The likelihood of experiencing chronic conditions increases with the advancing age. Generally, it is estimated that 80 % of people who is 65 and over experience at least one chronic condition. The entire auditory system changes with the age. The hearing loss developing over time due to aging is defined as Presbyacusis. It is the third common chronic health problem after rheumatoid arthritis and hypertension in the geriatric population. For audiological treatment, hearing aids, cochlear and middle ear implants and assistive hearing aids are effectively used in this group. However, audiological consultation is the key point to improve the quality of life in aging. Although there is not a long history as a science in Turkey, Audiology undertakes an important task for the diagnosis, treatment, rehabilitation of the hearing loss and the implementation and the development of academic studies in elderly as well as infant-children and adult population. Consequently the aim of this study is to evaluate the hearing loss with aging and to create an awareness about the importance of the collaboration between Audiology and Family Medicine and Primary Care regarding in the hearing loss and related problems. Key words: Elderly, presbyacusis, hearing aid, cochlear implant ÖZET İlerleyen yaşla birlikte kronik sağlık sorunları yaşama olasılığı da artmaktadır. Genel olarak 65 yaş ve üstü popülasyonda en az bir kronik rahatsızlık görülme oranı yaklaşık %80'dir. Yaşlanmayla birlikte tüm işitme sistemi de çeşitli değişikliklere uğramaktadır. Yaşlanmaya bağlı olarak zaman içinde gelişen işitme kaybı presbiakuzi olarak tanımlanır. Presbiakuzi, yaşlılık döneminde romatoid artrit ve hipertansiyondan sonra en sık görülen üçüncü önemli kronik rahatsızlıktır. Diğer kronik rahatsızlıklarla birlikte işitme kaybının da varlığı; yaşlı bireylerde depresyon, kaygı, sosyal hayattan izole olma, dikkatsizlik gibi psikolojik, sosyal ve bilişsel sorunları da beraberinde getirmektedir. Yaşlılarda işitme kaybına yönelik hava yolu ve kemiğe yerleştirilen işitme cihazları, koklear implantlar ve yardımcı dinleme cihaz uygulamaları yanı sıra danışmanlık faaliyetleri de uzman kontrolünde yapılmalıdır. Bir bilim dalı olarak uzun bir geçmişi olmamasına rağmen, ülkemizde sağlık alanında önemi yeni anlaşılmaya başlanan odyoloji bilimi, bebek-çocuk-yetişkin popülasyonun yanı sıra, yaşlı bireylerde de kulak ve işitme sorunlarının tanı, tedavi rehabilitasyonu ve ilgili akademik/uygulama çalışmalarının başlatılması, yürütülmesi ve geliştirilmesi konularında önemli bir görevi üstlenmiştir. Bu çalışmanın amacı yaşlılık döneminde görülen işitme kaybını değerlendirmek ve konu ile ilgili birinci derecede ilgili olan aile hekimliği ve odyoloji işbirliğinin önemi konusunda farkındalık yaratmaktır.
Ö ÖZ ZE ET T A Am ma aç ç: : Normal işiten kişilerde gürültünün varlığında işitsel kortikal P300 potansiyeli üze-rinde oluşan değişiklikleri incelemektir. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Çalışma, normal işitmeye sahip 35 kişi ile yapılmıştır. Çocuk grubu n=12, ortalama yaş±SD: 12,06±3,14 yıl; erişkin grubu n=23, ortalama yaş±SD: 31,51±10,40 yıl olarak bulunmuştur. İşitsel P300 yanıtları geleneksel değişikliği fark etme yöntemi ile /da/ ve /di/ konuşma uyaranları kullanılarak kaydedilmiştir. Testler; çocuk ve erişkin grupta a) Sessiz ortamda, b) Gürültünün varlığında ses geçirmeyen kabinde rastgele sırada yapılmıştır. Gürültülü ortamda sinyal/gürültü oranı -10 dB olarak ayarlanmıştır. B Bu ul lg gu ul la ar r: : Ço-cuklarda seyrek uyaran P1, N1, P2, N2 dalga latens ortalamaları incelendiğinde; sessiz ve gürül-tülü ortam arasında anlamlı bir fark elde edilmemiştir. Ancak, seyrek uyaran P3 dalga latensi ve P3 amplitüt ortalamaları gürültülü ortamda sessiz ortama göre daha uzamış olarak saptanmıştır. Erişkin grupta ise seyrek uyaran P1, N1 latens ortalamaları ve P3 amplitüt ortalamalarının arka plan gürültüsünden etkilenmediği; seyrek uyaran P2, N2 ve P3 latens ortalamalarının gürültülü ortamda istatistiksel açıdan anlamlı daha uzamış olduğu belirlenmiştir. Recordings were made in a sound proof test boot in the a) Quiet and b) Noisy conditions in a random order. Signal to noise ratio was -10 dB. R Re es su ul lt ts s: : In children, there was no statistically significant difference for the deviant P1, N1, P2, N2 mean latencies between the quiet and the noisy conditions. The deviant P3 mean latencies and P3 mean amplitudes were found significantly different between two conditions.
