Kasdieniai šlapimo pūslės tūrio ir dubens padėties pozicionavimo pokyčiai gali turėti įtakos taikinio (naviko) apšvitinimo tikslumui ir homogeniškumui. Ši radioterapijos problema ypač aktuali klinikinėje praktikoje, esant ribotiems ištekliams. Darbo tikslas – kiekybiškai įvertinti šlapimo pūslės tūrio ir padėties pokyčius invazinio šlapimo pūslės vėžio gydymo radioterapija metu ir nustatyti radioterapijos skyriuje (RS) gydytų pacientų dubens pozicionavimo paklaidą. Šlapimo pūslės pradinio klinikinio švitinimo taikinio tūrio (CTV) ir planuojamo apšvitinti taikinio tūrio (PTV) pokyčiams įvertinti pacientai kiekvieną savaitę buvo skenuojami kompiuteriniu tomografu SOMATOM sensation open (su virtualios simuliacijos funkcija), adaptuojant bei koreguojant švitinimo tūrius ir vertinant jų pokyčius radioterapijos metu. Atliekant kasdienį skaitmeninės rekonstrukcijos radiologinį paciento padėties patikrinimą, įvertintas dubens pozicionavimo tikslumas x, y, z ašyse. Pateikta retrospektyvi duomenų analizė. Tyrimo rezultatai parodė, kad radioterapijos gydymo metu pacientų vidutinis šlapimo pūslės tūris svyravo neženkliai (didžiausi nuokrypiai nuo 15,75 iki 20,75 procento). Reikšmingų tūrio pokyčių nenustatyta. Buvo pastebėti ir įvertinti šlapimo pūslės padėties pokyčiai, kai šlapimo pūslės tūris nepateko į pradinį CTV, tačiau jų tūrio reikšmės dydis nebuvo didelis, visais atvejais atitinkantis PTV, tad buvo homogeniškai apšvitintas. Nenustatyta su pacientais susijusių veiksnių, kurie galėtų kliniškai skatinti šlapimo pūslės tūrio pokyčius. Retrospektyviai vertinant dubens kaulinių anatominių struktūrų judėjimo poslinkio statistinės imties nuokrypius visomis trimis ašių kryptimis: vertikaliąja, išilgine ir šonine, nustatyta RS pacientų dubens pozicionavimo paklaida, kuri dažniausiai buvo ne didesnė nei 5 mm. Prieita prie išvados, kad tinkamas paciento imobilizavimas, kruopštus pozicionavimas, šlapimo pūslės ištuštinimo instrukcijų išaiškinimas ir jų laikymosi kontroliavimas sumažina galimus netikslumus, švitinant sergančiuosius invaziniu šlapimo pūslės vėžiu. Neturint galimybės prieš kiekvieną procedūrą įvertinti organo tūrio pokyčių ribų, tikslus paciento pozicionavimo protokolo vykdymas RS, kasdienės radiologinės verifikacijos ir kontrolinis radioterapijos perplanavimas, adaptuojantis prie anatominių struktūrų, šlapimo pūslės ir naviko tūrio pokyčių, užtikrina saugų radikalios dozės realizavimą.
Ilgametė perspektyvi stebėsena įrodė, kad konvencinė kairiosios krūties radioterapija didina širdies ligų riziką. Nuo individualios pacientės anatomijos priklauso, kiek priekinės širdies sienos ir kairiosios vainikinės arterijos priekinės tarpskilvelinės šakos radioterapijos metu patenka į švitinimo lauką, atliekant įprastinę konforminę 3D radioterapiją. Tyrimo tikslas − įvertinti dozes kritiniams organams, skiriant šiuolaikinę radioterapiją kairiosios krūties vėžio gydymui kvėpavimo kontrolės metodu, kai radioterapijos procedūra atliekama giliai įkvėpus, bei palyginti rezultatus su gautais atliekant konvencinę radioterapijos procedūrą. Retrospektyviai išanalizuoti 50 atsitiktinai atrinktų kairiosios krūties radioterapijos planų, įvertintos dozės kritiniams organams: vidutinė dozė širdžiai (D mean), didžiausia ir vidutinė dozė kairiosios vainikinės arterijos priekinei tarpskilvelinei šakai (D mean ir D max). Klaipėdos universitetinėje ligoninėje kairiosios krūties radioterapijos planai retrospektyviai randomizuotai analizuojami nuo 2016 metų. Nuo 2021 m. į onkologinės radioterapijos praktiką įdiegus naujausią 4D švitinimo metodiką pasitelkiant kvėpavimo kontrolę, buvo išanalizuoti radioterapijos skyriaus pirmo pusmečio rezultatai. Atlikta retrospektyvi nauju metodu švitintų pacienčių dėl kairiosios krūties vėžio analizė, pasitelkiant gilaus įkvėpimo metodą. Nustatyta, kad vidutinė dozė širdžiai (D mean) sumažėjo nuo 1,88 iki 1,24 Gy (34 proc.), o nuo 9,64 iki 3,81 Gy (60 proc.) sumažėjo D mean kairiosios vainikinės arterijos priekinei tarpskilvelinei šakai. Tyrimo rezultatai parodė, kad, suskirsčius pacientes į dvi grupes pagal švitinimo tūrį (I gr. švitinta krūtis, II gr. švitinta krūtis ir sritiniai limfmazgiai), dozė kairiosios vainikinės arterijos priekinei tarpskilvelinei šakai buvo kiek didesnė švitinant ir sritinius limfmazgius, nors vidutinė dozė širdžiai patikimai nesiskyrė. Išvada. Dozės kritiniams organams, švitinant kairiosios krūties vėžį kvėpavimo sulaikymo giliai įkvėpus metodu, yra mažos ir kliniškai spindulinių reakcijų arba koronarinės širdies ligos išsivystymui nėra reikšmingos. Švitinant kairiąją krūtį su sritiniais limfmazgiais, dozė kairiosios vainikinės arterijos priekinei tarpskilvelinei šakai gali būti didesnė. Siekiant ją sumažinti, individualiais atvejais reikia spręsti dėl sudėtingesnio radioterapijos planavimo metodo, pasirenkant moduliuojamo intensyvumo radioterapijos metodus (IMRT/VMAT).
Galvos smegenų metastazės klinikinėje praktikoje diagnozuojamos dažniau nei kiti piktybiniai galvos smegenų navikai. Ilgą laiką visų galvos smegenų švitinimas buvo galvos smegenų metastazių gydymo standartas. Kognityvinių funkcijų sutrikimas po spindulinės terapijos lėmė, kad per pastaruosius dešimtmečius ieškoma visų įmanomų neurochirurginių ir naujų radioterapinių galimybių, siekiant išvengti galvos smegenų švitinimo šalutinio poveikio. Pristatytas atvejis atskleidžia naujai įdiegto stereotaksinės radioterapijos (SRT) metodo Vakarų Lietuvoje galimybes ir privalumus. Pademonstruotas puikus radiologinis ir klinikinis atsakas į frakcijinę stereotaksinę radioterapiją (FSRT), kai kartu švitintos ne tik galvos smegenų renoceliulinės karcinomos metastazės, bet ir neurochirurgų pašalintos metastazės ložė.
