BackgroundGastroesophageal reflux disease (GERD) is the most common cause of children admissions to pediatric gastroenterology unit and affects about 30% of pediatric population. Body weight and height percentiles of children with GERD and their relationship between presence and the severity of reflux on scintigraphic images were studied.MethodsPatients who underwent reflux scintigraphy between 2005 - 2012 were retrospectively reviewed. Among 200 patients, 49 patients were involved that their ages were ranging from 0 to 18 years old and body weight and height percentiles were recorded. Accurately 37 MBq (1 mCi) 99mTc-MAA in 100 - 150 mL of milk was ingested by the patient. Presence, number, duration and level of reflux were evaluated on the dynamic images. Presences of reflux within last ten minute were also recorded. Region-of-interests were drawn on esophagus and stomach and reflux ratio (RR) was calculated.ResultsThe ratio of the presence of reflux which occurred within the last ten minutes was significantly higher in children with low body weight percentile. High-level reflux frequency was higher in these children than in normal’s. Presence of reflux which occurred within the last ten minutes was related with low body weight percentile.ConclusionsIf reflux is shown within the last ten minutes and there is high level of reflux, the clinician should be warned about possible low body weight percentile in the future and scintigraphic study should be a guide or a reference for the assessment of more effective treatment methods.
Yeni koronavirüs hastalığı (COVID-19), insanlarda solunum yolu enfeksiyonu, pnömoni, böbrek yetmezliği, kalp krizi hatta çoklu organ yetmezliği gibi ciddi sağlık sorunlarına yol açmaktadır. COVID-19 ve neden olduğu enfeksiyonun fiziksel sağlık üzerindeki etkilerinin yanında psikolojik sağlığı da önemli ölçüde etkilemektedir. COVID-19 dünya genelinde birçok insanda; panik bozukluk, anksiyete bozukluğu, keder, kayıp ve depresyon gibi çok çeşitli psikolojik sorunlarında oluşumunu tetiklemektedir. Salgında büyük ölçüde sorumluluk üstlenen sağlık profesyonellerinin ruhsal sağlığı da bu durumdan etkilenmektedir. Bu çalışmanın amacı, salgın sürecinde, bir eğitim ve araştırma hastanesinde görev yapan hemşirelerde, COVID-19 ile ilişkili psikolojik yansımaların ve kaygı düzeylerinin belirlenmesidir. Tanımlayıcı tipteki araştırmanın verileri çevrimiçi anket yöntemiyle elde edildi. Araştırmanın örneklemini bir üniversite hastanesinde salgın sürecinde görev yapan hemşireler oluşturdu (n=110). Veri toplamada tanımlayıcı bilgiler formu ve Corona Virüs Fobisi Ölçeği (C19P-S) kullanıldı. Hemşirelerin salgın sürecindeki COVID-19 Fobilerinin orta düzeyde olduğu, (ölçek ortalaması=57,55), salgın döneminde hastanede çalışmanın kendilerini ve ailelerini yüksek oranda endişelendirdiği görüldü. Çalışmada, cinsiyet, medeni durum, kronik hastalık tanısı olma, çalışılan birim ile C19P-S ölçek ortalaması arasında anlamlı farklılıklar bulunmamıştır. Bu süreçte çalışma koşullarının iyileştirilmesi, kişisel koruyucu ekipman temininin yapılması, stres ve kaygı düzeylerinin azaltılmasına yönelik destekleyici çalışma planlarının oluşturulması ve psikolojik danışmanlık sağlanması önerilmektedir.
ÖzetTeknoloji hayatın diğer alanlarını olduğu gibi eğitimi sistemlerini de etkisi altına almıştır. Yeni neslin öğrenme ihtiyaçları, değişen koşullar ve gelişen teknoloji hemşirelik eğitiminde yenilikçi yaklaşımları zorunlu kılmaktadır. Bu süreç hemşire eğitimcileri öğrenci kitlesine uygun ve verimli teknikleri araştırmaya teşvik ederken, öğrencilerin de aktif olarak eğitim faaliyetine katılmasını sağlar. Teknolojideki gelişmeler hız kesmeden sürerken eğitim sistemleri bu gelişmelere istendik ölçüde entegre olamamakta, çoğu eğitim sistemi klasik öğrenme yöntemlerini kullanmaya devam etmektedir. COVID-19 salgını eğitimcileri ve öğrencileri hızla yeni çözümler üretmeye teşvik etmiştir. Bu zor süreçte eğitim büyük oranda mobil cihazlardan ve çevrimiçi platformlardan sürdürülmüştür. Salgın dönemi neden olduğu zorluklarla birlikte eğitimde teknolojinin entegrasyonunu hızlandırmıştır. Bu makalede teknolojinin hemşirelik eğitimindeki etkileri ve teknolojiye dayalı öğrenme yöntemleri üzerinde durulacaktır.
Derin ven trombozu (DVT) cerrahi süreçte sıklıkla ortaya çıkan bir sorundur. Özellikle büyük cerrahi ameliyatlar sırasında koagülasyon sisteminin bozulması, derin venlerin travması ve ameliyat sonrası uzun süren yatak istirahati gibi nedenler DVT riskinin artmasında etkili olmaktadır. Cerrahi kliniklerindeki hastaların DVT riskinin belirlenmesi cerrahi hemşireleri açısından önemli konulardan biridir. Bu çalışma, genel cerrahi kliniğinde yatan hastaların ameliyat sonrası DVT riskinin ve profilaktik uygulamaların belirlenmesi amacıyla planlandı. Yöntem: Tanımlayıcı tipteki araştırma, etik kurul ve kurum izinlerinin alınmasının ardından, Mayıs-Ağustos 2021 tarihleri arasında bir eğitim ve araştırma hastanesinin genel cerrahi servisinde ameliyat olan 100 hasta ile gerçekleştirildi. Veriler, "Hasta Tanılama Formu" ve "Autar DVT Riski Tanılama Ölçeği" kullanılarak yüz yüze görüşme tekniği ile toplandı. Bulgular: DVT'yi önlemeye yönelik uygulamalar arasında; hastaların %69'u mobilize edilmiş, %35'ine dereceli kompresyon varis çorabı ve %80'ine heparin uygulaması yapılmıştı. Hastaların %12'sinde baldır ağrısı, %9'unda tek taraflı bacak ödemi, %11'inde çift taraflı bacak ödemi, %5'inde de bacakta hassasiyet mevcuttu. %4'ünde bacakta duyu kaybı, %3'ünde bacakta motor kayıp vardı. Hastaların DVT riski tanılama puan ortalaması 13,96±4,88 (min:5-max28)'di. Buna göre hastalar orta derece risk grubunda yer almaktaydı. Sonuç: Etkili DVT tanılamasının yapılabilmesi için ölçek kullanımının yaygınlaştırılması, cerrahi servislerde görev yapan hemşirelerin konuyla ilgili bilgilerinin güncel tutulması ve hastalara uygun profilaksi yöntemlerinin geliştirilmesi önerilebilir.
Amaç: Bu çalışma, kronik yarası olan bireylerin yara tanılama süreçlerinin belirlenmesi amacıyla gerçekleştirildi. Yöntem: Araştırma etik kurul ve kurum izinleri alındıktan sonra bir eğitim araştırma hastanesinin Yara Bakım Polikliniği’nde yürütüldü. Araştırmanın örneklemini 1 Ağustos 2021-31 Aralık 2021 tarihleri arasında polikliniğe başvuran 158 hasta oluşturdu. Araştırma verilerinin toplanmasında tanıtıcı bilgiler formu ile yara tanılama formu kullanıldı. Verilerin değerlendirilmesinde tanımlayıcı istatistiklerden yararlanıldı. Bulgular: Çalışmaya katılan hastaların %72,8’i erkek cinsiyette olup, %51,9’u 46-65 yaş aralığındaydı. En sık başvuru sebebi diyabetik ayak yaralarıydı (%67,7); bunun dışında venöz ülser, arteryel ülser, basınç yarası, travmatik yara, yanık ve cerrahi yara komplikasyonu hastaların tanıları arasındaydı. Yaraların özellikleri incelendiğinde yaranın çoğunlukla alt ekstremitede bulunduğu tespit edildi. Yara çevresinin bakımında bariyer krem ve sprey kullanıldığı, yara irigasyonu için ticari solüsyonların daha çok tercih edildiği, debridman yöntemlerinden en çok cerrahi debridmanın uygulandığı, pansuman için gümüş içerikli pansumanın daha çok kullanıldığı görüldü. Sonuç: İyileşmesi geciken veya sorunlu şekilde iyileşme gösteren yaralar sağlık profesyonelleri ve hastalar için büyük bir problem olamaya devam etmektedir. Yara tedavisi ve bakımıyla ilgili uygulamaların geliştirilmesi için yarası olan hastaların özelliklerinin iyi bilinmesi, bu hastalara özel bakım ve tedavi stratejileri geliştirilmesi önem taşımaktadır.
