Objetivo: describir algunas características sociodemográficas y clínicas de niños entre 1 y 5 años con trauma encefalocraneano (TEC).Diseño: serie prospectiva de casos en el Hospital Universitario San Vicente de Paúl, Medellín. Colombia.Participantes: noventa niños entre 1 y 5 años atendidos por TEC entre junio de 2003 y noviembre de 2004, residentes en el valle de Aburrá (noroccidente de Colombia) y sin problema neurológicos de base.Resultados: predominaron los pacientes de sexo masculino (64,4%). La mayoría provenían de las comunas orientales de Medellín y eran de estratos socioeconómicos bajos. El 82,2% ocurrieron por caídas, la mayoría desde terrazas, balcones o escalas. El 82,4% de los sitios desde donde cayeron los niños no tenían adecuadas barreras protectoras. En 37 casos (41,1%) el TEC fue de moderado a grave; murieron 3 niños (3,3%). Cincuenta y tres niños (58,9%) se encontraban con sus madres en el momento de la lesión. Los cuidadores generalmente eran adultos, con más de 5 años de escolaridad.Conclusión: para prevenir estas lesiones es prioritario activar un sistema de vigilancia epidemiológica del TEC e instalar barreras de protección adecuadas en las casas.
El tétanos es una enfermedad inmunoprevenible que todavía causa estragos en las naciones que no han logrado coberturas protectoras de vacunación. Su tasa de letalidad alcanza el 50%. Es necesario que nuestro personal de salud esté familiarizado con su prevención, diagnóstico, tratamiento y rehabilitación. Esta revisión tiene como objetivo presentar una actualización de conocimientos sobre esta importante enfermedad, haciendo énfasis en el diagnóstico y el tratamiento.El tétanos es una enfermedad aguda de distribución mundial y presentación esporádica, infrecuente en países industrializados. Por su elevada tasa de letalidad aún representa un problema de salud pública, en especial en regiones con programas preventivos deficientes.
Introducción: el aumento en el número y la complejidad de las Unidades de Cuidados Intensivos Pediátricos (UCIP), sumado a su alto costo, ha estimulado el interés y la necesidad de evaluar su funcionamiento, la eficacia de la labor realizada y la calidad de los servicios que prestan, mediante indicadores asistenciales que se derivan de estudios descriptivos de morbimortalidad. Objetivo: describir las causas de morbilidad y mortalidad y las características sociodemográficas de los niños admitidos a la UCIP del Hospital Universitario San Vicente de Paúl (HUSVP) de Medellín, Colombia, en el período comprendido entre el primero de enero de 2001 y el 31 de diciembre del 2005. Metodología: estudio descriptivo y retrospectivo de corte transversal, en el que por medio de una aleatorización simple se hizo la selección de 328 historias de pacientes que ingresaron a la UCIP durante el período de estudio. Se tabularon y analizaron estadísticamente los datos obtenidos de la revisión de las historias. Resultados: se encontró que 59,8% de los pacientes eran de sexo masculino, 40,9% eran menores de 1 año y 43% provenían del área metropolitana del valle de Aburrá. La edad promedio fue 44 meses. Solamente 47% tenían afiliación a la seguridad social. Las principales causas de admisión a la UCIP fueron: infección respiratoria (23,8%), ingresos postquirúrgicos (23,5%), falla respiratoria (18,6%), choque séptico (16,5%) y malformaciones congénitas (13,1%); el promedio de estancia fue 5 días. Requirieron ventilación mecánica 52,4% de los pacientes, 26% de los cuales presentaron alguna complicación derivada de ella. La mortalidad general fue 21,6% y los pacientes de sexo masculino miprovenientes del área metropolitana fueron los más afectados. Las principales causas de muerte fueron: falla orgánica múltiple (26,8%) y choque séptico (19,7%). 56% de las muertes ocurrieron durante las primeras 48 horas después de la admisión a la UCIP. Discusión: los lactantes menores son el grupo de mayor riesgo para ingresar a la UCIP. Los problemas respiratorios e infecciosos son los más frecuentes, lo cual puede reflejar fallas en los programas de promoción y prevención en los primeros niveles de atención. La mortalidad fue alta con respecto a la reportada en otras UCIP de América Latina; sin embargo, es difícil la comparación por la heterogeneidad de los pacientes. Más de la mitad de las muertes (56,3%) ocurrieron en las primeras 48 horas después de la admisión a la UCIP lo cual puede estar determinado por la gravedad de la enfermedad aguda, la edad, el estado premórbido y otros factores que no dependen del paciente, como las dificultades en la atención inicial o en el traslado.
Introducción: el síndrome de dificultad respiratoria aguda (SDRA) es secundario a inflamación originada en una enfermedad pulmonar primaria o una afección extrínseca al pulmón. Es frecuente en cuidado intensivo y conlleva alta mortalidad. Objetivos: determinar la eficacia y seguridad de los glucocorticoides en dosis bajas en personas mayores de 18 años con SDRA, en términos de mortalidad, días libres de ventilación mecánica, incidencia de infecciones nosocomiales, neuromiopatía y sangrado digestivo. Metodología: se hizo una búsqueda sistemática de ensayos clínicos controlados que compararon glucocorticoides con placebo, en adultos con SDRA en los desenlaces descritos. También, búsqueda secundaria de ensayos clínicos referenciados en los artículos primarios. Resultados: se encontraron siete ensayos clínicos. Se demostró disminución de la mortalidad hospitalaria al día 28 (OR: 0,56 [0,38-0,81], ganancia de 3,5 días libres de ventilación mecánica, disminución en la incidencia de infecciones nosocomiales y neumonía adquirida en el hospital. No hubo diferencias en la neuromiopatía asociada con esteroides, pero sí una tendencia, no significativa, al aumento del sangrado digestivo. Conclusión: los esteroides en dosis bajas disminuyen la mortalidad al día 28 de los adultos con SDRA y aumentan los días libres de ventilación mecánica sin aumentar los efectos adversos significativos.
