Background: Coronary artery bypass surgery is needed in different group of patients.Objective: This study was undertaken to see the different socio-demographic characteristics of patients undergone coronary artery bypass surgery. Methodology: This cross-sectional study was conducted in the Department of Cardiac Surgery at National Institute of Cardiovascular Diseases (NICVD), Dhaka, Bangladesh during January 2014 to December 2015 for a period of two (2) years. Patients' undergone CABG after fulfilling the inclusion and exclusion criteria was recruited for this study. The details sociodemographic characteristics of the study population were recorded. Result: A total number of 60 patients were recruited for this study after fulfilling the inclusion and exclusion criteria. The age of the patients undergoing OPCAB surgery ranges from 37 years to 70 years. But most of the patients were in the range of 41 to 60 years which was 50(83.3%). There was an overall male dominance among the patients which was 91.67%. the ratio of male and female was 11.0:1. NYHA class I was in 32(53.3%) cases and class II was 28(46.47%) cases. Hypertension was present in 37(63.3%) cases. Diabetes mellitus and dyslipidemia were present in 50(83.3%) cases and 37(61.7%) cases respectively. Obesity was present in 40(66.7%) cases. Smoking habit was present in 34(56.7%) cases. Conclusion: In conclusion the efficacy of topical tranexamic acid is helpful for reducing postoperative bleeding after OPCAB surgery.
Background: Arterial hypertension is one of the leading modifiable risk factor in coronary artery disease patients who underwent coronary artery bypass grafting (CABG) with the major impact on clinical outcome in these patients. This study was done to evaluate the effect of arterial hypertension on postoperative mortality and morbidity after off-pump CABG.Methods: 200 patients with mean age of 52 ± 6 years, (182 male/18 female) undergoing off pump CABG were divided equally into two groups, 100 patients with hypertension (Group A) and 100 patients without hypertension (Group B). Postoperative mortality and morbidities (Wound infection, AMI, TIA, Stroke) up to 6 months were recorded in the 2 groups.Results: In comparison with the normotensive patients, hypertensive patients had a higher mortality rate (Group A 10% and Group B 2%, p value-<0.05) and increased frequency of myocardial infarction (Group A 12 % and Group B 3%, p value - <0.05). There was also increased rate of infection in hypertensive patients (Group A 13 % and Group B 4%, p value - <0.05). We found that hypertensive patients developed post-operative cerebrovascular complications like TIA (Group A 8 % and Group B 1%, p value - <0.05) and stroke (Group A 11 % and Group B 2%, p value - <0.05) more frequently than normotensive patients. Post-operative ventilator support and duration of hospital stay were almost similar in both groups.Conclusions: Among CABG patients, those with a history of hypertension have an increased frequency of post-operative complications and increased mortality.Cardiovasc. j. 2017; 10(1): 13-16

In this paper, we presented a case study of severe mitral stenosis with giant LA with LA thrombus in a 42 years old female patient. Background: Postoperative mediastinal bleeding in patients undergoing cardiac surgery is still one of the most common complications. Objective: This study was intended to investigate the efficacy of topical tranexamic acid in reducing postoperative bleeding after post-operative cardiovascular surgery. Methodology: In this non-randomized, double blinded, clinical trial, patients undergoing Off-Pump Coronary Artery Bypass (OPCAB) surgery in NICVD, Dhaka, Bangladesh during January 2014 to December 2015 and fulfilling inclusion and exclusion criteria were recruited. They were assigned in two groups- 30 patients in tranexamic acid group (Group 1) and 30 patients in placebo group (Group 2). On completion of the grafting, before closure of the sternum tranexamic acid (2.5 g/25 mL) or placebo (25 mL of saline) diluted in 100 mL of warm saline (370 C) was instilled into the pericardial cavity including the mediastinal tissues and left for 5 minutes. Then it was cleared out by wall sucker and sternum was closed. Results: There was no significant difference in baseline demographic data, basic clinical characteristics and preoperative coagulation profile between the 2 groups (P > 0.05). Total mediastinal bleeding in group 1 and group 2 patients were421.67±70.32 vs 593.33±77.38ml, p<0.001. In case of, whole blood transfusion in group 1 and group 2 patients were 0.87±0.0.73 units and1.77±0.57 units respectively, p<0.001. Conclusion: No patient required reoperation for bleeding and there was no incidence of prolonged ventilation, MI, thromboembolism, DVT or CVA in any of the patients in either group.
