Introducción: Debido a la gran variedad de deformidades dentofaciales que involucran al maxilar se han diseñado diferentes técnicas para su corrección teniendo en cuenta las características propias de cada paciente y que cada una de ellas tiene indicaciones específicas. La técnica convencional Le Fort I para impactaciones mayores a 5 mm en ocasiones no brinda los resultados óptimos en cuanto a competencia labial se refiere además de generar problemas respiratorios obstructivos en vía aérea debido a la disminución del espacio del piso de fosas nasales de manera excesiva. Métodos: Paciente masculino de 16 años de edad que presenta exceso vertical maxilar de 10 mm, siendo tratado con la técnica Le Fort I alveolar total o herradura en dos segmentos, como tratamiento. Resultados: Al implementar la técnica de osteotomía alveolar total maxilar se observó que es el tratamiento más apropiado, tanto en estética como en función, para los pacientes con excesos verticales mayores a 5 mm.
Introducción: El miofibroma es una lesión neoplásica benigna de células miofibroblásticas de rara aparición en la región maxilofacial, se considera una patología homóloga en solitario de la miofibromatosis que ocurre cuando se identifican múltiples lesiones en un mismo paciente. Objetivo: Se presenta una variedad clínica periosteal no descrita previamente en la literatura del miofibroma mandibular en un paciente pediátrico. Presentación del caso: Se reporta un caso de siete años de evolución en un paciente femenino recibido a los tres años con diagnóstico de miofibroma mandibular periosteal tratada con la enucleación del tumor y la resección del borde óseo mandibular afectado, también se describen los aspectos clínicos, radiográficos e histopatológicos relevantes al manejo y tratamiento de esta enfermedad. Conclusiones: Es importante documentar casos clínicos de características particulares, en lesiones controversiales de difícil diagnóstico para evitar procedimientos agresivos innecesarios.
El quiste óseo solitario es una lesión benigna cuyo origen sigue siendo hasta hoy desconocido, siendo el traumatismo la principal teoría de su origen, atribuido a un sangrado intraóseo, el cual no permite la reparación ósea, lo que conlleva a la formación de una cavidad quística con contenido serohemático sin revestimiento epitelial siendo la mandíbula la más afectada en la mayoría de los casos; se presenta en pacientes entre 20 y 30 años, con prevalencia por género masculino, asintomática, de crecimiento lento y desplazamiento de corticales, pudiendo producir fracturas patoló- gicas mandibulares, su hallazgo es incidental durante el examen radiográfico, observándose como una imagen radiolúcida uni- o multilocular, con bordes definidos, sin zona esclerótica. Estudios complementarios de tomografía y resonancia magnética, han mostrado que estas cavidades no necesariamente están ocupadas por líquido, sino que puede haber gas o estar vacías. Se debe de hacer diagnóstico diferencial con quiste dentígero, ameloblastoma, tumor odontogénico queratinizante, tumor odontogénico epitelial calcificante, tumor odontogénico adenomatoide.
<p>Las fracturas del piso orbital, pared medial, pared lateral y techo pueden ocurrir aisladas, en combinación o en asociación con otras fracturas faciales; esto dependerá del nivel de energía transmitido durante el proceso de la lesión.</p><p>Las fracturas “blow-out” de la órbita son fracturas del piso orbital en donde el rim orbitario se mantiene intacto.</p><p>El mecanismo de esta fractura es debido a un aumento repentino de la presión intraorbital como podrá ser el traumatismo recibido a los tejidos blandos de la órbita por una pelota de tenis o el golpe por un puño humano.</p><p>El propósito de este artículo es el de describir una técnica de reconstrucción del piso de la órbita mediante la colocación de una malla de titanio en un paciente del Hospital Reg. “Lic. Adolfo López Mateos” ISSSTE así como su evolución post operatoria.</p>
