Achalasia is a relatively rare primary motor esophageal disorder, characterized by absence of relaxations of the lower esophageal sphincter and of peristalsis along the esophageal body. As a result, patients typically present with dysphagia, regurgitation and occasionally chest pain, pulmonary complication and malnutrition. New diagnostic methodologies and therapeutic techniques have been recently added to the armamentarium for treating achalasia. With the aim to offer clinicians and patients an up-to-date framework for making informed decisions on the management of this disease, the International Society for Diseases of the Esophagus Guidelines proposed and endorsed the Esophageal Achalasia Guidelines (I-GOAL). The guidelines were prepared according the Appraisal of Guidelines for Research and Evaluation (AGREE-REX) tool, accredited for guideline production by NICE UK. A systematic literature search was performed and the quality of evidence and the strength of recommendations were graded according to the Grading of Recommendations Assessment, Development and Evaluation (GRADE). Given the relative rarity of this disease and the paucity of high-level evidence in the literature, this process was integrated with a three-step process of anonymous voting on each statement (DELPHI). Only statements with an approval rate >80% were accepted in the guidelines. Fifty-one experts from 11 countries and 3 representatives from patient support associations participated to the preparations of the guidelines. A doença do refl uxo gastroesofágico (DRGE), considerada uma das afecções digestivas de maior prevalência nos paises ocidentais (22) , tem sido bastante estudada. Merece destaque o grande número de publicações existentes na literatura médica sobre a mesma. Contudo, há controvérsia em relação a vários aspectos relevantes da afecção.O objetivo do presente estudo foi salientar tópicos importantes da doença e apresentar revisão de literatura do tema. Foram revistos os seguintes tópicos: defi nição, prevalência, manifestações clínicas, diagnóstico, indicações e limitações dos principais métodos diagnósticos e tratamento clínico, cirúrgico e endoscópico. Estratégia de pesquisa e critérios de seleção de trabalhosPesquisou-se, via Internet, artigos de revisão, consensos, "guidelines" e revisões sistemáticas de literatura com meta-análise, publicados nos últimos 5 anos (2000 a 2005), nas seguintes bases de dados: Central de Registro de Estudos Controlados da Cochrane Library e na PubMed. Conclusões -Dentre as 12 conclusões apresentadas, destacam-se duas: 1. a impedanciometria esofágica tem contribuído substancialmente para o melhor conhecimento da doença do refl uxo gastroesofágico e se desponta, quando associada à pHmetria (impedanciopHmetria esofágica), como o novo padrão-ouro para o diagnóstico do refl uxo gastroesofágico, e 2. dentre os sintomas da afecção há os que podem ser considerados ácido-dependentes que costumam responder bem ao tratamento com inibidores de bomba de prótons. Contudo, há também sintomas que são mais dependentes da presença física do refl uxo do que da sua acidez. One hundred and twenty-two patients with gastroesophageal reflux disease were studied (90 with and 32 without esophagitis) with the objective of analyzing possible differences between those with and without esophagitis. Evaluation consisted of clinical interview, endoscopy of the high digestive tract, esophageal manometry, and pH monitoring. There was no significant difference between the groups in age, sex, or symptoms. The incidence of hiatal hernia was greater in the group with esophagitis. Although the frequency of motor changes was similar, the type of anomaly was different. The reflux pattern was very similar in both groups. Therefore, the concept of reflux disease, esophagitis, and pathological reflux still needs a broader definition for greater diagnostic precision and for comparing the results of different studies on the subject. Normal reflux (confirmed using pH esophageal monitoring) in 12.2% of patients with esophagitis suggests that other factors are implicated in the etiology of the disease besides those measured using this examination.
BACKGROUND Achalasia is a rare benign esophageal motor disorder characterized by incomplete relaxation of the lower esophageal sphincter (LES). The treatment of achalasia is not curative, but rather is aimed at reducing LES pressure. In patients who have failed noninvasive therapy, surgery should be considered. Myotomy with partial fundoplication has been considered the first-line treatment for non-advanced achalasia. Recently, peroral endoscopic myotomy (POEM), a technique that employs the principles of submucosal endoscopy to perform the equivalent of a surgical myotomy, has emerged as a promising minimally invasive technique for the management of this condition. AIM To compare POEM and laparoscopic myotomy and partial fundoplication (LM-PF) regarding their efficacy and outcomes for the treatment of achalasia. METHODS Forty treatment-naive adult patients who had been diagnosed with achalasia based on clinical and manometric criteria (dysphagia score ≥ II and Eckardt score > 3) were randomized to undergo either LM-PF or POEM. The outcome measures were anesthesia time, procedure time, symptom improvement, reflux esophagitis (as determined with the Gastroesophageal Reflux Disease Questionnaire), barium column height at 1 and 5 min (on a barium esophagogram), pressure at the LES, the occurrence of adverse events (AEs), length of stay (LOS), and quality of life (QoL). RESULTS There were no statistically significant differences between the LM-PF and POEM groups regarding symptom improvement at 1, 6, and 12 mo of follow-up ( P = 0.192, P = 0.242, and P = 0.242, respectively). However, the rates of reflux esophagitis at 1, 6, and 12 mo of follow-up were significantly higher in the POEM group ( P = 0.014, P < 0.001, and P = 0.002, respectively). There were also no statistical differences regarding the manometry values, the occurrence of AEs, or LOS. Anesthesia time and procedure time were significantly shorter in the POEM group than in the LM-PF group (185.00 ± 56.89 and 95.70 ± 30.47 min vs 296.75 ± 56.13 and 218.75 ± 50.88 min, respectively; P = 0.001 for both). In the POEM group, there were improvements in all domains of the QoL questionnaire, whereas there were improvements in only three domains in the LM-PF group. CONCLUSION POEM and LM-PF appear to be equally effective in controlling the symptoms of achalasia, shortening LOS, and minimizing AEs. Nevertheless, POEM has the advantage of improving all domains of QoL, and shortening anesthesia and procedure times but with a significantly higher rate of gastroesophageal reflux.
RESUMO -Racional -As anomalias do processo da deglutição têm etiologia multifatorial e complexa. A videofl uoroscopia da deglutição tem sido apontada como o exame de maior utilidade na investigação diagnóstica desses casos. Tal método, quando precedido de anamnese clínica adequada, consegue caracterizar convenientemente o grau de disfunção e, freqüentemente, identifi car a causa da anomalia com grande precisão. Objetivos -Estudar as queixas clínicas e os achados da videofl uoroscopia em pacientes com distúrbios da deglutição, sem evidências clínicas de afecções neurológicas associadas, distribuídos em duas faixas etárias -adultos e idosos -e analisar as manifestações sintomáticas, o tipo de disfunção (orofaríngea ou esofágica) e a capacidade de elucidação da queixa clínica pelo método de imagem. Material e Métodos -Foram analisados 70 pacientes com queixa de alteração na capacidade de deglutição, sem evidências clínicas de afecções neurológicas associadas, distribuídos em dois grupos: adultos (GI) -idade inferior ou igual a 65 anos (n = 36) e idosos (GII) -idade superior a 65 anos (n = 34). Todos foram submetidos a anamnese para obtenção de informações sobre queixas em relação à deglutição; as queixas foram caracterizadas como altas ou baixas, de acordo com o local predominante de manifestação. Todos os pacientes foram submetidos a videofl uoroscopia da deglutição; as alterações observadas com esse exame foram caracterizadas como orofaríngeas ou esofágicas. Avaliou-se a capacidade de elucidação da queixa clínica pela videofl uoroscopia nos dois grupos. Resultados -Dentre as queixas analisadas, a única cuja análise estatística demonstrou diferença signifi cante entre os grupos, foi a de pirose, que ocorreu com maior freqüência no grupo de adultos (oito pacientes (22,2%) do GI e um (2,9%) do GII referiam tal queixa). Ao estudo videofl uoroscópico, observou-se maior ocorrência de disfunção orofaríngea no grupo de pacientes idosos (41,2% GII x 13,9% GI) e ocorrência semelhante, nos dois grupos, de disfunção esofágica (35,3% GII x 33,3% GI). Dezenove (52,8%) pacientes do GI e 23 (67,6%) do GII tiveram suas queixas clínicas elucidadas pela videofl uoroscopia. Conclusões -1. As queixas clínicas associadas ao quadro de difi culdade de deglutição ocorrem com freqüência semelhante em adultos e idosos, com exceção da queixa de pirose que predomina no grupo com idade menos avançada; 2. pacientes idosos apresentam maior ocorrência de alterações altas de deglutição (orofaríngeas); 3. a videofl uoroscopia da deglutição representa método de grande importância diagnóstica, pois permite a identifi cação de alterações morfofuncionais relevantes que não são passíveis de identifi cação adequada à anamnese, e 4. a capacidade de elucidação diagnóstica da videofl uoroscopia da deglutição é maior no grupo com idade mais avançada.
