Background The aim of this study was to assess target diabetic goal achievements and to explore variables associated with them. Methods A cross-sectional study was conducted between December 2015 and April 2016 on 188 type 2 diabetic patients attending Ayder Referral Hospital's outpatient diabetic clinic. Glycemic control was assessed using fasting plasma glucose values and total cholesterol and triglyceride were used to evaluate lipid profiles. Bivariate and multivariate logistic regression analyses were done to identify factors associated with poor glycemic control, hypertension, and dyslipidemia. Result Mean duration of diabetes was 6.5 years. Combined glycemic, lipid, and blood pressure targets were achieved only in 8.5% of the participants. More males achieved combined targets than females. Separately, while above two-thirds of the patients had poor glycemic control (67%), more than half of the participants have had poor lipid (58.5%) and blood pressure (52.1%) control. A significant portion of the patients (68.1%) had also comorbidities other than hyperglycemia. In bivariate and multivariate analyses, longer duration of diabetes disease (AOR: 3.4; P = 0.013) and marked month to month fasting plasma glucose (FPG) variability as measured by large standard deviation (AOR: 2.5; P = 0.023) were significantly associated with overall poor mean FPG results. Female sex was also significantly associated with dyslipidemia (AOR: 1.9; P = 0.049). Conclusion The study showed that achievements of combined diabetic goals are generally poor.

Objectives. Early antiretroviral therapy (ART), isoniazid preventive therapy (IPT), and isoniazid-rifapentine (3HP) are effective strategies for preventing tuberculosis (TB) among people living with HIV (PLHIV). The study aimed to determine the effect of IPT on the TB incidence, follow-up CD4+ T cells, and all-cause mortality rate. Participants. Eligible patients on ART (n = 1, 863) were categorized into one-to-two ratios of exposed groups to IPT (n = 621) and nonexposed groups to IPT (n = 1, 242). Exposed groups entered the cohort at their first prescription of IPT, and unexposed groups entered into the study at the first prescription of ART and then followed until the occurrence of the outcome or date of administrative censoring (June 30, 2017). The outcome endpoints were TB incidence, follow-up CD4+ T cells, and all-cause mortality rate. Results. The follow-up CD4+ T cells for the exposed and nonexposed groups were 405.74 and 366.95 cells/mm (World Health Organization (WHO), 2017), respectively, a statistically significant finding (t1861 = −3.770, p<0.0001; Cohen’s d = 0.186). Nine percent of the exposed patients (620 incidence of TB per 100,000 person-years (PYs)) and 21.9% of the nonexposed patients (3160 incidence of TB per 100,000 PYs) developed TB. Mortality rate (per 100,000 PYs) was 440 for the exposed and 1490 for the unexposed patients. Statistically significant determinants of the all-cause mortality were unscheduled follow-up (AHR = 1.601; 95% CI: 1.154–2.222) and unable to work properly (AHR = 2.324; 95% CI: 1.643–3.288). Conclusion. This study demonstrates the effect of IPT in reducing incidence of TB and all-cause mortality rate and improving follow-up CD4+ T cells. Promoting IPT use can help to achieve the TB eradicating national agenda in Ethiopia.

ObjectiveTo characterize anemia and evaluate hematological responses to universal iron-folic acid (IFA) supplementation in Ethiopian pregnant women.MethodA hospital- based prospective follow up study was done between December 2016 and June 2017. Hematological profiles were measured in pregnant women before and after a minimum of one-month IFA supplementation. Mean values and abnormal proportions of hematological profiles were compared before and after supplementation using paired t-test and McNemar test, respectively. Univariate and multivariate analysis were used to analyze the association between independent variables and poor treatment responses.ResultLack of adequate hemoglobin response was found in 48.5%(95/196) of the participants. Prevalence of anemia and low hematocrit value were decreased significantly after IFA supplementation (p = 0.002, and p = 0.001, respectively). Normocytic hypochromic anemia was the commonest form of anemia found in this study followed by normocytic normochromic anemia. There was no statistically significant association between poor hemoglobin responses and all studied factors such as educational level, household size, parity, recent illness, stage of pregnancy, coffee consumption, and duration of iron treatment.ConclusionOur study revealed poor hemoglobin responses in nearly half of the study participants and a high proportion of anemias morphologically atypical of iron deficiency anemia. There is a need to consider anemia attributable to etiologies other than an iron deficiency in anemia intervention programs.

Psoriasis is a chronic inflammatory skin disease characterized by the immune system activation due to genetic and environmental factors that leads to the development of skin lesions. Reactive oxygen species produced as a result of skin inflammation may cause disorders of the antioxidant defense systems and increased oxidative stress in psoriasis which was proposed to have a consequent function in psoriasis. The study was planned to evaluate oxidative stress by measuring Total Oxidant Capacity, Total Anti-oxidant capacity and Oxidative Stress Index in psoriatic patients. Methodology: A comparative cross-sectional study design was used. A total of 90 participants between age of 18-65 years comprising 45 newly diagnosed psoriatic patients and 45 sex and age matched control groups were enrolled in the study. Result: The present study showed significantly increased levels of serum Total Oxidant Capacity (p<0.001) as well as Oxidative Stress Index (p<0.001) in psoriasis patients as compared to controls and positively correlated with severity and duration of the disease. Total Antioxidant capacity levels were significantly (p<0.001) lower in patients than in controls and negatively correlated with severity and duration of the disease. Conclusion: These results provide evidences regarding the role of increased reactive oxygen species with decreased antioxidant activity in psoriatic patients.
