeinflusst werden k?nnen. Die Effekte einer seriellen Radonstollen-Hyperthermie-Behandlung auf den Schmerz, die Krankheitsaktivit?t sowie auf molekulare Marker des Knochenmetabolismus stehen bei Patienten mit ankylosierender Spondylitis daher im Fokus vorliegender Betrachtung. Patienten und Methoden: Bei 24 Patienten mit gesicherter ankylosierender Spondylitis (AS) im chronischen Stadium (?-Alter 58 Jahre) und einer altersvergleichbaren Gruppe mit Arthrose (OA, n=24), wurde der Einfluss einer seriellen Radonstollen-Hyperthermie (12 Anwendungen in 3 Wochen) auf folgende Outcomeparameter untersucht: Schmerzen (VAS), Aktivit?tskriterien (BASDAI, BAS-G), Osteoprotegerin (OPG), receptor activator of NF kappa-B ligand (RANKL) und Tumornekrosefaktor(TNF)-alpha. Ergebnisse: Nach serieller Radonstollen-Hyperthermie zeigte sich bei AS-Patienten eine signifikante Schmerzabnahme (p<0,005), die auch 3 Monate nach Behandlungsende nachweisbar war (p<0,005), w?hrend die OA-Patienten erst nach 3 Monaten eine signifikante Schmerzverbesserung zeigten (p<0,005). Die Krankheitsaktivit?t der AS-Gruppe (BASDAI, BAS-G) war nach Therapieende deutlich gemindert. Bei den AS-Patienten war ein tendenzieller, bei den OA-Patienten ein signifikanter Abfall der TNF-alpha Spiegel (p<0,0005) nach Therapie objektivierbar. In beiden Gruppen kam es zu einem signifikanten Abfall von RANKL (AS: p<0,0005; OA: p<0,004). Nur in der AS-Gruppe zeigte sich ein signifikanter Anstieg des OPG (p<0,0005) und ein signifikanter R?ckgang des RANKL/OPG-Quotienten (p<0,005). Schlussfolgerung: Die serielle Radonstollen-Hyperthermie bewirkte bei Patienten mit chronischer AS eine mittelfristig anhaltende Schmerzlinderung, einen R?ckgang der Krankheitsaktivit?t, einen Anstieg der knochenanabolen und ein Abnahme der knochenkatabolen Zytokine, was einer Verminderung der osteoklast?ren Aktivit?t im Rahmen der entz?ndlich bedingten, sekund?ren Osteoporose entspricht. Der seriellen Radonstollen-Hyperthermie kommt somit ein wichtiger Stellenwert im multimodalen Therapiekonzept der AS zu.
Objective. To compare power Doppler sonography (PDS) findings inside the bicipital tendon sheath in patients with rheumatoid arthritis (RA) and degenerative disorders of the shoulder, in order to evaluate the diagnostic value of PDS in distinguishing between inflammatory and noninflammatory shoulder pain.Methods. The glenohumeral joints of 41 consecutive patients with shoulder pain were examined by ultrasound. Using ventral transverse and longitudinal scanning, the vascularity near and/or inside the bicipital tendon sheath was visualized by PDS. One fully trained and experienced examiner performed the sonography. Representative images were digitally stored and were read, under blinded conditions, by 2 independent investigators, who categorized the Doppler signals as being either inside or outside the tendon sheath.Results. Biceps tendon sheath effusion, represented by the typical hypoechoic rim, was found in 95.8% of the RA patients (23 of 24) and in 58.8% of the patients with degenerative disorders (10 of 17). PDS signals were localized to inside the tendon sheath in 22 of the RA patients (91.7%) and in none of the patients with degenerative disorders. Although no PDS signal was found inside the tendon sheath in patients with degenerative disorders, in 9 of these patients (52.9%), signals could be localized to the environment of the tendon sheath.

Loss of bone mass in AS is the result of an imbalance of catabolic and anabolic mechanisms of bone metabolism in response to diff erent factors. Question/Method: The eff ects of serial lowdose radon and hyperthermia exposure in a therapeutic adit (12 visits to the adit in 3 weeks) on the serum levels of the cytokines osteoprotegerin (OPG), receptor activator of NF kappa-B ligand (RANKL), tumor necrosis factor-alpha (TNF-alpha), and also on the RANKL/OPG ratio as a marker of bone metabolism were investigated in 24 patients with chronic AS (mean age 58 years) and an age-matched group of 24 patients with osteoarthritis (OA). Cytokine measurements were performed at baseline and after completion of the serial treatment. Medication in both groups was limited to non-steroidal antiinfl ammatory drugs (NSAIDs) as needed, whereas treatment with TNF inhibitors or any drug, which could potentially infl uence bone metabolism, was excluded. Results: AS-patients showed a tendency towards decrease of TNF-alpha serum levels whereas OA-patients achieved a signifi cant decrease (p < 0.0005). Both groups showed signifi cantly decreased levels of RANKL (AS: p < 0.0005, OA: p < 0.004). In contrast, only the AS-patients presented a signifi cant increase of OPG (p < 0.0005) and a signifi cant decrease of the RANKL/OPG ratio (p < 0.005). Conclusions: In patients with chronic AS, serial low-dose radon and hyperthermia exposure in a therapeutic adit results in a reduction of osteocatabolic and an increase of osteoanabolic cytokine levels, which represents the molecular basis for inhibiting osteoclastic activity in secondary osteoporosis of infl ammatory disease. Thus, serial radon(adit) therapy has an important role in a multimodaly treatment strategy for AS.
Zusammenfassung Fragestellung Nach klinischen Beobachtungen profitieren Patienten mit einer rheumatoiden Arthritis (RA) ?berdurchschnittlich gut von einer seriellen Radonstollen-Hyperthermie (sRSTH), ohne dass die Gr?nde hierf?r genau bekannt sind. Material und Methodik Bei Patienten mit gesicherter RA (n=25, Altersdurchschnitt 58 Jahre) und einer altersvergleichbaren Patientengruppe mit Osteoarthrose (OA, n=24) wurde der Einfluss einer sRSTH (12 Anwendungen in 3 Wochen) auf die Schmerzen (visuelle Analogskala ? VAS), den Schmerzmittelbedarf und die Funktionskapazit?t (Funktionsfragebogen Hannover ? FFbH) prospektiv untersucht. Ferner wurden die Serumspiegel der Zytokine Tumornekrosefaktor ? (TNF-?), Interleukin(IL)-6, IL-10, receptor activator of NF kappa-B ligand (RANKL), Osteoprotegerin (OPG) und der RANKL/OPG-Quotient (Marker zur Quantifizierung des Knochenmetabolismus) analysiert. Bei RA-Patienten erfolgte zudem eine Analyse der anti-CCP-Antik?rper (ACPA). Die Messungen erfolgten vor und nach sRSTH, die Parameter VAS, Schmerzmittelbedarf und FFbH wurden auch noch nach 3 Monaten evaluiert. Eine Biologika-Therapie sowie Medikation mit m?glichem Einfluss auf den Knochenstoffwechsel bestand nicht. Ergebnisse Die RA-Patienten zeigten direkt nach der sRSTH eine signifikante Schmerzlinderung (p<0,001), anhaltend bis zu 3 Monaten (p<0,001). Die OA-Patienten hatten erst nach 3 Monaten eine signifikante Schmerzlinderung (p<0,01). Die RA-Gruppe zeigte zudem eine Minderung der Schmerzmedikation sowie eine signifikante Besserung im FFbH direkt nach der Therapie als auch nach 3 Monaten (p jeweils<0,001). In beiden Gruppen zeigte sich ein signifikanter Abfall der TNF-? Spiegel (RA: p<0,001; OA: p<0,001). Die IL-6 Spiegel blieben ohne erkennbare ?nderungen, bei den RA-Patienten waren beim IL-10 eine signifikante Zunahme (p<0,001), beim RANKL/OPG-Quotienten ein signifikanter Abfall (p<0,01) und bei den ACPA ebenso ein signifikanter Abfall (p<0,001) objektivierbar. Diskussion Die sRSTH bewirkte bei RA mittelfristig eine signifikante Schmerzlinderung und Abnahme der Schmerzmedikation sowie signifikante Zunahme der Funktionskapazit?t. Ferner war ein Anstieg der knochenanabolen und ein R?ckgang der knochenkatabolen Zytokine, neben einem R?ckgang der ACPA objektivierbar, was der molekularen Grundlage f?r eine Verminderung des osteoklast?ren Knochenabbaus im Rahmen der entz?ndlich bedingten sekund?ren Osteoporose entspricht. Schlussfolgerung Der Radonstollen-Hyperthermie kommt ein wichtiger Stellenwert im multimodalen Therapiekonzept der RA zu.
