Bu çalışma Ankara’da bulunan bir Aile Sağlığı Merkezi’ne başvuran beden kütle indeksi (BKİ) farklı 19-64 yaş aralığında ki 155 kadının beslenme durumunu ve yeme tutumunu değerlendirmek amacıyla yapılmıştır. Kadınların genel özellikleri, uyku süreleri sorgulanmış, antropometrik ölçümleri alınmıştır. Ayrıca bireylerin yeme tutum ve davranışlarını değerlendirmek için Yeme Tutum Testi–40 (YTT-40) skoru hesaplanmıştır. Her BKİ sınıflamasından homojen sayıda birey alınarak, sonuçlar 3 gruba (zayıf ve normal, fazla kilolu ve obez) göre incelenmiştir. Zayıf ve normal BKİ grubundaki bireylerin %61.6’sı 19-34 yaş aralığındayken, fazla kilolu grubundaki bireylerin %64.7’si 45-64 yaş aralığında, obez bireylerin ise %77.0’ı 45-64 yaş aralığındadır (p<0.05). Eğitim süresi ortalaması zayıf ve normal BKİ’ndeki bireylerin 13.7±3.36 yıl, fazla kilolu bireylerin 11.5±4.31 yıl ve obez bireylerin ise 8.0±3.85 yıl olarak bulunmuştur (p<0.05). Üzüntü, öfke ve stres anlarında; obez bireylerde iştah durumunda artma, diğer gruplardaki bireylere göre anlamlı bir şekilde daha fazla görülmüştür (p<0.05). Yeme tutum testi skoru ortalaması zayıf ve normal bireylerde 16.8±8.59, hafif şişman bireylerde 19.7±8.35, obez bireylerde 22.8±8.8 puan olarak bulunmuştur. Obez bireylerde anormal yeme davranışı diğer gruplara göre daha fazla görülebileceğinden, içinde doktor, diyetisyen ve psikoloğun da bulunacağı interdisipliner bir yaklaşım modeli örnek alınarak, bireylerin sağlıklı yaşam tarzı alışkanlıkları kazanmasına yardımcı olunmalıdır.
Irritable bowel syndrome (IBS) is one of the most common chronic functional disorders among gastrointestinal system diseases. IBS, which has a high prevalence worldwide, negatively affects the quality of life of patients. The mechanisms that are effective in the development of IBS has not clearly been defined. However, its mechanism is thought to be multifactorial. IBS is characterized by certain symptoms, such as abdominal pain, diarrhea, constipation, flatulence, and indigestion. Nevertheless, these symptoms aren’t specific to IBS and can often be confused with lactose intolerance (LI) symptoms. On the other hand, the probability of having LI at the same time is quite high in IBS patients. IBS patients restrict lactose-containing foods, especially milk and dairy products, due to the thought that they trigger their symptoms, and thus calcium deficiency may occur as a result. Due to insufficient calcium intake for a long time, the risk of osteoporosis and osteoporotic fractures becomes an important problem. In line with all these factors, this review aims to examine osteoporosis that may develop due to the presence of LI in IBS patients and to make nutritional recommendations.
Binge Eating Disorder is an eating disorder characterized by binge-eating episodes in which the person loses control, recurring at least once a week for the last 3 months without compensatory behaviors. Conditions such as poor self-perception, depression, having to follow a certain diet, and increased body weight create a bidirectional relationship between Type 2 Diabetes Mellitus and Binge Eating Disorder. It is thought that the coexistence of the two disorders may make it difficult to achieve metabolic control in people with diabetes. Although there are conflicting results in the literature on this subject, it is widely believed that body weight is a moderator in the relationship between glycemic control and eating disorders. In studies conducted on the prevalence, it was determined that between 0.1% and 25.6% of Type 2 diabetes patients have Binge Eating Disorder. This wide range in prevalence is thought to be due to the differences in the characteristics of the participants in the studies and the screening tools used. There is no gold standard screening test developed for the diagnosis of Binge Eating Disorder in people with type 2 diabetes. However, some researchers have stated that the Eating Disorder Examination-Questionnaire and the Questionnaire of Eating and Weight Patterns are useful methods for screening. In this review, the relationship between Binge Eating Disorder and Type 2 Diabetes Mellitus, their incidence, metabolic effects and diagnostic tools were examined.
Öz Beslenme bireylerin hayatta kalması için gerekli bir eylem olmakla birlikte, çeşitli duyu organlarımıza hitap eden keyif verici bir durumdur. Ancak duyularımıza hitap eden lezzetli besinlerin sürekli tüketimi bazı bireylerde bağımlılık benzeri durumlar oluşturabilmektedir. Besin bağımlılığı; bireyin düzenli olarak tükettiği bir veya daha fazla besine duyduğu spesifik bir adaptasyon olarak tanımlanan bir kavramdır. Çeşitli psikolojik, sosyal problemlerin dışında bazı hormonal dengesizlikler, beyin yapılarındaki anomaliler ve çeşitli ilaçların kullanımından kaynaklanan yan etkiler besin bağımlılığının patofizyolojisinin oluşturabilmektedir. Besin bağımlılığı üzerine yapılan klinik çalışmalar artmakta, ancak henüz uluslararası otoriteler tarafından resmi olarak bağımlılık türü olarak tanımlanmaması nedeniyle belirli bir tedavi metodu bulunmamaktadır. Genel olarak obez bireylerde uygulanan tedavi metotlarına ek olarak obezitenin altında yatan patolojik durumlar tespit edilmekte ve buna uygun multidisipliner tedavi stratejileri geliştirilmektedir. En sık kullanılan tedavi stratejileri; tıbbi beslenme tedavileri, psikoterapiler, farmakolojik tedaviler ve nöromodülasyon tedavileridir. Bu derleme çalışmada; besin bağımlılığı ve obezite arasındaki ilişkinin güncel literatür eşliğinde incelenerek tartışılması amaçlanmıştır.
