Актуальність. Стабільність і конгруентність є важливими передумовами ефективного функціонування ліктьового суглоба. Ліктьовий суглоб є одним з найбільш рухливих суглобів тіла і відіграє велику роль у точному позиціонуванні верхньої кінцівки у просторі й забезпеченні координованих рухів кисті. Ліктьовий суглоб дуже вразливий і схильний до посттравматичного й післяопераційного обмеження рухів. Показання до лікування контрактур або обмеження рухів у ліктьовому суглобі відносні й залежать від оцінки пацієнтами функціонального дефіциту. Будь-яке задане обмеження рухливості може мати різні наслідки для різних людей залежно від дискомфорту й бажаного рівня активності. У роботі розглянуто вплив низькочастотної вібрації і постізометричної релаксації на контрактуру ліктьового суглоба, що виникла внаслідок тривалої іммобілізації після хірургічного втручання, пов’язаного з позасуглобовими переломами плечової, променевої і ліктьової кісток, а також виростка плечової кістки. При всіх травмах не було ушкоджень тканин суглоба. Матеріали та методи. Проаналізовано результати вібротерапії двох груп пацієнтів. І група пацієнтів — термін обмеження рухливості суглоба від 21 до 45 діб. ІІ група пацієнтів — термін обмеження рухливості ліктьового суглоба від 90 до 180 діб. Пацієнтам проводили курс вібротерапії тривалістю 14–15 сеансів по 10 хв. Вимірювання обсягу рухів (розгинання/згинання) проводили до і після кожної процедури. Частота вібраційного впливу становила 20 Гц. Результати. Для розробки іммобілізаційних контрактур ліктьового суглоба поєднали метод низькочастотного локального вібраційного впливу з вправами з елементами постізометричної релаксації. Проведений аналіз даних вібротерапії показав, що пацієнти, у яких контрактури сформувалися після короткої (до 1,5 місяця) іммобілізації, краще відповідають на вібраційну розробку, приріст розгинання становить близько 50 % від початкового рівня, обсяг згинальних рухів збільшується до 30 %. У пацієнтів з контрактурами після тривалої іммобілізації понад 3 місяці розробка йде дуже повільно, досягнуті обсяги рухів дуже далекі від нормальних. Приріст згинання/розгинання в ліктьовому суглобі не перевищує 20 %. Висновки. Низькочастотна локальна вібраційна розробка рухів у ліктьовому суглобі й розтягувальні вправи з елементами постізометричної релаксації є прогресивним методом розробки іммобілізаційних контрактур, у тому числі застарілих. Дані процедури в більшості випадків не приводять до нормалізації обсягу рухів, але дають поштовх до подальшого відновлення класичними методами механо- і фізіотерапії. Низькочастотна вібрація є тим механізмом, який запускає процеси відновлення, тому доцільно починати її якнайшвидше після припинення лікувальної іммобілізації. Дана робота є першою в комплексному дослідженні впливу вібрації низької частоти на відновлення іммобілізаційних контрактур.
Disruption of joint mobility (contracture) is a frequent consequence of the treatment of injuries or orthopedic diseases, when the method of immobilization is used in the treatment process. One of the physiotherapeutic methods of preventing contractures and restoring joint mobility after long-term immobilization is vibration therapy. Objective. Create a concept of the effect of low-frequency vibration on joints and peri-articular tissues after immobilization. Methods. The work was performed using a meta-analysis of literature sourcesfrom scientific databases. The publications were analyzed, which considered the impact of vibration on joints or had data on its biologicalimpact on body tissues. Results. The physiological impact of vibration on the body is based on mechanical, physico-chemical and thermal effects. The expression of the physiological response depends on the frequency and amplitude of oscillations, conditions of conducting and localization. The concept is based on the features of the impact of vibration on the structural elements of limbs and joints. The vibration applied to the limb irritates mechanoreceptors, which send a signal to the central nervous system, and thanks to motoneurons, muscle contraction occurs, which affects peripheral blood flow and blood oxygenation. Accordingly, redox processes in tissues are launched. Micromovements caused by vibration contribute to the nutrition of cartilage and metabolism in the synovial fluid, improving the nutrition of periarticular tissues. This contributes to the recovery of both joint and adjacent tissues. Contraction of muscles through motoneurons gives impetus to a gradual increase in their motor activity and strength, restoration of nutrition and cartilage surface - to restoration of mobility. Conclusions. The created conceptual model of the effect of low-frequency vibration on joints with limited mobility due to immobilization takes into account tissue changes under its influence. The concept involves restoration of nutrition of tissues and muscles adjacent to the joint. It is vibration, due to the possibility of transmitting vibrational energy between tissues, that enables the processes of muscle contraction, which increase the blood supply and metabolism of the joint.
Contractures — limitation of passive movements in the joint — are a fairly frequent complication after immobilization or limitation of mobility and loading of the limb due to injuries, but the exact cause of their formation has not been clarified. Objective. Based on the meta-analysis of the results of experimental modeling and clinical studies of immobilization contractures, create a conceptual model of their formation. Methods. Literature sources from scientific bases were analyzed: Cochrane Library, Scopus, National Library of Medicine, ReLAB-HS Rehabilitation Resources Repository, Mendeley Reference Manager, The Physiological Society library, Google Scholar. Results. A conceptual model of the development of contractures was created. It is shown that immobilization of the joint of the injured limb blocks the execution of the signal of motor impulses. The lack of movement in the joint leads to a decrease in muscle strength and a slowdown in blood circulation. These processes are interrelated: hypotonia of the muscle is due to the restriction of nutrition through the blood supply, and the lack of contractile activity of the muscles leads to the rearrangement of the blood vessels. Articular cartilage is nourished through the subchondral bone and due to osmosis from the synovial fluid during movements. The lack of movement limits nutrition, protein synthesis is disrupted, the surface of the cartilage, synovial membrane and fluid begins to be rebuilt, the joint capsule, ligaments, and tendons thicken. At the same time, the structure of the muscles changes, they shorten and become denser. With long-term immobilization, degenerative processes in the tissues of the joint worsen its general condition, which can eventually lead to complete immobilization. Conclusions. The created conceptual model of the formation of immobilization contractures of joints takes into account the morphological changes of tissues as a result of immobilization. Immobilization affects all components of the joint and adjacent tissues from the first days, the changes progress over time. The use of the model will allow the development of a system of treatment measures to prevent the development of contractures.

Introduction.The term "joint contractures" describes the loss of the passive range of motion of diarthrosis joints, the most common and mobile type of joint. Measuring the passive or active range of motion of a contracture joint is critical to assessing the importance of joint contractures [1]. Joint contractures caused by immobility have both myogenic and arthrogenic components. In a rat model, less than two weeks of immobilisation periods produced contractures associated with muscle restriction, and the contractures were reversible after remobilisation. In the case of immobilisation for four or more weeks, the joint structures contributed more to limiting the range of motion, and the resulting contractures were irreversible [2,3].The aim of the study.To determine the impact of immobilization on the formation of contracture of the knee joint in laboratory animals (rats) and to evaluate the possibility of restoring mobility in the case of using low-frequency vibration during and after immobilization.Object and research methods.Joint contractures are loss of passive range of motion of the diarthrosis joint. Immobilization contractures are one of the common types of joint contractures. The study aimed to determine the effect of immobilization on the formation of knee joint contracture in laboratory animals (rats) and assess the possibility of restoring mobility using low-frequency vibration during and after immobilization. Three groups of laboratory rats, ten animals each, were studied: I -4 weeks of immobilization, 4 weeks of observation without rehabilitation measures; II -4 weeks of immobilization, 4 weeks of low-frequency therapy; III -4 weeks of immobilization, and 4 weeks in a row rats received a course of vibration therapy. The knee joint range of motion was measured weekly. Vibration development was carried out in the mode of 20 Hz with an amplitude of 1.5 mm. The rapid growth of contracture occurs from the 2nd week of immobilization. In the animals of the I and II groups, the process happened in the same way. In the III group, contractures were formed by the 4th week, but significantly less than in the previous groups. After the removal of immobilization, a slow increase in movements was observed in the first group, which did not reach average values by the 4th week after the removal of immobilization. Group II rats had a rapid reduction in contracture, and recovery occurred by week 4. In the III group, recovery was complete in 2 weeks. Low-frequency vibration reduces the impact of immobilization and significantly accelerates the recovery of the joint after its completion.
