Based on our clinical experience of using small-bore catheters, we believe that they are potentially effective for almost all pleural pathologies. Our results concur with those in the literature, but this series also includes different pleural diseases requiring chest tube insertion.
Yabancı cisim aspirasyonları (YCA) çocukluk döneminde daha çok görülmekle birlikte erişkin yaşta da karşılaşılabilen acillerdendir. Hayatı tehdit eden komplikasyonlara yol açabilirler. Asemptomatik olabileceği gibi, çok ciddi solunum yolu komplikasyonlarına hatta boğulma ve ölüme neden olabilmektedirler. Kazara yaralanmalar arasında ölüm nedenlerinde 4. sıradadır. Amerika'da yapılan çalışmalarda 4 yaş altındaki her 100000 popülasyonda 0.5 ölüm nedeni aspirasyondur (1). Ülkemizde bu tür istatistiki bilgilere ulaşmak zordur. Ancak yapılan bebek ölümlerinin araştırıldığı 378 bebek otopsisinde anoksi en sık ölüm nedeni olarak saptanmış, bunların içinde ikinci sırada yabancı cisim aspirasyonlarının görüldüğü bildirilmiştir (2). Hastanemiz bölgenin referans hastanesi olduğundan çocuk acil servisine başvuran yılda ortalama 40-50 hastaya yabancı cisim aspirasyonu tanısı konulmaktadır. Her zaman çok yönlendirici bir hikaye söz konusu olmadığından yabancı cisim aspirasyonun klinik ve radyolojik bulgularının çok iyi bilinmesi tanının erken dönemde konulması açısından oldukça önemlidir. Bronkoskopideki gelişmelere paralel olarak tedavilerinde çok büyük ilerleme olmuş ve mortalite oranları dramatik olarak düşmüştür. EpidemiyolojiTrakeobronşiyal yabancı cisimler daha çok çocukluk çağının sık görülen morbidite ve mortalite nedenlerindendir. Yirminci yüzyılda bronkoskopi işleminin geliştirilmesi öncesinde oldukça yüksek olan mortalite oranları bronkoskopi ile birlikte oldukça düşmüştür. Bronkoskopi YCA'ın tedavisinde bir milat oluşturmuştur.Özellikle ilk 3 yaş YCA açısından oldukça risklidir. Çocuğun kendi başına hareket edebilmesi, çevreye ve objelere artan ilgi, nöromuskuler mekanizmaların yeterince gelişmemiş olması, objeleri tanımak için ellerine aldıkları objeleri ağızlarına götürmeleri (el-ağız ilişkisi), molar dişlerinin olmaması, çiğneme işleminin efektif yapılamaması, doğru yiyecek maddelerinin seçilememesi ve ailede daha büyük çocukların daha küçüklerle ilgileniyor olması nedeniyle ilk 3 yaşta görülme oranları fazladır. ÖzetYabancı cisim aspirasyonları ve özefageyal yabancı cisimler özellikle çocukluk çağında hayatı tehdit eden acillerdendir. Hastalar çok farklı klinik gösterebilirler. Zamanında tanınmaları ve tedavi edilmeleri durumunda ciddi komplikasyonlardan kaçınılmış olur. Bunlar oldukça riskli klinik durumlar olmakla birlikte çıkartılmaları için yapılan işlemler de risklidir. Çocukluk çağının bu grup acillerinin büyük bir bölümü önlenebilir. Bu konuda aile eğitimi oldukça önemlidir.
BACKGROUND: Tracheal stenosis constitutes one of the most frequently seen problems in thoracic surgery. Although many treatment modalities to prevent fibroblast proliferation, angiogenesis, or inflammation that causes tracheal stenosis have been attempted, an effective method has not yet been found. In this study, a transforming growth factor beta3 (TGF-3)/chitosan combination was used for this purpose. METHODS: A slow-release preparation containing a thin layer of TGF-3 with a chitosan base was made. Thirty albino Wistar rats were divided into 3 groups. A full-layer vertical incision was made in the anterior side of the trachea of each rat between the second and fifth tracheal rings. The tracheal incision was sutured. Group A was evaluated as the control group. In Group B, a chitosan-based film was placed on the incision line. In Group C, a slow-release TGF-3/chitosan-coated substance was placed on the incision line. The rats were killed on day 30, and their tracheas were excised by cutting between the lower edge of the thyroid cartilage and the upper edge of the sixth tracheal ring together with the esophagus. Epithelialization, fibroblast proliferation, angiogenesis, inflammation, and collagen levels were evaluated histopathologically by the same histopathologist. RESULTS: Statistically significant differences were not found among the 3 groups. Cold abscesses were observed at the incision sites in both the TGF-/chitosan and chitosan groups. These were thought to have formed due to the chitosan. CONCLUSIONS: As this was the first experiment in the literature to use this type of TGF-3 formulation, we intend to change the formulation and perform this study again with a different TGF-3/chitosan preparation.
The majority of foreign-body aspirations are seen in children. In adults, neurological dysfunction, trauma, alcohol abuse, or psychological disorders can lead to aspiration, but normal adults can also suffer foreign body aspiration. The symptoms include acute asphyxiation, with or without complete airway obstruction, cough, dyspnea, choking, and fever, which also occur in many other medical conditions. Bronchoscopic removal of the foreign body is necessary, and flexible bronchoscopy is effective in the diagnosis and removal. We saw a patient with COPD who aspirated a plastic cigarette filter while using his bronchodilator inhaler.
Bu çalışmada deneysel sıçan modeli üzerinde uygulanan künt akciğer travmasının akciğer dokusu ve kandaki etkileri ile Ngammanitro-L-arginin metil ester ve N-iminoetil-L-ornitin uygulamasını takiben proenflamatuvar sitokinler, oksidan-antioksidan enzimler ve histopatolojik parametreler araştırıldı. Ça lış mapla nı:Çalışmaya 50 erişkin erkek Wistar albino sıçan (ağırlık 350-400 g) dahil edildi. Sıçanlar rastgele dört gruba ayrıldı. Kontrol dışındaki diğer tüm gruplarda orta şiddette pulmoner kontüzyon oluşturuldu. Grup 1 ve 2'ye intraperitoneal salin solüsyonu, grup 3'e 25 mg.kg-1 Ngamma-nitro-L-arginin metil ester, grup 4'e 20 mg.kg-1 N-iminoetil-L-ornitin uygulandı. Kan ve akciğer dokuları biyokimyasal ve histopatolojik açıdan incelendi. Bul gu lar: Malondialdehit yanıtı, müköz ve histopatolojik değerler incelendiğinde, en iyi sonuçlar istatistiksel olarak anlamlı şekilde Ngamma-nitro-L-arginin metil ester ve N-iminoetil-L-ornitin uygulanan gruplarda kaydedildi. Yarışmalı nitrik oksit sentaz inhibitorü Ngamma-nitro-L-arginin metil ester uygulanan grupta süperoksit dismutaz değerlerinde anlamlı iyileşme saptandı. Nitrik oksit değerleri N-iminoetil-L-ornitin grubunda oldukça düşük idi ancak anlamlılık saptanmadı. So nuç: Künt akciğer travması sonrası pulmoner kontüzyonda serbest oksijen radikalleri ve lipid peroksidasyon rol oynadı. Biyokimyasal ve histopatolojik sonuçlara göre, enflamasyon etkileri hem Ngamma-nitro-L-arginin metil ester hem N-iminoetil-L-ornitin uygulaması ile azaldı ve koruyucu etkiler oluştu. Background: Foreign body aspiration from tracheostomy is very rare, and materials related to tracheostomy are usually aspirated. This condition, which can lead to serious complications, can be treated using bronchoscopic procedures. In this study, we aimed to present our clinical experience in foreign body aspiration via tracheostomy. Methods: Data from 26 patients who presented to our hospital for foreign body aspiration via tracheostomy from 2006 to 2020 were analyzed retrospectively. Results: Foreign bodies were removed by fiber optic bronchoscopy in 15 (57.7%) cases, by rigid bronchoscopy in 9 (34.6%) cases and both methods were used in 2 (7.7%) cases. During bronchoscopy, local anesthetic procedures were used in 13 (50%) cases and general anesthesia was used in 11 (42.3%) cases. No anesthesia was used in two (7.7%) patients who underwent bronchoscopy under intensive care conditions. While the mean operative time for flexible bronchoscopy was 8.77±0.83 (CI: 26.03–29.43) minutes, the mean operative time for rigid bronchoscopy was 27.73±2.53 (CI: 26.03–29.43) minutes. Conclusion: Both rigid bronchoscopy and fiberoptic bronchoscopy (FOB) have advantages and disadvantages in foreign body removal. In our opinion, it is more reasonable to perform fiber optic bronchoscopy first in patients with a tracheostoma. In the light of our experiences, fiber optic bronchoscopy does not require general anesthesia and the operation time is shorter than rigid bronchoscopy. This feature makes fiber optic bronchoscopy advantageous.
