The insulin tolerance test (ITT) is considered the test of choice for the diagnosis of GH deficiency (GHD). However, in patients with contraindications to ITT, alternative provocative tests must be used with appropriate cut-offs. The glucagon stimulation test has proved to be a safe, low-cost and effective means of stimulating GH secretion, and therefore can be considered as a suitable alternative to the ITT. We have studied the GH response to the glucagon test in 33 patients with known pituitary disease, 12 males and 21 females, aged between 21 and 60 yr (41.18 +/- 9.47 yr); 5 had isolated GHD and 28 had panhypopituitarism. We also evaluated a control group of 25 individuals, matched for age and sex (8 males and 17 females), aged between 20 and 60 yr (39.28 +/- 12.10 yr). They were selected via the ITT if their peak GH response was > 5.0 ng/ml. GH peak after glucagon was significantly lower in the group of patients compared to the control group (0.49 +/- 0.85 vs 8.69 +/- 5.85 ng/ml, p = 0.0001). Receiver-operating characteristic (ROC) plot analyses of the control and GHD group showed an area under the curve of 0.982 for GH peak response to glucagon. The response value of 3.0 ng/ml showed the best pair of sensitivity (97%)/specificity (88%), and was chosen as the cut-off defining GHD. After evaluation of positive predictive values (PPV) and negative predictive values (NPV) through simulation of different prevalences of the disease, we concluded that the cut-off point of 3.00 ng/ml maximizes both PPV and NPV (100%). In conclusion, we have shown that the glucagon stimulation test has a good performance and great diagnostic accuracy for the diagnosis of GHD.
O objetivo deste estudo foi analisar o acesso a e a utilização de serviços odontológicos no Estado do Rio de Janeiro, Brasil, em 1998, a partir da Pesquisa Nacional por Amostra de Domicílios (PNAD 1998). A população de estudo foi composta por 7.756 indivíduos subdivididos em três estratos etários (15 a 19, 35 a 44 e 65 a 74 anos). A avaliação do acesso baseou-se na pergunta sobre a última vez que o entrevistado foi ao dentista. O fato de um indivíduo nunca ter ido ao dentista na vida foi considerado como indicativo de restrição de acesso, analisado segundo variáveis demográficas, sócio-econômicas e epidemiológicas. As proporções de indivíduos que nunca consultaram dentista foram 7,6%, 1,8% e 2,6% respectivamente, entre os jovens, adultos e idosos. Os resultados sugerem que indivíduos menos favorecidos em termos sócio-econômicos apresentavam maior limitação de acesso, ou seja, os menos escolarizados (jovens e adultos), com menor renda per capita (jovens e idosos) e com menor posse de eletrodomésticos (adultos). Entre os jovens e os adultos, a posse de plano de saúde mostrou-se inversamente associada à restrição de acesso.
Objectives:To evaluate the impact of subclinical hypothyroidism (sHT) treatment on health--related quality of life (QoL), psychiatric symptoms, clinical score, and muscle function. Materials and methods: In this randomized double-blind study, patients were assigned either to treatment (n = 35) or placebo (n = 36). Clinical and psychiatric symptoms were assessed by the Zulewski, Hamilton and Beck scales. QoL was assessed by the SF-36 questionnaire. Assessments of quadriceps (QS) and inspiratory muscle (IS) strength were performed by a chair dynamometer and a manuvacuometer. Results: Treatment improved IS (+11.5 ± 17.2; p = 0.041), as did QoL domains "Pain" and "Role Physical" (+19.7 ± 15.2, 0.039 and +22.1 ± 47.5, p = 0.054; respectively). Clinical and psychiatric symptoms showed similar responses to both interventions. Conclusions: sHT treatment improved IS and physical aspects of QoL, despite no impact in other muscle parameters. Este artigo apresenta uma investigação preliminar da presença da seleção adversa e do risco moral (moral hazard) na demanda ativa por planos de saúde no Brasil, a partir dos dados da PNAD/98. O presente estudo compara indivíduos cujas coberturas decorrem do vínculo de trabalho, com aqueles cujos planos resultam de uma demanda individual às empresas que os comercializam e os que não têm acesso a esquemas assistenciais alternativos ao SUS. A elaboração de uma tipologia de planos de saúde, combinada com variáveis relacionadas com as condições de saúde, utilização de serviços e gastos com saúde, sugere a existência de falhas de mercado. A percepção de uma condição de saúde mais desfavorável parece estar associada à busca de cobertura e o tipo de cobertura com o maior uso de serviços de saúde. Quando analisadas através de dois modelos de regressão logística com múltiplos controles, onde a variável de desfecho é indicadora de seleção adversa ou moral hazard, essas diferenças se atenuam, com exceção dos gastos com saúde. Os resultados não evidenciam uma inquestionável assimetria de informações, mas sinalizam a necessidade de aprofundar o conhecimento sobre as relações entre morbidade, utilização de serviços de saúde, gastos com saúde e tipo de cobertura.
SUMMARY BackgroundAlthough the stomach is the most frequent site of intestinal lymphomas, few data are available on both clinical endoscopic presentation of gastric lymphoma and possible differences between low-grade and high-grade lymphomas.
Many studies have found clinical and metabolic alterations in subclinical hypothyroidism, however, there are disagreements about the benefits of levothyroxine therapy. The objective of the present study was to analyze the effects of 6 months of treatment on the lipid profile of patients with subclinical hypothyroidism. A randomized double blind, placebo-controlled clinical assay was conducted. Sixty patients were enrolled in stratified random allocation by TSH levels that generated similar groups in average: free thyroxine levels, lipid levels, age, clinical score, and sedentary. At 6 months, 18 patients in the levothyroxine and 20 in the placebo group were reevaluated and a fall in all atherogenic lipid variables was observed with treatment. The TC and LDL-c variations (-22.6+/-37.2 and -18.5+/-34.6 mg/dl, respectively) in the group that received LT4 were statistically different (p=0.023 and p=0.012) from those occurring in the placebo group (+7.3+/-37.1 and +14.7+/-40.6 mg/dl). Baseline characteristics associated with better improvement in the levels of TC and LDL-c were the presence of TPO-Ab, TSH levels >8.0 microUI/ml, Body Mass Index >or=25 kg/m2, and the presence of menopause. We concluded that treatment with dose-adjusted levothyroxine reduced atherogenic lipid levels in some patients. Further studies to determine the effects of LT4 replacement in specific subgroups of SH patients are still necessary, especially in patients with TSH <8.0 microUI/ml.
A b s t r a c t Objective: To evaluate the prevalence of psychiatric disorders and symptoms in patients with subclinical hypothyroidism. Method: Ninety-four outpatients with at least two elevated serum thyrotrophin levels (> 4 μU/ml) and normal FT 4 , and 43 euthyroid outpatients, both groups from HUCFF-UFRJ, were evaluated. Psychiatric diagnosis was based on the Structured Clinical Interview Diagnostic for the DSM-IV axis I (SCID-I/DSM-IV), the psychopathological symptoms on Hamilton anxiety and depression scales, and the Beck Inventory. Results: Our data showed an increased prevalence of psychiatric disorders in the subclinical hypothyroidism patients when compared to the euthyroid group (45.7% vs 25.6%; p = 0.025), mood disorders being the most frequent. The prevalence of depressive symptoms based on Beck's Scale among subclinical hypothyroidism patients was about 2.3 times higher than among euthyroid ones (45.6% vs 20.9%, p = 0.006). Anxiety symptoms were also more frequent among subclinical hypothyroidism patients (87.0% vs 60.5%, p < 0.001), mainly clinical anxiety (44.6% vs 23.3%; p = 0.001). Conclusion: Our results showed a significant association of subclinical hypothyroidism with psychiatric disorders and an increased frequency of subsyndromic depression and anxiety symptoms in subclinical hypothyroidism in relation to the euthyroid group.Descriptors: Hypothyroidism; Anxiety disorders; Mood disorders; Symptoms; Prevalence Resumo Objetivo: Avaliar a prevalência de sintomas e alterações psiquiátricas em pacientes com hipotireoidismo subclínico. Método: Foram estudados 94 pacientes ambulatoriais com pelo menos duas dosagens plasmáticas elevadas de tireotropina (> 4 μU/ml) e com T 4 livre normal e, 43 eutireoidianos, ambos os grupos do HUCFF-UFRJ. Para diagnóstico psiquiátrico foi utilizada a entrevista clínica estruturada do eixo I (SCID-I/DSM-IV) e, para sintomas psicopatológicos, as escalas de ansiedade e depressão de Hamilton (HAM-A e HAM-D) e inventário de Beck. Resultados: Encontramos uma prevalência aumentada de transtornos psiquiátricos no hipotireoidismo subclínico em comparação ao grupo eutireoidiano (45,7% vs 25,6%; p = 0,025), sendo o transtorno do humor o de maior freqüência. Sintomas de depressão no grupo com hipotireoidismo subclínico foram cerca de 2,3 vezes mais freqüentes que entre os eutireoidianos (45,6% vs 20,9%; p = 0,006) quando o instrumento utilizado foi a escala de Beck. Da mesma forma, sintomas de ansiedade também foram mais freqüentes no hipotireoidismo subclínico (87,0% vs 60,5%; p < 0,001), principalmente ansiedade clínica (44,6% vs 23,3%; p = 0,001). Conclusão: Os resultados indicaram uma associação do hipotireoidismo subclínico com os transtornos psiquiátricos, além de uma freqüência aumentada de sintomas de depressão e ansiedade subsindrômicos em relação ao grupo eutireoidiano.
