Introdução: A sífilis gestacional e a congênita, apesar de apresentarem diagnóstico e tratamento de fácil manejo, ainda permanecem com elevadas incidências no Brasil. Objetivo: Caracterizar a situação epidemiológica dos casos de sífilis em gestantes (SG) e sífilis congênita (SC) notificados em um Hospital Universitário. Métodos: Estudo de série de casos, com utilização de dados secundários disponibilizados pelo SINAN, cuja população do estudo incluiu os casos de SG e SC no Hospital Universitário Cassiano Antônio Moraes (HUCAM), em Vitória-ES, no período de 2015 a 2017. Resultados: No período estudado, foram notificados 49 casos de SG e 56 casos de SC. Observou-se que, das mães cujos filhos tiveram desfecho de SC, 44,6% obtiveram tratamento considerado inadequado durante o pré-natal, e 60% dos parceiros não receberam o tratamento. Dentre os conceptos, 01 foi natimorto, 01 evoluiu para óbito associado à sífilis e 01 óbito por outras causas. Conclusão: As características epidemiológicas analisadas apontaram uma predominância de mulheres jovens, pardas e com baixa escolaridade, além de um considerável número de mães e parceiros que não realizaram tratamento adequadamente, fatores que podem ter contribuído para o desfecho em sífilis congênita nos recém-nascidos e evidencia dificuldades no alcance da meta de eliminação da SC estabelecida pela Organização Mundial da Saúde (0,5 ou menos casos / 1.000 nascidos vivos). Os dados apresentados apontam a necessidade de incluir manejos clínicos diferenciados para essa população.
