Background: Chronic obstructive pulmonary disease (COPD) has become a huge public health problem in the world. In Asy-Syaafi Hospital, COPD is the most commonly found disease after bcronchitis among the patients in out patient unit of lung desease. Self efficacy showed patients’ confidence in independently managing chronic desease. Wheter they want or not to start the treatment is determined by their self efficacy. Peak expiratory flow rate showed condition and problems of lung function and the narrowing or blockage of the airway. Objective: Identify conditions and problems on self efficacy and Peak expiratory flow rate of COPD patients in Asy-Syaafi Hospital Pamekasan, East Java. Method: This research was descriptive study used non analytic cross sectional design, with total sampling, involving 30 respondents. Data were collected with a questionnaire of the COPD self efficacy (CSES) and peak flow meter. Data analysis was performed with a univariate analysis. Results: Self efficacy was in low category with score less than 99 (86.7%) and peak expiratory flow rate was less than <50% of the PEF (90%). Conclusion: Most of COPD self efficacy in Asy-Syaafi Hospital Pamekasan were in not good category and peak expiratory flow rate contained in red zone or the occurrence of major constriction of the airways. Gencarnya gaya hidup back to nature berdampak pada perilaku masyarakat dalam konsumsi tanaman obat baik yang masih alam maupun yang sudah dalam bentuk sediaan. Minimnya pengetahuan mengakibatkan penggunaan Tanaman Obat Keluarga (TOGA) yang kurang bijak, selain itu minimnya lahan terbatas juga membuat sulit untuk membudidayakan. Untuk itu pengabdi tergerak untuk meluruskan dari penyimpangan penggunaan TOGA dan mengajarkan tanaman tumbuh dalam pot (Tambulampot). Mitra yang digaet adalah Lembaga Lingkungan Hidup dan Penanggulangan Bencana Pimpinan Wilayah Aisyiyah Daerah Istimewa Yogyakarta (LLHPB PWA DIY) yang memiliki fokus terhadap permasalahan lingkungan, dengan menggerakkan potensi wanita. LLHPB memiliki 5 kader Aisyiyah di tiap kabupaten sehingga harapannya bisa ditularkan di tingkat ranting. Kegiatan ini dilaksanakan pada tanggal 15 Maret 2020 di Kampus Terpadu Universitas Muhammadiyah Yogyakarta dihadiri oleh 35 peserta dari LLHPB. Pada kegiatan ini juga menyosialisasikan Modul SALING TOGA di mana tiap peserta diukur dari pretes dan postes untuk evaluasi tingkat pemahaman. Pada kesempatan ini tiap daerah/POS diberikan paket TOGA sesuai penyakit yang marak dikalangan masyarakat diantaranya gangguan pada Nafas, Hormon, Cerna, Jantung dan Syaraf. Hasil program ini menunjukan adanya peningkatan pemahaman sebesar 13,7% tentang TOGA. Hal ini menunjukkan bahwa sosialisasi bermanfaat bagi peningkatan pengetahuan bagi para kader dan harapannnya bisa secara rutin.
BACKGROUND: Care transition after hospital discharge is challenging for patients and their families. For post an acute cardiac event patients, the 0–14 days period is a vulnerable period. After hospitalization, patients and their families report having unmet information needs at a time of hospital discharge. It can increase the complication and hospital readmission. AIM: This study aim to explore the patient’s transition experiences from hospital to home after an acute cardiac event. METHODS: An exploratory qualitative research design with phenomenological approach was used in the study. The data were collected from April to July 2020, using semi-structured interviews with 15 patients diagnosed with acute coronary syndrome (ACS). Transcribed interview data were thematically analyzed. RESULTS: There are five themes obtained from the study: Positive perception after an acute cardiac event, the changes experienced, the knowledge which was required, the support needs, and expectations and goals. CONCLUSION: Transition from hospital to home depends on the perceptions of the participants themselves. The experience of transitioning patients from hospital to home was influenced by the existence of support from family, spiritual meaning, and social environment to be able to adapt to health conditions. The finding of this study can assist the nurses in understanding better the needs of patients to prepare the care transition from the hospital to home.
Introduction: Patients with post hospitalized heart attack often feel insufficiently ready for the hospital to home transition. Problem focused on patients and health care providers can contribute to the significant residual risk seen in patients with acute coronary syndrome. Therefore, nurses could play an integral role in ensuring suitable care. The aim of this research was to investigate the role of nurses in patients with for acute coronary syndrome preparing for discharge. Method: An exploratory qualitative research design with phenomenology approach was used in the study. Data collection used semi-structured interviews with 15 nurses form 2 private hospitals in Yogyakarta. Data were collected from March to June 2020 with face to face interview and telephone call. Result: There are 5 themes of the role of nurses which are obtained from the result of the qualitative analysis consisting of: (1) Educator; (2) Care provider; (3) Motivator; (4) Collaborator; (5) Communicators. Conclusion: The roles of nurses in the care of patients with post acute coronary syndrome are as an educator, care provider, motivator, collaborator and communicator for both ACS patients and their families.
