ZusammenfassungDer Excimer-Laser begann seinen Siegeszug Anfang der 80er-Jahre mit Anwendungen im Bereich der Kornealchirurgie in der Ophthalmologie und wird seit Ende der 90er-Jahre in der Dermatologie eingesetzt. Die Wellenlänge von 308 nm stellte sich als besonders viel versprechend dar, da bereits gute Erfahrungen mit Bestrahlungen mit 311 nm bei Psoriasis und Vitiligo bestehen. Die Erwartungen zeigten sich bei der Therapie mit dem Excimer-Laser gerechtfertigt und es lassen sich durchweg gute therapeutische Erfolge nachweisen. Nachteile der Therapie mit dem Excimer-Laser sind vor allem hohe Verbrauchskosten. Aus diesem Grund wurden Blitzlampen entwickelt, die deutlich geringere Verbrauchskosten haben und ein Emissionsspektrum im UVB-Bereich aufweisen. Die vorliegende Anwendungsbeobachtung stellt die Therapiemöglichkeiten der Indikationen Vitiligo und Psoriasis bei der Behandlung mit dem Excimer-Laser Talos und den Blitzlampen Theralight und B-Clear vor. Es wurdenPatienten mit den o. g. Erkrankungen, die sich in der Klinik mit der Frage nach alternativen Therapiemethoden und Möglichkei-ten vorstellten, in die Studie mit aufgenommen. Nach Ermittlung der MED wurde eine Behandlung mit steigender Energiedichte und Anfangsenergiedichte von 1,5-facher MED dreimal wö-chentlich durchgeführt. Mit dem Excimer-Laser wurden bei der Indikation Psoriasis durchschnittlich 8,5 Wiederholungsbehandlungen durchgeführt, was zu einer kompletten Abheilung bei 33 %, zu einer deutlichen Besserung bei 50 % der Probanden und nur bei 17 % zu keiner ¾nderung der Hautveränderungen führte. Die Anzahl der Wiederholungsbehandlungen war mit 15,7 bei der Vitiligo deutlich höher.
