Rev bras Coloproct, 2011;31(1): 17-25. tumores de células em anel de sinete, 2 (0,4%) tumores de pequenas células, 45 (8,6%) tumores mucinosos, 1 (0,2%) tumor basaloide, 3 (0,6%) do tipo escamoso, 6 (1,2%) adenoescamosos, 1 (0,2%) tumor classificado como indiferenciado e 3 (0,6%) peças em que não foi identificado tumor residual. O aspecto macroscópico dos tumores foi 130 (25%) lesões polipoides, 47 (9,0%) sésseis, 18 (3,5%) em placa, 176 (33,8%) úlcero-infiltrativos, 138 (26,5%) anulares constritivos, 3 (0,6%) com aspecto difuso, 1 (0,2%) ulcerado e 8 (1,5%) com outros aspectos macroscópicos. Do ponto de vista da diferenciação celular, esta série englobou três (0,6%) tumores indiferenciados, 34 (6,5%) pouco diferenciados, 396 (76%) moderadamente diferenciados, 83 (15,9%) bem diferenciados e 5 (1,0%) em que esta classificação não se aplicava. o número médio de linfonodos dissecados por peça cirúrgica foi de 22,13 (±14,27). o número médio de linfonodos com metástases foi de 2,57 (±5,34). Dos tumores avaliados, 130 (25,0%) apresentaram invasão venosa, 267 (51,2%) invasão linfática e 141 (27,1%) invasão neural. com relação à resposta linfocítica peritumoral, 163 (31,3%) não apresentavam resposta, 264 (50,7%) discreta resposta, 76 (14,6%) resposta moderada e 18 (3,5%) resposta acentuada. RESUMO: Introdução: A incidência e a mortalidade por câncer colorretal (CCR) têm apresentado, em todo mundo, uma tendência ao crescimento, em especial em países desenvolvidos e em áreas urbanas de países menos desenvolvidos. O estudo das características anatomopatológicas do tumor é importante para o estadiamento e a definição da terapêutica a ser empregada. O objetivo deste estudo é avaliar as características anatomopatológicas de peças cirúrgicas de ressecções colorretais por neoplasias no Hospital dasA classificação T dos tumores avaliados foi a seguinte: 25 (4,8%) pt1; 105 (20,2%) pt2; 316 (60,7%) pt3; 66 (12,7%) pt4; 7 (1,3%) t0 e dois (0,4%) ptis (Figura 3). Já a classificação N foi a seguinte: 112 (21,5%) pn1; 95 (18,2%) pn2; 8 (1,5%) pn3, 93 ( os tumores tinham características semelhantes tanto em homens como em mulheres, sem diferença em relação à idade, localização, dimensões do tumor, diferenciação tumoral, invasão venosa, linfática e neural, classificação PT e pN, Astler-Coller e Dukes, nú-mero de linfonodos totais e com metástase (tabela 1).os pacientes com tumores do cólon direito apresentaram idade maior que os pacientes com tumo- . nesse estudo, características como prevalência dos tumores à esquerda e idade de acometimento foram semelhantes a esta série. nossos resultados são divergentes se comparados a esse estudo em relação à prevalência de tumores por gênero e classificação de Dukes. INTRODUÇÃO: A partir de 1991, a videolaparoscopia começou a ser considerada no tratamento de doenças colorretais. O aprimoramento da técnica cirúrgica associado aos benefícios encontrados em diversos estudos publicados levou a modificações nas perspectivas da videolaparoscopia. A partir da publicação do estudo COST as ressecções oncológicas laparoscópicas foram reconhecidas como alternativa viável, com resultados semelhantes à cirurgia convencional. PACIENTES E MÉTODOS: Realizou-se pesquisa através de formulário específico e consulta a prontuários dos principais serviços de coloproctologia de Belo Horizonte. Avaliando-se sexo, idade, indicação cirúrgica, procedimento realizado, técnica laparoscópica, complicações, conversão, estadiamento e recidiva (no caso de neoplasias). RESULTADOS: Foram levantados dados sobre 503 cirurgias colorretais laparoscópicas: 347 (68,9%) em mulheres e 156 (31,1%) homens. A técnica cirúrgica foi totalmente laparoscópica em 137 casos, vídeo-assistida 245 casos. O procedimento mais realizado foi a retossigmoidectomia (41,1%), seguido pela colectomia direita (12,5%), colectomia esquerda (6,9%). Doenças benignas foram responsáveis por 259 (51,5%) casos, destes as principais indicações cirúrgicas foram endometriose 126 (48,6%), pólipos 40 (15,4%), doença diverticular 30 (11,6%). Das 240 cirurgias realizadas por doenças malignas as mais frequentes foram retossigmoidectomia 102 (42,5%), colectomia direita 46 (19,1%), colectomia esquerda 18 (7,5%), amputação abdominoperineal 18 (7,5%). Houve 54 conversões (10.7%) dos casos, 12,9% (31/240) nos casos de neoplasias, 8,5% (22/259) nos de doenças benignas. Complicações sistêmicas ou cirúrgicas ocorreram em 31 (6,1%) e 56 (11,1%) casos, respectivamente. Foram registrados onze (2,18%) óbitos nos primeiros 30 dias após a cirurgia. CONCLUSÃO: O estudo atual foi o primeiro levantamento da implantação de cirurgias colorretais laparoscópicas realizado de forma multicêntrica em Minas Gerais. Os dados levantados são consistentes com registros nacionais de videocirurgia colorretal, mostrando a eficiência do método de aprendizado com realização de cirurgias com tutor. Além disso, que pequena parte das cirurgias colorretais são realizadas por via laparoscópica no estado, restritos apenas a centros especializados, sobrecarregando esses serviços e limitando o acesso para a população.
RESUMO Objetivo: avaliar os fatores associados ao não fechamento de ileostomia protetora após ressecção anterior do reto com excisão total do mesorreto por câncer retal, a morbidade associada ao fechamento destas ileostomias e a taxa de estomia permanente em pacientes com adenocarcinoma retal. Métodos: estudo retrospectivo de 174 pacientes consecutivos com diagnóstico de tumores retais, dos quais 92 foram submetidos à ressecção anterior do reto com intenção curativa, anastomose coloanal ou colorretal e ileostomia de proteção. Foi realizada análise multivariada visando a determinar os fatores associados à permanência definitiva da estomia, assim como o estudo da morbidade nos que se submeteram à reconstrução do trânsito. Resultados: no período de seguimento de 84 meses, 54 dos 92 pacientes avaliados (58,7%) tiveram a ileostomia fechada e 38 (41,3%) permaneceram com a estomia. Entre os 62 pacientes que tiveram a ileostomia fechada, 11 (17,7%) apresentaram algum tipo de complicação pós-operatória: três com deiscência de anastomose ileal, cinco com obstrução intestinal, dois com infecção de ferida operatória e um com pneumonia. Oito destes pacientes necessitaram de um novo estoma. Conclusão: de acordo com a análise multivariada, os fatores associados à permanência da estomia foram fístula de anastomose, presença de metástases e fechamento da ileostomia durante quimioterapia.
A síndrome do aprisionamento da artéria poplítea caracteriza-se pela compressão desta artéria sendo a principal causa de claudicação intermitente em jovens. Homem, 18 anos, branco, apresentava parestesia, frialdade e palidez do pé direito, iniciada 24 horas após exercício físico. Em membro inferior direito, ausência de pulsos tibial posterior e dorsal do pé. À flexão dorsal e flexão plantar forçadas, houve diminuição dos pulsos tibial posterior e dorsal do pé à esquerda. Tratado cirurgicamente, o paciente apresentou pulso em ambas as artérias. A síndrome é mais frequente em homens e a prevalência varia entre 0,16 e 3,5%. O aprisionamento da artéria poplítea tipo III é mais comum. A falta de tratamento pode levar à embolia, trombose e aneurismas pós-estenóticos. Esta síndrome deve ser lembrada como causa de dor na perna, especialmente em homens jovens e de prática esportiva intensa.
