Hysterectomized women are more than 3.71 times more likely to have Gardnerella vaginalis. Gardnerella vaginalis predominated in the 41 to 50 year old age group of hysterectomized women, and in the group of non-hysterectomized women with less than 40 years of age. Maintenance of the vaginal pH is important to prevent Gardnerella vaginalis infections and hysterectomized patients must take preventive care against this infection.
A prevalência da vaginose bacteriana é estimada entre 25% e 36% das mulheres que procuram o atendimento ginecológico 5,15 , podendo provocar leucorréia vaginal com odor fétido ou, em alguns casos, ser assintomática.Esta infecção é diagnosticada quando três dos quatro seguintes critérios estiverem presentes: pH vaginal maior que 4,5; presença de "clue cells" ou células-alvo no fluído ou esfregaços vaginais; leucorréia fluída, cinza ou branca; ou teste com KOH positivo 15 . Embora a citologia cérvico-vaginal não seja o exame de escolha para avaliação da microbiota vaginal, este exame apresenta sensibilidade de 92% e especificidade de 97% para detecção de vaginose bacteriana 3 . Estudos demonstram uma evidente associação entre esta condição e complicações durante a gravidez, como parto prematuro, corioamnionite, endometrite pós-parto, complicações pós-parto para o recém-nato, e infecções após procedimentos ginecológicos, como histerectomia 8,11 , e um possível aumento do risco para contrair o vírus da imunodeficiência humana (HIV) 15 . Os fatores de risco para o desenvolvimento de vaginose bacteriana são idade, com maior incidência entre a terceira e a quinta década de vida 8 , número de parceiros sexuais, uso de duchas vaginais, uso de calças justas, uso de dispositivo intra-uterino (DIU) Esta é uma infecção associada à alteração do pH vaginal. O pH vaginal está relacionado ao pH endocervical em mulheres na pré e pós-menopausa, e o pH vaginal nas mulheres histerectomizadas é mais ácido do que nas não histerectomizadas 10 . O objetivo de nosso trabalho é avaliar a freqüência de Gardnerella vaginalis em esfregaços cérvico-vaginais de pacientes com histerectomia total comparando com os esfregaços de pacientes não histerectomizadas. MÉTODOS Seleção das pacientesRealizou-se um estudo retrospectivo no Laboratório de Anatomia Patológica da Unoeste, sendo revisados os laudos de 39.900 citologias cérvico-vaginais no período de 1999 a 2004.
