The widespread use of high-resolution obstetric ultrasound has allowed extensive evaluation not only of the fetus, but also of the placenta and umbilical cord. Most of the available data on complication rates and clinical course of fetuses with umbilical cord cysts are limited and is in the form of case reports and small case series. We aimed to present the outcome of a fetus with an isolated umbilical cord cyst on the third trimester. The fetus was delivered by cesarean section because of fetal distress and died three days later following the operation due to perinatal asphyxia. It should be remembered that large umbilical cysts may cause fetal distress by pressure on the cord vessels in case of rapid growth. It is vital to consider the ultrasonographic examination of the umbilical cord as one of the important parts of the examination. ÖZYüksek çözünürlüklü obstetrik ultrasonun yaygın kullanımı ile sadece fetusun değil, aynı zamanda plasenta ve umbilikal kordun geniş çaplı değerlendirilmesi mümkün olmuştur. Umbilikal kord kistleri olan fetusların klinik seyri hakkındaki mevcut verilerin çoğu sınırlıdır, daha çok komplikasyon oranları ile ilgilidir ve vaka raporları ve küçük vaka serileri biçimindedir. Bu vaka sunumunda; üçüncü trimesterde izole umbilikal kord kisti ile prezente olan bir fetusun sonucunu sunmayı amaçladık. Fetüs, fetal distres nedeniyle sezaryen ile doğurtuldu ve operasyondan üç gün sonra perinatal asfiksi nedeniyle exitus oldu. Büyük umbilikal kistlerin hızlı büyüme durumunda kordon damarları üzerinde baskı ile fetal sıkıntıya neden olabileceği unutulmamalıdır. Umbilikal kordun ultrasonografik incelemesinin, muayenenin önemli kısımlarından biri olarak kabul edilmesi hayati önem taşımaktadır. Anahtar kelimeler: Umbilikal kist; fetal anomali; umbilikal kord.
Amaç: Geleneksel olarak, cerrahi insizyonlar için bir bistüri kullanılır. Bu çalışmanın amacı, yara iyileşmesi ve deri insizyonunun kozmetik görünümü açısından koter ile bistüriyi karşılaştırmaktır. Yöntemler: Çalışma kadın hastalıkları ve doğum kliniğinde Phannenstiel insizyonu olan hastalarda yapıldı. Aynı hastanın deri insizyonunun yarısı koterin kesme modu ile, diğer yarısı neşter ile açıldı. Koter ile açılan insizyon kısmı, yara iyileşmesi ve kozmetik görünüm açısından neşter ile açılan kısım ile karşılaştırıldı. Değerlendirmeler POSAS (Hasta ve Gözlemci Skar Değerlendirme ölçeği) puanlama ölçeğinin gözlemci kısmına dayandırılmıştır. Değerlendirmeler kısa vadede (postoperatif 15. gün) ve uzun vadede (45. gün) tek kör (gözlemci kör fakat cerrah değil) olarak yapıldı. Bulgular: Pfannenstiel insizyonu olan toplam 73 hasta değerlendirildi. Hastaların ortanca yaşı 33 (21-52) idi. Bistüri ile açılan bölümlerin toplam POSAS skorları, 15. ve 45. günlerde koter ile açılanlara göre istatistiksel olarak farklı değildi (15. gün için 27,3±5,2 ve 27,0±5,1 p=0,88; 45. gün için 11,8±3,2, p=0,56). Sonuç: Çalışma, bistüri kullanımı ile karşılaştırıldığında, kesme modu koter kullanımının, Pfannenstiel insizyonlarında kötü yara iyileşmesi ve kozmetik görünüm olarak bir etkisi olmadığını göstermiştir. Bu sonuç, cerrahları koter ile deri insizyonu açmaya teşvik eder.
Oocyte matched follicular fluid oxidant, antioxidant status, and pro- and anti-inflammatory cytokine levels were assessed to reveal a possible effect of local-intrafollicular levels of these markers on the individual oocyte with its quality, ability to achieve fertilization, further embryo development, and pregnancy. A cross-sectional study of infertile women with diminished ovarian reserve undergoing antagonist protocol in vitro fertilization (IVF); in the form of ICSI, and fresh single embryo transfer were included. When follicular fluid was collected, each ovarian follicle was aspirated independently, and each follicular fluid was collected into a separate test tube to match it with a single cumulus-oocyte complex obtained from the same follicle. Oocyte matched follicular fluid samples and blood specimens were taken from the participants. Relationships of total antioxidant status, total oxidant status, oxidative stress index, total thiol, interleukin (IL)-6, IL-8, and IL-10 levels of each follicle with oocyte grade, grade of transferred embryos, and pregnancy rate of a given follicle were assessed. A total of 23 infertile women with diminished ovarian reserve and 79 individual follicles of these women were assessed. Serum total oxidant status level of metaphase II (MII) group was significantly lower than non-MII group (P < .001). Follicular fluid IL-6 level of MII group was significantly lower than non-MII group (P = .005). Follicular fluid IL-8 value was significantly low with positive pregnancy results (P < .001). Serum oxidative stress status and follicular fluid pro-inflammatory cytokines were associated with IVF outcomes. This unique study might guide IVF practice with the aim of developing and establishing more effective therapeutic strategies and choosing embryos with more potential for success.
Object: In this study, it was aimed to look at the effect of COVID-19 on male fertility by comparing the results of spermiogram samples given before and 6 months after COVID-19 disease in men with a history of infertility. Material and Methods: The method of this study is retrospective. In this study, the parameters were compared in the spermiogram in which the samples given before and 6 months after COVID-19 disease of patients who are 41 male patients between the ages of 21-41, who did not have chronic disease, no continuous medication, and did not receive infertility treatment, who applied to the Urology and Urology-Infertility polyclinics of Istanbul Training and Research Hospital between 01.03.2020-31.03.2022. Results: When the parameters were compared in the spermiogram that samples taken before the COVID-19 disease and 6 months after the COVID-19 disease, it was seen that the total sperm count was significantly lower, while the motility ratio, forward motility ratio, in situ motility ratio and immotile ratio did not differ significantly. Conclusion: Compared to the samples taken before COVID-19 disease, a decrease in the total sperm count was observed in the samples taken 6 months later.
