Zusammenfassung
In einer multizentrischen klinischen Beobachtungsstudie wurden 2003 bis 2005 in 105 urologischen Praxen bei 314 Patienten mit prim?r organisch bedingter Erektiler Dysfunktion (ED) Vertr?glichkeit, Wirksamkeit, Handhabbarkeit und Akzeptanz von intraurethral zu applizierendem Alprostadil (CAS 745-65-3; MUSE; Medikament?ses Urethrales System zur Erektion) untersucht. Die statistisch auswertbaren 306 Patienten waren im Mittel 61,3 ? 9,2 (MW ? SD) Jahre alt (181 Patienten zwischen 60 und 80 Jahren). Die mittlere Dauer ihrer ED lag zwischen 2 und 120 Monaten bei 21,5 ? 22 Monaten (MW ? SD). Die Genese war bei 55% der Patienten lokal bedingt, davon bei 42,8% nach operativen Eingriffen im urogenitalen Bereich 35 % der Patienten waren einer Prostatektomie nach Prostata-Karzinom unterzogen worden. Bei 46% der Patienten lagen vaskul?re, bei 28% metabolische Erkrankungen einschlie?lich Diabetes mellitus und bei 11% Nervensch?digungen vor. Bei 51,3% der Patienten waren Medikamente, von denen Potenzst?rungen bekannt sind, Ursache oder Teilursache der St?rung. Der Grad der ED-Symptomatik war in 93 % der F?lle mittel bis schwer. Alprostadil (MUSE8) wurde in Dosierungen von 250, 500 oder 1000 ug dreimal angewandt, wobei sich bei der dritten Anwendung wegen der besseren Wirksamkeit 65 % der Patienten die Dosis mit 1000 ug applizierten. Sehr gute und gute Wirksamkeit von Alprostadil (MUSE) stiegen von 45,8% der Patienten nach der ersten ?ber 63,7% nach der zweiten auf 69,3% nach der dritten Anwendung an. Bei Patienten mit Zustand nach Prostatektomie wegen Prostata-Karzinoms verbesserten sich sehr gute und gute Wirksamkeit nach der dritten Anwendung gegen?ber der ersten um etwa 20%, betrafen zuletzt 53,9 % der Patienten. Der Vollzug des Geschlechtsverkehrs war im Gesamtkollektiv nach der ersten Anwendung von 67%, nach der zweiten von 83% und nach der dritten von 87% der Patienten m?glich.
Die Handhabbarkeit von Alprostadil (MUSE) wurde von 75% der Patienten als sehr gut und gut, die Akzeptanz des Systems von 76% als sehr gut und gut beurteilt. Keine Schwierigkeiten mit der Handhabung hatten 96 % der Patienten. 81,1 % wollten die Behandlung fortsetzen.
Im retrospektiven Vergleich mit anderen Medikamenten war injizierbares Al-prostadil (SKAT; Schwellfc?rper-iluto-Injektions-Therapie) bei 32,1 % der Patienten gegen?ber Alprostadil (MUSE) bei 25 % etwas wirksamer. Die Alprostadil (MUSE)-Anwendungsform wurde aber von 82,1 % als zweckm??iger angesehen und von 78,6% bevorzugt. Gegen?ber PDE-5-Hemmstoffen war Alprostadil (MUSE) bei der behandelten Klientel zu 77,7% wirk-samer. 79,6% der Patienten bevorzugten es gegen?ber diesen Stoffen. Im Vergleich zu Apomorphin bevorzugten Alprostadil (MUSE) 94,1 % der Patienten bei in 98% der F?lle besserer Wirksamkeit. Die Vertr?glichkeit von Alprostadil (MUSE) wurde von 90% der Patienten mit sehr gut bzw. gut angegeben. Es wurden f?nf F?lle von Nebenwirkungen in Form leichter urethraler Schmerzen angegeben, die nicht zum Absetzen der Therapie f?hrten.
