Resumo Introdução Cirurgiões bucomaxilofaciais frequentemente tratam fraturas do complexo zigomaticofacial e, com isso, os sinais e sintomas auxiliam o profissional a estabelecer o diagnóstico e a conduta frente a cada caso. A presença de alteração de sensibilidade é um sintoma frequente neste tipo de trauma. Objetivo Avaliar comparativamente a presença e as alterações de sensibilidade subjetiva e sensibilidade objetiva após fraturas de zigoma. Metodologia Foram selecionados 14 pacientes com fraturas unilaterais de zigoma. A sensibilidade subjetiva foi avaliada por meio de um questionário e a sensibilidade objetiva, mensurada por meio do monofilamento de Semmes-Weinstein. Resultado Os resultados mostraram alteração de sensibilidade em 13 pacientes (92,84%); destes, oito pacientes (57,13%) apresentaram alterações de ordem subjetiva e dez (71,42%), de ordem objetiva. Alterações concomitantes de sensibilidade subjetiva e sensibilidade objetiva foram encontradas em cinco pacientes (35,71%). Afetados exclusivamente por um tipo de alteração de sensibilidade somaram oito pacientes (57,13%); destes, cinco pacientes (35,71%) apresentaram somente alterações objetivas e três pacientes (21,42%), apenas alterações subjetivas. A única queixa de sensibilidade subjetiva encontrada foi a hipoestesia, com sete casos (50%). Conclusão As alterações de sensibilidade são frequentemente encontradas após fraturas de zigoma, existindo uma forte correlação entre a perda da percepção subjetiva e a perda da sensibilidade objetiva; porém, ocorre predominância de alterações de ordem objetiva.
Introduction: The side effects and adverse reactions related to cancer therapies may cause significant alterations in the oral cavity, discomfort or even severe pain in parts of the body, patient’s nutritional deficiency, delay in the administration of oncologic drugs or dose limitation, an increase of hospitalization time and of the related expenses, as well as a decrease in the patient’s quality of life. Objective: The purpose of this study was to determine the deleterious effects of cancer therapies in the oral cavity. Material and methods: Data was gathered from medical records of the treatment of 643 cancer patients at the São José Hospital, in Joinville, state of Santa Catarina, from January to September 2012. Among the records selected for this study, 59.41% were female patients, with a mean age of 51 to 60 years. Results: Oral complications were reported in 72.47% of the patients, and the complication with the highest prevalence was mucositis (14.62%) followed by dry mouth (10.58%). Most of the patients had not received dental care prior to the cancer treatment, and no dental record was found. Conclusions: Several different oral disorders were reported as a result of cancer treatment, and a significant number of patients needed dental evaluation prior to the treatment.
Objetivo: o objetivo desta revisão de literatura é apresentar os diferentes protocolos clínicos randomizados utilizando o laser de baixa intensidade no manejo da IntroduçãoOs efeitos do laser de baixa intensidade podem levar a uma aceleração no processo de cicatrização de feridas, os quais são atribuídos ao estímulo de diversos processos metabólicos 1 , mediante a conversão da energia luminosa que ele aporta, transformando, por meio de processos bioquímicos e fotofísi-cos, a luz em energia útil para a célula 2 .A conversão fotoquímica é absorvida pelos fotorreceptores celulares, onde a molécula que absorve a luz transfere essa energia para outras moléculas, resultando na ativação celular e em reações quími-cas no tecido circunvizinho. Essa energia celular disponível, envolvendo as mitocôndrias e demais estruturas celulares, provoca o fenômeno de biomodulação celular 3,4 . O Tumor Odontogênico Ceratocístico (TOC) é um cisto benigno, porém agressivo. Surge a partir dos restos celulares da lâmina dentária, e cerca de 60% dos casos são diagnosticados em indivíduos entre 10 a 40 anos. A mandíbula é acometida em 60% a 80% dos casos, principalmente na região de ramo e corpo. O presente trabalho tem como objetivo relatar um caso clínico de TOC tratado cirurgicamente e proservado durante oito anos. Paciente do sexo feminino, 20 anos, compareceu ao ambulatório de Cirurgia e Traumatologia Bucomaxilofacial do Hospital Municipal São José, para avaliação dos terceiros molares. Após avaliação clinica e radiográfica, verificou-se a presença de extensa área radiolúcida em região de corpo e ângulo de mandíbula, envolvendo os dentes 37 e 38. Inicialmente, realizou-se punção aspirativa da lesão, que possibilitou o diagnóstico de TOC. Posteriormente, foi realizada ressecção total da lesão e exodontia dos dentes envolvidos. Após um ano de acompanhamento clínico e radiográfico, foi instalado um implante osseointegrado na região do segundo molar inferior esquerdo. O caso foi proservado através de radiografia panorâmica por um período de oito anos sem recidiva local. Com isso, podemos concluir que o tratamento cirúrgico do TOC e a posterior instalação do implante foram eficazes.
O Odontoma complexo é um tumor odontogênico benigno, formado por tecidos dentários que se apresenta como uma massa calcificada, de forma arredondada ou irregular, e localizado preferencialmente na região de molares. O tratamento da lesão consiste na remoção cirúrgica. Este trabalho tem como objetivo relatar um caso clínico de Odontoma complexo, tratado e acompanhado durante seis anos. Paciente do sexo feminino, 19 anos, leucoderma, compareceu ao ambulatório do Serviço de Cirurgia e Traumatologia Bucomaxilofacial do Hospital Municipal São José (HMSJ), relatando aumento de volume em mandíbula e dificuldade de abertura bucal. Na avaliação clinica e radiográfica, suspeitou-se de um Odontoma, sendo indicada a biopsia da lesão. Durante a enucleação da lesão, ocorreu fratura patológica da mandíbula, sendo realizado bloqueio maxilo-mandibular por 30 dias. A peça cirúrgica foi encaminhada para o serviço de patologia do HMSJ que confirmou a hipótese diagnóstica. Foram realizados acompanhamentos clínico e radiográfico nos períodos de 30, 90, 180 e 360 dias do pós-operatório. Passados seis anos da cirurgia, uma nova avaliação clínica e radiográfica foi realizada para observar a consolidação da fratura e a remodelação óssea no local da lesão. Baseado nesse tempo de acompanhamento pode-se concluir que o tratamento cirúrgico do Odontoma complexo foi eficiente.
