Nonpharmacologic analgesic measures were effective for the treatment of procedural pain in term infants. The combination of oral 25% dextrose treatment and skin-to-skin contact acted synergistically to decrease acute pain in healthy neonates.
ResumoObjetivo: analisar os conhecimentos dos pediatras que atuam com pacientes neonatais em relação à avaliação e o tratamento da dor do recém-nascido.Método: estudo transversal com 104 pediatras (de um total de 110) que trabalhavam em 1999 a 2001, nas sete unidades de terapia intensiva e nos 14 berçários da cidade de Belém, e responderam a um questionário escrito com perguntas a respeito do seu perfil demográ-fico e do conhecimento de métodos de avaliação e de tratamento da dor no recém-nascido.Resultados: cem por cento dos médicos referiram acreditar que o recém-nascido sente dor, mas apenas um terço deles conhecia alguma escala para avaliar a dor nessa faixa etária. A maioria dos entrevistados referia perceber a presença de dor no recém-nascido por meio de parâmetros comportamentais. O choro foi o preferido para avaliar a dor do bebê a termo; a mímica facial para o prematuro, e a freqüência cardíaca para o neonato em ventilação mecânica. Menos de 10% dos entrevistados diziam usar analgesia para punções venosas e capilares; 30 a 40% referiam empregar analgesia para punções lombares, dissecações venosas, drenagens de tórax e ventilação mecânica. Menos da metade dos entrevistados referiu aplicar medidas para o alívio da dor no pós-operatório de cirurgia abdominal em neonatos. O opióide foi o medicamento mais citado para a analgesia (60%), seguido pelo midazolam (30%).Conclusão: os pediatras demonstraram pouco conhecimento a respeito dos métodos de avaliação e tratamento da dor no período neonatal. Há necessidade de reciclagens e de atualização no tema para os profissionais de saúde que atuam com recém-nascidos doentes. Results: 100% of the pediatricians believed that newborns feel pain, but only one-third of them declared to know any scale for the evaluation of pain for this age group. The majority of the interviewees perceived the presence of pain in newborns by means of behavioral parameters. Crying was the preferential parameter to evaluate pain in full-term newborns; facial activity was the parameter chosen for premature infants; and heart rate for mechanically ventilated neonates. Less than 10% of the pediatricians reported using analgesia for venous and capillary puncture, while 30 to 40% said that they used analgesia for lumbar puncture, venous dissection, chest tube placement and mechanical ventilation. Less than half of those interviewed reported applying postoperative pain relief measures following abdominal surgery. Opioid was the most frequent medication for analgesia (60%), followed by midazolam (30%).Conclusion: these results demonstrate that it is necessary to refresh and update pediatricians' knowledge about pain assessment and relief. Objetivo: A presente pesquisa objetivou avaliar os fatores de risco associados à prematuridade e baixo peso ao nascer dos recém-nascidos das parturientes nos extremos da vida reprodutiva. Métodos: Participaram do estudo 1.117 prontuários de gestantes. Utilizou-se um protocolo de pesquisa previamente formulado pelos pesquisadores para registro das características sociodemográficas maternas, bem como dados sobre: paridade; idade gestacional; realização de pré-natal; intercorrências; via de parto; e outros. Resultados: Os achados evidenciaram que no grupo de mães acima de 34 anos a ocorrência de menos de seis consultas no pré-natal e o índice de Apgar no quinto minuto menor que sete predizem maiores chances de prematuridade e baixo peso ao nascer. Conclusão: Conclui-se que a idade materna influencia significativamente na ocorrência de parto prematuro e baixo peso ao nascimento, sendo justificado essencial políticas de saúde estaduais mais operante em relação ao pré-natal de qualidade.
