Because of the physiological changes that occur during pregnancy and lactation, diagnostic ultrasound of the breast during these periods is a challenge for physicians. Therefore, a comprehensive understanding of imaging, anatomy, and physiology of the breast is important to effectively diagnosing diseases that can arise in women who are pregnancy or lactating. The aim of this article was to review the physiological changes that occur in the breasts during pregnancy and lactation, as well as to describe the main features of the breast diseases that occur most frequently during these periods.
Artigos originaisResumo OBJETIVO: analisar pacientes com candidíase vulvovaginal quanto a sintomatologia, fatores de risco e resultados da cultura anal, identifi car a freqüência de Candida albicans e não C. albicans e correlacionar as colonizações anal e vaginal. The yeasts were identifi ed using the classic method, in addition to the growth test at 42º and 45ºC and the Hypertonic Saboraud broth test. Symptomatology, risk factors and anal colonization were assessed according to positive or negative culture for Candida spp. The cultures positive for C. albicans at the two sites were compared with other results encountered. Yates' χ 2 test and Fisher's exact test were used for statistical analysis. RESULTS: the most frequent was C. albicans in 69% of the cases. Wearing tight and/or synthetic underclothing, the presence of allergic diseases, the occurrence of itching, leukorrhea and hyperemia showed a signifi cant association with positive culture for Candida spp in the vagina. The chance of a patient with positive anal colonization to present with concomitant positive vaginal colonization was 2.8 and 4.9 times greater for Candida spp and C. albicans, respectively. IntroduçãoO abortamento, uma das intercorrências mais freqüentes e preocupantes na gestação, por acometer anualmente grande parcela da população feminina na faixa etária economicamente ativa, vem sendo considerado importante proble-RESUMO Objetivo: fazer um estudo comparativo entre a curetagem uterina e a aspiração manual a vácuo (AMV) no tratamento do abortamento do primeiro trimestre da gestação. Métodos: foram incluídas 102 pacientes até a 12 a semana de gestação, com diagnóstico de abortamento, admitidas na Maternidade Escola Januário Cicco, entre janeiro de 1998 e julho de 2001, as quais foram aleatoriamente submetidas à curetagem uterina ou a AMV. As variáveis analisadas foram: controle da dor, necessidade de dilatação cervical mecânica, tempo de esvaziamento uterino, incidência de complicações e permanência hospitalar. As pacientes foram reavaliadas clínico-ecograficamente entre 7 e 10 dias após os procedimentos. Para a análise estatística foi utilizado o teste χ 2 . Resultados: a anestesia geral foi empregada em todas as pacientes submetidas a curetagem uterina e em nenhuma daquelas que realizaram AMV, cuja dor foi controlada em 64% dos casos com anestesia local. As diferenças entre os dois métodos quanto à necessidade de dilatação cervical mecânica, tempo de esvaziamento e incidência de complicações não foram significativas. A permanência hospitalar foi significativamente menor nas pacientes submetidas a AMV. Conclusões: não foram observadas vantagens de um método sobre o outro, quanto à técnica e à incidência de complicações. O uso não necessário de anestesia geral e a permanência hospitalar significativamente menor indicam que a AMV deve ser recomendada para todos os serviços com assistência obstétrica, aumentando a resolutividade e diminuindo os riscos, melhorando a qualidade da assistência.PALAVRAS-CHAVE: Curetagem uterina. No Brasil, em função do sub-registro, os números relacionados a abortamento e mortalidade são apenas estimados. Dados da última década indicam que anualmente são realizados entre 300.000 e 3 milhões de abortamentos ilegais, determinando mais de 200 mortes 3 .
ResumoOBJETIVO: Comparar, empregando a ultrassonografia transvaginal, a adequação da posição do dispositivo intrauterino (DIU) na cavidade uterina, em função do período: inserção pós-parto e pós-aborto versus inserção durante o ciclo menstrual. MÉTODOS: Estudo epidemiológico do tipo individuado, observacional e transversal, realizado entre fevereiro e julho de 2013. Foram incluídas 290 mulheres, sendo 205 com inserção no ciclo menstrual e 85 no pós-abortamento/ pós-parto. As variáveis independentes foram: idade, paridade, tempo de uso, época de inserção, número de retornos ao planejamento familiar, satisfação com o método, desejo de continuidade, queixas e complicações. A variável dependente foi a adequação do DIU na cavidade uterina. Para a análise estatística empregou-se o teste do χ 2 , com correção de Pearson, e o teste exato de Fisher, considerando um nível de significância de 5%. RESULTADOS: A idade média foi de 29,4 anos e o tempo médio de uso foi de 2,7 anos; 39,3% das mulheres tiveram queixas associados ao método, sendo a menorragia a mais frequente (44,7%). A satisfação foi de 85%, e 61,4% retornaram duas ou mais vezes para consultas. A faixa etária, a paridade e a posição do útero não se associaram com má adequação do DIU na cavidade uterina (p>0,05). A inserção no ciclo menstrual associou-se mais à posição adequada do DIU do que a inserção pós-parto/pós-abortamento, com significância estatística (p=0,028). CONCLUSÃO: A inserção no pós-parto e pós-abortamento apresentaram piores resultados quanto à adequação do DIU, não sendo observado o mesmo com a faixa etária, a paridade e a posição do útero na cavidade pélvica. METHODS: Epidemiologic, observational and cross-sectional study carried out between February and July, 2013. A total of 290 women were included, 205 of them with insertion during the menstrual cycle and 85 during the postpartum and post-abortion periods. The independent variables were: age, parity, time of use, insertion time, number of returns to family planning, satisfaction with the method, wish to continue using the device, symptoms and complications. The dependent variable was the adequate position of the IUD inside the uterine cavity. The χ 2 test with Pearson's correction and the Fisher exact test were used for statistical analysis, with the level of significance set at 5%. RESULTS: The average age was 29.4 years and the average time of IUD use was 2.7 years; 39.3% of the women had symptoms associated with the method, the most frequent being menorrhagia (44.7%). The degree of satisfaction was 85% and 61.4% of the women returned two or more times for consultation about family planning. Purpose of Investigation:To assess the ultrasound aspects of breasts and laser Doppler flow measurements of the internal mammary arteries (IMA) in pregnant, lactating, and non-pregnant (control) women. Materials and Methods: A cross-sectional observation study of 102 women, divided into three groups: control (CG), third trimester (G3), and lactation group (LG). The study variables were skin, subcutaneous adipose tissue, fibroglandular tissue, retromammary adipose tissue thickness, duct diameter, and pulsatility (PI) and resistance indices (RI) of the internal mammary arteries. Statistical analysis calculated the means and standard deviations, using two models Multivariate Analysis of Variance (MANOVA) models to assess the effect of groups on the mean values found and adjusted ANOVA models for individual means. The Tukey-Kramer multiple comparisons test was used to analyze intergroup. Results: There was no intergroup age difference (mean = 27 ± 4.76 years). A significant difference was observed between the G3/LG and the CG in terms of skin (p = 0.001), fibroglandular tissue thickness, duct diameter, and laser Doppler flow measurements of the IMA (all three with p < 0.001) and subcutaneous adipose tissue (p = 0.045). Conclusion: This study showed alterations in skin, subcutaneous adipose tissue, fibroglandular tissue, and duct diameter measurements, with a difference between pregnant/lactating women and the non-pregnant controls.
