Background & Objectives:The study was conducted with objective to compare the horizontal condylar guidance (HCG) obtained by protrusive interocclusal records and panoramic radiographic images in completely edentulous patients.Materials & Methods:The horizontal condylar guidance was measured in 25 completely edentulous patients by protrusive interocclusal records using zinc oxide eugenol paste through a face bow transfer (HanauTM Spring bow, Whip Mix Corporation, USA) to a semi-adjustable articulator (HanauTM Wide-Vue Articulator, Whip Mix Corporation, USA). In the same patients, HCG was traced in the panoramic radiograph. The angles formed by the intersection of two lines: Frankfurt’s horizontal plane and posterior slope of articular eminence was measured using protractor to represent the horizontal condylar guidance angle on each side.Results:The mean difference between the horizontal condylar guidance angles values obtained using protrusive interocclusal record and panoramic radiograph was 2.68 degrees and 3.40 degrees for the right and the left side respectively, with the panoramic radiograph values being higher. This difference between the values was found to be highly significant between the two methods for the right side (t = 2.70, p = 0.012) and left side (t = 3.69, p = 0.001). A significant positive correlation was found between the horizontal condylar guidance obtained from protrusive interocclusal record and panoramic radiograph for the right (r = 0.643, p = 0.001) and left sides (r = 0.622, p = 0.001) separately.Conclusion:The panoramic radiographic tracing can be used to calculate the mean horizontal condylar guidance in the completely edentulous patients and these values can be used to programme semi-adjustable articulators avoiding the cumbersome process of obtaining protrusive interocclusal records.
Introduction: Partial edentulousness is a dental arch in which one or more but not all natural teeth are missing due to dental caries, periodontal problems, trauma etc. It causes difficulty in chewing food, alteration of speech and unpleasant esthetics which adversely affect the general health and quality of life. Methods: This study was conducted in the Department of Prosthodontics, College of Dental Surgery, Gandaki Medical College, Pokhara from November 2017 to March 2018. All patients above the age of 15 years presenting with partial loss of dentition and willing to give informed consent were included in the study. Data regarding age, gender, missing teeth, cause of permanent tooth loss and denture status were collected from 342 patients. Results: The most common cause of permanent tooth loss was dental caries (58.2%) followed by periodontal (18.1%), dental caries and periodontal (9.4%), trauma (7.9%), dental caries and trauma (2.9%), periodontal and trauma (2.3%) and others (1.2%).The distribution of partially edentulous areas in maxillary arch (35.7%), mandibular arch (45.9%) and both arches (18.4%).The most common type of partial edentulism was found to be Kennedy’s class III followed by class II, class I and class IV in both the arches. Conclusion: The most common cause of permanent tooth loss was dental caries. Mandibular arch was more affected than the maxillary arch. The most common type of partial edentulism was Kennedy’s class III.
Introduction: Endodontics is specialty field, but the majority of endodontic treatment in Pokhara is provided by the general dental practitioners. The aim of this study was to assess the practice of endodontic treatment protocols among general dental practitioners in Pokhara, Nepal. Materials and Methods: A cross sectional questionnaire based study was conducted among all the general dental practitioners working in various government, private hospital and clinic of Pokhara. Information about the materials and techniques used in endodontic treatment was collected and descriptive statistical analysis was done. Results: Out of 89 respondents, only 82 completely filled the questionnaires which were included in the study. Most general dental practitioners (96.34%) regularly treat single rooted endodontic cases. Only 10.97% occasionally use rubber dam during endodontic treatment. Most general dental practitioners (96.34%) used radiographic technique for working length determination. Ninety two percent used stainless steel hand files and step back was the choice of technique by 90.24%. Sodium hypochlorite irrigation and calcium hydroxide intra canal medicaments were used by 93.9% and 98.78% of general dental practitioners respectively. 75.6% used zinc oxide eugenol as root canal sealer and 95.12% obturated root canals with lateral compaction technique. The common complication encountered was mid treatment pain. They seem to overuse antibiotics in cases requiring endodontic therapy. Only 52.43% used autoclave for sterilization of endodontic files. About 92.68% felt the need of further endodontic training and 74.39% preferred post graduate program. Conclusion: This study indicates that many general dental practitioners of Pokhara are not following well acknowledged endodontic quality guidelines; hence there is a need for further endodontic training.
Introduction: Endodontics is the study of prevention, diagnosis and treatment of diseases or injuries to the dental pulp. The ultimate goal of modern dental care is tooth preservation and root canal therapy/treatment is an available therapeutic strategy to retain teeth. The aim of the study is to find the prevalence of patients visiting a tertiary care center who had endodontic indications. Methods: A descriptive cross-sectional study was conducted over one year from April 15, 2020, to February 15, 2021 at a tertiary care hospital. The ethical approval was obtained from the Institutional Review Committee of Nobel Medical College (Reference number: 409/2020). Convenience sampling method was used. The patients admitted for alcohol use and related problems in the Psychiatry Department, Nobel Medical College Teaching Hospital, Biratnagar were included. Data entry was done using Microsoft Excel and statistical analysis was done by using Statistical Packages of Social Sciences Version 16.0. Point estimate at 95% Confidence Interval was calculated along with frequency and percentage. Results: Out of 1740 patients, 516 (29.66%) (95% Confidence Interval= 21.46% - 27.51%) had endodontic indications. Symptomatic irreversible pulpitis 306 (59.30%) was the most prevalent pulpal disease. Maxillary teeth 300 (58.13%) had more endodontic diseases. While in individual dental elements most affected by endodontic diseases was the mandibular molar teeth 149 (28.87%). Females 348 (67.44%) were predominant for demanding endodontic management than males 168 (32.5%). Conclusions: More female patients and of younger age group in this study population demanded endodontic treatment. Irreversible pulpitis was responsible for the majority of the cases treated and more affected were the posterior teeth
Rehabilitation of severely resorbed maxilla poses a clinical challenge for fabrication of a successful maxillary complete denture. The increased interridge distance often results in a heavy maxillary complete denture that further reduces the retention and stability of the denture. This case report describes the rehabilitation of an edentulous patient with resorbed maxillary and mandibular ridges and an increased interridge distance by hollow maxillary complete denture so as to reduce the weight of the denture which enhances the retention and stability of the denture.
