Two cases of cystadenocarcinoma (CAC) of the pancreas are reported. In the first case, mucinous CAC of the head of the pancreas was diagnosed 16 years after removal by left pancreatectomy of a benign papillomatous cystadenoma. In the second case, malignancy was diagnosed 32 years after cystojejunal diversion for a benign mucinous cyst. The possibility of late recurrence or malignant transformation justify life-long clinical, sonographic or tomographic follow-up after pancreatic surgery for these disorders.
