Introdução: A técnica de aspiração percutânea de espermatozóides do epidídimo (PESA) tem sido discutida amplamente na literatura. Pontos discrepantes são relatados, como o número de espermatozóides na etiologia estudada, idade dos pacientes e incidência dos achados de azoospermia. No trabalho em questão, estudamos estas características levantadas e fundamentamos a utilização e importância desta técnica nos dias de hoje. Material e Métodos: Foram avaliados 265 pacientes submetidos à técnica de aspiração percutânea de espermatozóides do epidídimo em nível ambulatorial com anestesia local. Paralelamente, realizou-se estudo clínico para caracterização da etiologia da infertilidade e análise dos dados obtidos em laboratório. Resultados: A azoospermia obstrutiva esteve presente em 229 pacientes (86,41%) dentre os quais o perfil de pósvasectomia apresentou maior incidência, com 101 casos (44,10%). A azoospermia secretora esteve presente com 34 casos (12,83%). Por fim, a síndrome de Kartagener e o trauma raque-medular apresentaram 1 caso (0,38% cada). Conclusões: A azoospermia obstrutiva mostrou-se ser o principal achado no presente estudo. Ocorreram casos interessantes de fibrose cística, síndrome de Kartagener e trauma raque-medular. Por outro lado, é variável o número de espermatozóides obtidos por esta técnica. Concluímos assim, que a aspiração percutânea de espermatozóides do epidídimo poderá se firmar ainda mais com o advento de novos conhecimentos da anátomofisiologia testicular epididimária.
