RESUMENObjetivos: El propósito de este estudio fue evaluar las dimensiones transversales del arco maxilar (ancho intercanino y ancho intermolar) y esqueléticas (ancho maxilar y ancho facial) en pacientes con secuela de Fisura Labio Alveolo Palatina Unilateral (FLAPU) y compararlas con las de pacientes no fisurados. Material y Métodos: Se utilizaron modelos de estudio y radiografías postero-anteriores de 94 niños de 6 a 15 años de edad atendidos en el Instituto Especializado en Salud del Niño, 47 con secuela de FLAPU y 47 no fisurados. Se tomaron fotografías digitales para la evaluación de los modelos de estudio, las distancias se midieron en el maxilar superior para el ancho intercanino de centroide a centroide de caninos deciduos o permanentes contralaterales y para el ancho intermolar de centroide a centroide de primeras molares permanentes contralaterales. Se realizaron trazos cefalométricos utilizando los criterios de Ricketts para la evaluación de las radiografías. Resultados: En los pacientes con secuela de FLAPU la distancia promedio + desviación estándar para el ancho intercanino, ancho intermolar, ancho maxilar, y ancho facial, fue de 26,94 + 4,30 mm, 47,77 + 4,61 mm, 65,06 + 4,96 mm y 125,50 + 5,42 mm, respectivamente; en los pacientes no fisurados fue de 31,77+ 2,03 mm, 47,86 + 2,84 mm, 65,04 + 3,47 mm y 124,38 + 6,60 mm, respectivamente Conclusiones: En las dimensiones transversales del arco maxilar, el ancho intercanino fue menor en los pacientes con secuela de FLAPU. Sin embargo, no existieron diferencias entre las dimensiones transversales esqueléticas en los dos grupos de pacientes.PALABRAS CLAVE: Labio hendido, paladar hendido, arco dental/anomalías, maxilar/anomalías. Las anomalías dentarias más frecuentes e importantes que afectan a los niños son la presencia de piezas supernumerarias, piezas retenidas y odontomas. Algunos dientes retenidos se pueden encontrar en transposición. Reportamos el caso de una paciente de sexo femenino de 11 años de edad con presencia de diente supernumerario, canino en transposición y odontoma complejo situados en la hemiarcada derecha del maxilar superior. El motivo de consulta fue por la presencia de dientes anteriores sobresalidos que no le permitía realizar un buen sellado labial. Al examen clínico, presentaba gingivitis generalizada asociada a placa bacteriana, secuencia de erupción retardada por permanencia de pieza 53, ausencia clínica de pieza 13, aumento de volumen en palatino a nivel de piezas 12 y 11 de consistencia dura y coloración blanquecina que a la presión presentaba isquemia. La presencia del odontoma complejo a nivel cervico mesial de la pieza 16 fue un hallazgo radiográfico. El abordaje quirúrgico se realizó en dos fases debido a las zonas donde se encontraban las tres patologías. El diagnóstico y tratamiento multidisciplinario fueron de vital importancia. En este caso, odontopediatría, radiología, ortodoncia y cirugía estuvieron involucrados.
Supernumerary and impacted permanent teeth are common in children. Some impacted teeth may be associated to dentigerous cysts. We report a case of an eleven years old male patient with presence of three supernumerary teeth in the maxilla; tooth 1.1 impacted, and 1.3 impacted associated to dentigerous cyst. The chief complaint was precisely the lack of 11 and 13 in mouth. Intraoral examination showed: generalized gingivitis associated to dental plaque, dental caries, clinical absence of 11 and 13, and increased volume on the palatal area at 12 and 11. The radiographic examination revealed that 11 and 13 were impacted, and the three supernumerary teeth were a radiographic findings. Surgical treatment was performed under general anesthesia. Early diagnosis and multidisciplinary treatment were vital.
El propósito del presente estudio fue determinar la frecuencia y distribución de intervenciones quirúrgicas de dientes supernumerarios, odontomas y dientes incluidos en pacientes atendidos en el servicio de Odontopediatría de la clínica de postgrado de la Universidad Peruana Cayetano Heredia entre los años 2002-2009. Se evaluó: alteración dental, género, edad, localización, tipo de intervención quirúrgica y forma de presentación. La muestra estuvo constituida por 192 historias clínicas seleccionadas de un total de 329 historias clínicas de pacientes entre 2 a 15 años de edad. Para el análisis estadístico se usó la prueba chi cuadrado y la prueba exacta de Fisher. Se encontró una mayor frecuencia de intervenciones quirúrgicas de dientes supernumerarios que de odontomas y dientes incluidos. Se evidenció el mayor número de intervenciones quirúrgicas de dientes supernumerarios, en el grupo de 6 a 11 años de edad, la forma de presentación que predominó fue la única, con mayor frecuencia en la zona anterior del maxilar superior, y la intervención quirúrgica realizada fue la exodoncia compleja. Con respecto a las intervenciones de odontomas, la forma de presentación que predominó fue el odontoma compuesto y la intervención quirúrgica realizada fue la enucleación. Las intervenciones quirúrgicas de dientes incluidos fueron más frecuentes en el grupo de 6 a 11 años de edad, la zona de mayor predilección fue la zona anterior, siendo los incisivos superiores los más afectados, la intervención quirúrgica realizada con mayor frecuencia fue la exodoncia compleja seguida de la colocaciónde un botón ortodóntico. Estas alteraciones, dientes supernumerarios, odontomas y dientes incluidos, demostraron tener una alta frecuencia entre los pacientes pediátricos de ahí la importancia de establecer un diagnóstico precoz y un plan de tratamiento óptimo procurando evitarcomplicaciones futuras.
