Objetivos: Valorar los resultados obtenidos mediante la artroplastia de carbono pirolítico en pacientes con artrosis postraumática de las interfalángicas proximales. Material y Método: Se analizan un total de 25 pacientes (24 hombres, 1 mujer) con artrosis postraumática de la articulación interfalángica proximal (IFP), que fueron valorados clínica y radiográfica-mente, después de haber sido intervenidos mediante una artroplastia de carbono pirolítico. Todos los casos fueron operados por vía dorsal, siguiendo siempre el mismo protocolo rehabilitador postquirúrgico. El seguimiento medio de estos pacientes fue de 42 meses, con un mínimo de 12 meses y un máximo de 72 meses. Resultados: Comparando la escala analógica de dolor y el rango de movilidad en el pre y postoperatorio, se obtuvieron en 24 casos, unos buenos resultados. El único caso que fue considerado como malo, presentó una subluxación dorsal del componente distal del implante, que requirió de una reducción quirúrgica. En cuanto a los hallazgos radiográficos, en el 10 % de los casos se objetivaron finas bandas radiolúcidas peri-implante, si bien no se correlacionó este hallazgo con un deterioro clí-nico. Objectives:To measure the results obtained using a pyrocarbon arthroplasty in patients with post-traumatic arthrosis of the proximal interphalangeal joints. Materials and Methods: Twenty five patients (24 male, 1 female) with post-traumatic arthrosis of the proximal interphalangeal joints were studied, with a clinical and radiological assessment after undergoing a pyrocarbon arthroplasty. All cases were operated through a dorsal approach and underwent the same post-operative regime. Average follow-up was 42 months, range 12 months to 72 months. Results: The change in the analog pain scale and range of movement pre and pot-operative indicates that the results were good in 24 cases. The only result considered poor presented a dorsal subluxation of the implant, requiring surgical reduction. Radiologically, 10% of cases show fine radiolucent lines around the implant, but this finding did not correlate with a clinical deterioration. Conclusions: Replacement arthroplasty of the PIPJ using a pyrocarbon implant, presents a valid alternative in cases with post-traumatic arthrosis. However, larger series with longer follow up are required to validate this treatment.
Instituição: HOSPITAL UNIVERSITÁRIO GETÚLIO VARGAS Introdução As doenças inflamatórias intestinais são enfermidades crônicas, que afetam significativamente a qualidade e expectativa de vida dos pacientes. As formas mais comuns são retocolite ulcerativa idiopática (RCUI) e doença de crohn (DC). Objetivo Avaliar o perfil e a terapêutica dos pacientes em acompanhamento ambulatorial em um hospital universitário federal do Amazonas. Métodos Estudo tipo série de casos. Foram incluídos pacientes atendidos no período de janeiro de 2016 a janeiro 2017. Os itens analisados foram gênero, idade, tempo da doença, tipo da doença, extensão e localização baseada na classificação de Montreal, sendo na RCUI (E1 -Proctite ulcerativa; E2 -RCU do lado esquerdo; E3 -RCU extensa) e na DC (L1 -íleo; L2 -colônica; L3 ileocolônica; L4 TGI superior isolado), esquema terapêutico e complicações de doença. Resultados Foram incluídos 26 pacientes, sendo que 24 tinham o diagnóstico de RCUI e 2 de DC. A maioria era do gênero feminino com 15 pacientes. A média de idade foi de 40,8 (18 -67) anos. As localizações mais frequentes na RCUI foram E3 (12/25), E1 (9/24), E2 (6/25) e na DC observamos L1 (1/2) e L3 (1/2). A respeito da terapia na RCUI, a monoterapia com salicilatos foi a menos utilizada, correspondendo a 7 dos pacientes. A associação de medicamentos foi necessária em 17 casos, sendo a combinação mais frequente mesalazina e azatioprina. Quatro pacientes com RCUI faziam uso de adalimumabe, pois apresentaram falha terapêutica ao tratamento com mesalazina e azatioprina. Dos pacientes com DC, o tratamento foi feito apenas com azatioprina. Em relação ao tempo de doença média foi 9,8 (0,3 -35) anos. Nenhum paciente apresentou complicação com necessidade de cirurgia. Conclusão A doença inflamatória intestinal possui várias formas de apresentações, assim como os pacientes avaliados no presente estudo. Faz-se necessário o estudo detalhado da expressão da doença para que seja aplicada a terapêutica adequada e assim minimizar os riscos de complicações ao longo do seguimento.
