Artificial mechanical ventilation in the course of stroke presents a high mortality. Even today, with advanced knowledge in the physiopathology of the neurological event and modern techniques of respiratory support, it continues to be a problem of interest, which invites to reflect on the benefits of ventilation in this field. In Cuba, there is not enough research on the subject, however, mortality due to this cause is high. This phenomenon, which implies high costs and limited functional recovery in survivors, should continue to be studied.
Fundamento: El estudio de la gasometría arterial en el paciente críticamente enfermo es elemental para establecer la conducta a seguir. Entre sus alteraciones se destaca la acidosis metabólica, trastorno más común, letal y de mayor complejidad terapéutica en estos pacientes. Objetivo: Profundizar en los aspectos relacionados con la descripción de los algoritmos de interpretación gasométrica para la acidosis metabólica basada en la aproximación fisiológica Henderson Hasselbalch. Método: Se realizó una búsqueda en PubMed de los artículos que abordan la sistemática de lectura e interpretación de la gasometría arterial en la acidosis metabólica. Se consultaron artículos en español e inglés. Se expone en cada acápite una valoración sobre la impresión personal de los autores, basado en la práctica clínica diaria en el hospital donde laboran. Resultados: Los pasos lógicos establecidos para la interpretación de la gasometría arterial permiten inicialmente identificar la acidosis metabólica, luego los trastornos asociados y la compensación respiratoria de una forma sistemática. Conclusiones: El cálculo correcto de cada fórmula, independientemente del algoritmo escogido, representa la semiología del trastorno que establecerá el diagnóstico definitivo.
La hernia vesical se define como una porción de la vejiga urinaria o divertículo que sale a través del orificio de una hernia. Es una entidad rara presente en tan solo el 1 al 3 % de las hernias inguinoescrotales. Su diagnóstico se basa en la alta sospecha clínica y la realización de una prueba de imagen. Se presenta el caso de un paciente masculino de 43 años, con hernia vesical, la cual se manifestó como una masa inguinoescrotal izquierda, relacionada a sintomatología urinaria obstructiva, con necesidad de ejercer presión sobre el testículo homolateral para ejecutar la micción. Fue diagnosticado por uretrocistografía retrógrada y ultrasonido. En las placas antero-posterior y oblicua se evidenció hernia vesical, aproximadamente del 40 % por el conducto inguinal izquierda. En la placa post-miccional, ésta disminuyó su tamaño, pero sin vaciamiento completo.
La necrolisis tóxica epidérmica es una entidad caracterizada por desprendimiento de la epidermis. Su etiología más descrita es la medicamentosa, también está asociada a otras causas, fundamentalmente en pacientes que manifiesten una respuesta inmune inadecuada. El lupus eritematoso sistémico se ha interpretado como posible cofactor etiológico, siendo una presentación atípica y de difícil manejo. Se presenta un caso de una paciente femenina de 54 años de edad con antecedentes de lupus eritematoso sistémico y abandono de su tratamiento que ingresó por cuadro diarreico agudo y 48 horas después inició con lesiones en piel ulcerocostrosas diseminadas con despegamiento epidérmico generalizado, daño ocular grave y signo de Nikolsky positivo. A pesar de las medidas tomadas falleció a las 72 horas de diagnosticada debido a shock séptico. Se presenta el caso debido a que la necrolisis tóxica epidérmica ocurrió como una forma infrecuente y potencialmente fatal de complicación del lupus eritematoso sistémico en un centro hospitalario con poca experiencia en la atención a estos pacientes. La presencia de la enfermedad dermatológica, la ausencia de una causa evidente y el antecedente de lupus sin tratamiento llevaron a considerar que la paciente manifestó esta dermatopatía grave como complicación de su enfermedad de base.
