В останні десятиріччя значно збагатилися знання щодо ме таболізму заліза у ссавців. Дослідження у галузі генетики, біохімії і молекулярної біології дозволили встановити та охарактеризувати ряд молекул, що беруть участь у регу люванні гомеостазу заліза. Значний прогрес відбувся після відкриття у 2000 році невеликого пептиду-гепсидину, який, як було доведено, відіграє центральну роль у регу ляції метаболізму заліза, а також забезпечує зв'язок між метаболізмом заліза, запаленням і вродженим імунітетом. Гепсидин безпосередньо взаємодіє із феропортином-єди ним відомим експортером заліза у ссавців, який експре сується ентероцитами, макрофагами і гепатоцитами. Пря ма взаємодія гепсидину з феропортином забезпечує адап таційні відповіді організму у ситуаціях, що змінюють нор мальний гомеостаз заліза (гіпоксія, анемія, дефіцит заліза, перевантаження залізом, запалення).
Colorectal cancer is an extremely urgent issue in modern medicine. This disease is often complicated by anemia, which has specific pathogenetics mechanisms of development and forms a mutual burden syndrome of diseases in cancer patients. The anemic syndrome is accompanied by the development of tissue hypoxia, which in turn activates the processes of oxidative stress and leads to increased release of biologically active compounds, in particular, biogenic amines. One of these is histamine. Its high concentrations cause spasm of the arterioles, which exacerbates tissue hypoxia. We have examined (n=153) patients with colorectal cancer without anemia, (n=75) patients with colorectal cancer complicated bymalignant tumor anemia, and (n=53) patients with iron deficiency anemia. The content of plasma free serotonin fractions was determined by the fluorometric method proposed by Mikhailychenko B.V., Vydyborets S.V. (1999). The patients with iron deficiency anemia and malignant tumor anemia have shown to have a significant increase in plasma free histamine, compared with the control group and the group of patients with colorectal cancer with out anemia. Plasma free histamine was increasing together with the severity of anemia. The article discusses the feasibility of using the content of plasma free histamine, as an option, to assess the state of compensation of secondary metabolic disorders in iron deficiency anemiaand malignant tumor anemia during treatment and its possible differential diagnostic value.
Due to the growing incidence of cancer in the world, it is becoming more relevant to study the indicators of secondary changes in blood in malignancies to use them as diagnostic and prognostic markers. The objective of the work is to conduct a morphometric analysis of peripheral blood erythrocytes in patients with iron deficiency anemia (IDA) and malignant anemia in colorectal cancer to identify specific changes and use them in a differential diagnostic practice. As the study material blood of 110 patients (58 men and 52 women) was taken. Among them 53 patients (31 women and 22 men) with IDA were examined, they formed the first (I) observation group and 57 patients (36 men and 21 women) with colorectal cancer, where the course of the underlying disease was burdened by malignant anemia second (II) observation group. The age of the patients under the survey is from 22 to 69 years. All patients were examined before any treatment was prescribed. The control group consisted of 50 healthy primary donors. Results. The data on the clinical significance of laboratory determination of morphometric changes in peripheral blood erythrocytes is highlighted in this paper. Differential-diagnostic and prognostic value of morphometric changes of erythrocytes in peripheral blood with iron deficiency anemia and anemia of malignancies is discussed. Indicators of morphometric characteristics of erythrocytes can be used in the differential diagnosis of anemia.
Синдром масивн ого цитолізу пухлини (СМЦП)-стан, за якого швидке руйнування пухлинних клітин призводить до загрози життю пацієнта, супроводжується гіперури кемією, гіперфосфатемією, гіперкаліємією, гіпо кальціємією, і нерідко гострою нирковою недостатністю. Без своєчасної та адекватної терапії СМЦП може при звести до загибелі хворого. Рання діагностика та терапія зменшують ризик летальних ускладнень. Ключові слова: синдром масивного цитолізу пухлини, хіміотерапія, гіперкаліємія, гіперфосфатемія, гіперури кемія, гостра ниркова недостатність, цитоліз, пухлина, гострий лейкоз.
Синдром масивного цитолізу пухлини (СМЦП)-стан, за якого швидке руйнування пухлинних клітин призводить до загрози життю пацієнта, супроводжується гіперурикемією, гіперфосфатемією, гіперкаліємією, гіпокальціємією і нерідко гострою нирковою недостатністю. Не дивлячись на те, що патогенетичні механізми цього стану досить непогано вивчені та розроблені більш систематизовані терапевтичні заходи, смертність від СМЦП залишається високою. Раннє виявлення хворих з груп підвищеного ризику та запобігання розвитку даного ускладнення-нагальна проблема. СМЦП частіше спостерігається у хворих з пухлинами лімфоїдної тканини, гострими лейкозами, проте зустрічається і при солідних пухлинах. Під час проведення хіміотерапії пацієнти з групи підвищеного ризику повинні перебувати під ретельним наглядом, необхідні щоденний моніторинг маси тіла, визначення концентрації сечової кислоти, рівня електролітів. Метаболічні та електролітні порушення можуть погіршуватись приєднанням ниркової недостатності. Без своєчасної та адекватної терапії СМЦП може призвести до загибелі хворого. Рання діагностика та терапія зменшують ризик летальних ускладнень.
