EinleitungIn Deutschland werden derzeit jährlich etwa 100.000 Großimplantate, d. h. Hüft-und Knieendoprothesen implantiert, von denen innerhalb einer Zeitspanne von zehn Jahren bis zu 5% wegen aseptischer Lockerung versagen. Diese aseptische Lockerung ist das Resultat einer chemischen und mechanischen Instabilität der Grenzfläche zwischen unzementiertem Implantat und Knochen. Im Falle einer chemischen Instabilität wird häufig eine das Implantat isolierende Bindegewebskapsel gebildet, die die feste mechanische Anbindung des Implantats an den umgebenden lebenden Knochen verhindert. Im Falle der mechanischen Instabilität tritt wegen des großen Unterschieds in den Steifigkeiten des metallischen Implantatmaterials und des (kortikalen) Knochens so genanntes "stress shielding" auf, das zur späteren Knochenresorption und damit zum Implantatversagen führen kann. Eine Vielzahl von klinischen Resultaten zeigt, dass das erste Problem in unzementierten Endoprothesen durch das Aufbringen einer osteokonduktiven keramischen Schicht, überwiegend aus Hydroxylapatit (HAp) zunächst befriedigend gelöst werden kann [1, 2, 3]. Als ökonomisches und funktionell akzeptables Beschichtungsverfahren hat sich atmosphärisches Plasmaspritzen (APS) bewährt. Das zweite Problem ist derzeit ungelöst und Versuche, durch "isoelastische" Endoprothesen Abhilfe zu schaffen, waren bisher von zweifelhaftem Erfolg. Obwohl mit osteokonduktiven Plasma-Beschichtungen gute Ergebnisse erzielt werden, ist die Langzeit-Stabilität der Grenzfläche der Schicht zum metallischen Substrat immer noch problematisch. Bis zu 10% aller aseptischen Lockerungen nach 10 Jahren [4] sind darauf zurückzuführen, dass wegen unzureichender Bindung der keramischen Schicht zum Metall erstere sich ablöst und in den entstehenden Spalt azelluläres Bindegewebe einwandert, das eine feste Anbindung an den Knochen verhindert. Reduzierung der Mit Hydroxylapatit (Gruppe B) und Hydroxylapatit + Titanoxid-Haftvermittlerschicht (Gruppe C) beschichtete sowie unbeschichtete (Gruppe A) Ti6Al4V-Würfel (5 x 5 x 5mmm) wurden in die Femur-Knochen erwachsener Hunde in unterschiedlichen Positionen (proximal links und recht, distal links und rechts) für einen Zeitraum von 3 (1 Hund) bzw. 6 (4 Hunde) Monate implantiert. Die Knochen-Implantat-Grenzfläche wurde histologisch in H&E-gefärbten Dünnschnitten und im Querschnitt rasterelektronenmikroskopisch untersucht. Messungen der Osseointegration (in Prozent) zeigten nach 6 Monaten Beobachtungszeit einen statistisch signifikanten Unterschied zwischen den unbeschichteten (Gruppe A) und den beschichteten (Gruppe B und C) Implantaten (p<0.05). Jedoch wurde kein Unterschied zwischen den Gruppen B und C nach 3 Monaten (p = 0.458) und 6 Monaten (p = 0.512) beobachtet. In Bezug auf die Bindungsfestigkeit der Schichten zum Titanlegierungs-Substrat konnte nach 3 Monaten kein statistisch signifikanter Unterschied zwischen den Gruppen B und C festgestellt werden (p = 0.117).
