Leerkrachtinterventies voor de aanpak van adhd in de klas: een overzicht van effectstudiesKarlijn Pieterse, Marjolein Luman, Arga Paternotte en Jaap Oosterlaan samenvatting Attention deficit hyperactivity disorder (adhd) wordt gekenmerkt door aandachtstekort, hyperactiviteit en impulsiviteit. Daarnaast hebben kinderen met adhd vaak motivatieproblemen, problemen met plannen en organiseren, en problemen op sociaal-emotioneel gebied, zoals beperkte sociale vaardigheden. Juist op school lopen deze kinderen tegen problemen aan en de aanpak van adhd in de klas is dan ook van groot belang. Dit overzichtsartikel beschrijft: (1) een inventarisatie van leerkrachtinterventies voor de aanpak van adhd in de klas en (2) de resultaten van effectstudies naar deze interventies op adhd-symptomen, aan adhd gerelateerd probleemgedrag, en het sociaal-emotioneel en schools functioneren. De inventarisatie omvat vier opzichzelfstaande leerkrachtinterventies en zeven multimodale interventies. In de effectstudies zijn de interventies op effectiviteit onderzocht door de vergelijking met een controleconditie: een onbehandelde groep of een groep die alleen medicatie kreeg. De resultaten laat zien dat leerkrachtinterventies gebaseerd op gedragstherapeutische methoden effectief zijn in het verminderen van adhdsymptomen, het verminderen van aan adhd gerelateerd probleemgedrag en in het vergroten van de sociale vaardigheden, waarbij een aantal studies een effect laat zien dat nagenoeg gelijk is aan dat van medicatie. Duidelijk is dat de leerkracht een essentiële rol kan spelen in de aanpak van aan adhd gerelateerd probleemgedrag in de klas.Drs. K. Bij de behandeling van kinderen met ADHD spelen ouders (en leerkrachten) een cruciale rol. Zij zijn het immers die de meeste tijd met het kind doorbrengen en derhalve in de gelegenheid zijn het gedrag van het kind te beı¨nvloeden. Ook op beleidsniveau dragen ouders tegenwoordig hun steentje bij in het denken over diagnostiek en behandeling van deze aandoening. Bij de opstelling van het advies van de Gezondheidsraad omtrent ADHD, dat in november 2000 verscheen (Gezondheidsraad, Publicatie nr. 2000/24) benoemde de minister van Volksgezondheid een vertegenwoordiger van de vereniging Balans* tot adviseur van de ingestelde commissie. In dit artikel gaat het over de situatie van ouders van kinderen met ADHD en de rol die een oudervereniging kan spelen als belangenbehartiger. ADHD in het gezinKinderen en jongeren met ADHD roepen door hun rusteloze, impulsieve gedrag veel ergernis op bij mensen in de omgeving. De algemene klacht van hun ouders is dat ze geen vat hebben op het onvoorspelbare gedrag van hun kind omdat het niet reageert op de gewone opvoedkundige aanpak. Kritiek en vooroordelen uit de sociale omgeving worden als pijnlijk ervaren en hebben een negatieve invloed op het competentiegevoel van de ouders (Mulder en Paternotte, 1998). Grote frustratie over henzelf en hun kind brengt het risico met zich mee dat ouders interacties met hun kind gaan vermijden en zichzelf daarmee ook de kans op positieve ervaringen met hun kind ontnemen. Dit legt een niet geringe schuldenlast bij ouders die in principe wel anders zouden willen. Onzekere, uitgeputte ouders vormen een prooi voor sensatiebeluste media en (alternatieve) behandelaars die al of niet op materie¨le of immaterie¨le winst uitzijn.Meer en meer komt de laatste jaren uit onderzoek (vooral naar tweelingen en adoptie) naar voren dat er bij ADHD sprake is van een sterke genetische factor. De kans op ADHD bij broertjes of zusjes van een kind met ADHD is drie tot vijf keer zo hoog ten opzichte van de gewone populatie. Bij tweedegraads familieleden is dat een factor twee (Gezondheidsraad, Publicatie nr. 2000/ 24). Hiermee is ook zicht gekomen op de problematiek van volwassenen met ADHD. Meer bekendheid met de specifieke zaken die in deze gezinnen kunnen spelen zal mogelijk ernstige frustraties, en daarmee de kans op secundaire problematiek bij de gezinsleden, kunnen voorkomen. Uitgebreide voorlichting, verbetering van zorg en onderwijs, alsook een betere samenwerking tussen de verschillende actoren in het veld, worden voor deze doelgroep noodzakelijk geacht. Interacties binnen het gezinUit onderzoek naar interacties tussen ouder en kind en interacties tussen kinderen onderling in gezinnen met ADHD-problematiek, is bekend dat deze gezinsleden vaker stressvol reageren en meer negatieve interacties hebben met hun omgeving dan gezinsleden van kinderen zonder ADHD ( Barkley 1990).
