Objective: To evaluate and collect epidemiological data on injuries of the anterior cruciate ligament of the knee in professional soccer players of the Brazilian Championship. Methods: Transversal study, with information extracted from the online data platform www.transfermrkt.com.br (Transfermarkt GmbH & Co. KG), with 5 seasons (2015 - 2019) of the Brazilian championship series A and B being analyzed and revised by 3 researchers. Results: 52 injuries of the anterior cruciate ligament were detected, with a mean age of 26,3 years, time off due to injury of 244.5 ±31.6 days, regardless of the division. Forwarders and defenders had the highest incidences of injury without statistically significant difference, but they had greater chances than goalkeepers, left/right backs and midfielders. Conclusion: The incidence of injury to the anterior cruciate ligament of the knee in professional football players in the first and second divisions of the Brazilian soccer championship between 2015 and 2019 is 0.414 per 1,000 hours of play, a value similar to described in the literature. The incidence of the injury varies according to the player’s field position, being the forwarders and defenders the most injured players. Level of Evidence II, Retrospective study.
Pigmented villonodular synovitis (PVNS) is a non-neoplastic proliferative process that involves synovial tissue in the joints, tendon sheaths and bursae. It usually occurs in young adults, aged 20–50 years, is characteristically monoarticular and of slow progression. Clinical symptoms are nonspecific, and joint stiffness and pain are common in long-term cases. Shoulder PVNS is known to be extremely rare, especially when affecting the subacromial bursal region without joint involvement. Magnetic resonance is the imaging modality of choice in PVNS and is useful for diagnosis, surgical planning and monitoring. Complete surgical synovectomy remains the treatment of choice, and all pathological synovial tissue should be removed. Patients who are not properly treated can progress to joint destruction. We describe a case of PVNS in the subacromial bursa of a 15-year-old patient, with exuberant symptoms and 2 years of evolution, treated with extensive synovectomy and good clinical results in the 1-year follow-up.
Objective: This study aims to understand the way fighting athletes respond to bone block surgery in the treatment for shoulder instability. Methods: Prospective clinical study with competitive fighters with shoulder instability who underwent bone block surgery from 2013 to 2016, followed by a postoperative rehabilitation protocol. For the evaluation, eight combat athletes with anterior shoulder instability were treated, with a total of nine shoulders, since one athlete underwent bilateral surgery. All patients signed the Free and Informed Consent Form. The evaluation protocol included medical consultation, radiography of the operated shoulder, degree of active and passive lateral rotation; degree of active and passive elevation; visual analogue scale (VAS) for pain; Athletic Shoulder Outcome Rating Scale (EROE; acronym in Portuguese) scores; Western Ontario Shoulder Instability Index (WOSI), and American Shoulder and Elbow Surgeons (ASES). Results: We observed a decrease in the range of passive and active movement in the recent postoperative period. In later postoperative, values were close to those in the preoperative period at the end of the follow-up. There was improvement in pain, and in all ASES, WOSI and EROE scores no complications were documented. As for returning to sport, two athletes did not return, one of them due to shoulder pain and the other due to retirement. Conclusion: Bone block surgery has shown good functional results in uncomplicated combat athletes. Level of Evidence IV, Prospective Case Series.
