To treat genuine stress urinary incontinence, the surgical technique of choice at present is suburethral tension-free vaginal tape (TVT) procedure. Because of its good results and low morbidity, TVT is now offered to younger women still of childbearing age. We describe a patient who delivered vaginally 2 years after undergoing a retropubic TVT procedure. The woman remained continent throughout her pregnancy and at 6 months postpartum, with normal urodynamic parameters.
L'incontinence urinaire, notamment à l'effort, est un problème fréquent, quel que soit l'âge des patientes, notamment pour certaines patientes toujours en âge de procréer. Un certain nombre de ces patientes vont bénéficier d'une cure chirurgicale d'incontinence urinaire à l'effort (IUE) avant d'avoir terminé leur désir de maternité. Depuis environ dix ans, la chirurgie « mini-invasive » de l'IUE est devenue très populaire du fait de sa faible morbidité et surtout de ses très bons résultats estimés à 85 % à long terme. Ainsi, le nombre de patientes toujours en âge de procréer et ayant eu ce type de chirurgie est en augmentation. Dans la littérature internationale, on répertorie peu de cas et peu d'études concernant la conduite à tenir sur le mode d'accouchement. Le but de cet article est de colliger les différents cas publiés afin de décrire et de discuter ce qui est couramment réalisé dans les différents services.Mots clés : Grossesse, accouchement -Césarienne -Incontinence urinaire à l'effort -Soutènement sousurétral -TVT -TOT Pregnancy and delivery after stress urinary incontinence surgery Abstract: Regardless of age, urinary incontinence, especially stress urinary incontinence (SUI), is a frequently occurring disorder, particularly significant for certain patients of childbearing age. Some of these patients undergo surgery to cure SUI prior to deciding to have children. For approximately ten years, mini-invasive surgery for SUI has become very popular because of its low morbidity and, most notably, its very good outcome in 85% of cases at long-term follow-up. The number of patients of childbearing age who undergo this type of surgery is on the rise. The literature reports few cases and there are few studies concerning the mode of delivery recommended after SUI surgery. This article brings together various published case reports and describes and discusses the policies of various medical facilities.
