BackgroundClostridium difficile infection is an important cause of morbidity and mortality but the optimal method of diagnosis for both patient management and infection prevention remains controversial.MethodsOur hospital made a decision to switch from the use of toxin immunoassay to a stand-alone nucleic acid test. This change was accompanied by the provision of clear sampling guidance and rejection criteria and this study aimed to assess the impact of that change. We analysed sample numbers, numbers of positive results, and the proportion of cases assessed as healthcare acquired over a 6-year period during which the testing method was changed from a toxin A/B immunoassay to a stand-alone commercial nucleic acid test after the first two years.ResultsSample numbers and numbers of cases assessed as healthcare acquired fell following the introduction of the nucleic acid test and sampling guidance, while infection rates in other hospitals in the same region remained relatively stable.ConclusionsIt is our opinion that the use of a highly sensitive assay together with clear sampling guidance offers the optimal approach to patient management and best use of isolation facilities.
We report a case of Helicobacter pylori transient bacteremia in a woman with ulcerated antral gastric cancer. The patient was hospitalized for laparoscopy and subtotal gastrectomy. After surgery she developed fever (39°C) and was empirically treated with levofloxacin. Blood cultures, collected and sent immediately to Laboratory, were positive for a spiral Gram-negative bacterium. This isolate was identified as H. pylori and the specific susceptibility test was performed. One day after the fever was decreased but antibiotic treatment with levofloxacin was continued and it was maintained until discharge. In summary, H. pylori transient bacteremia may occur as a rare complication after stomach surgery. Further studies are necessary to elucidate the potential role of Helicobacter pylori presence in blood.

Da considerare che gli errori più frequentemente riscontrati sono stati errori dovuti alla mancata identificazione del ceppo (7 su 242 pannelli) Anche per i risultati relativi all'esecuzione dell'antibiogramma Phoenix ha dimostrato estrema accuratezza. Le repliche hanno evidenziano grande riproducibilità dei dati. Le rare discrepanze riscontrate rispetto Kirby-Bauer hanno interessato soltanto intervalli Sensibile-Intermedio o Intermedio-Resistente Eccezionali sono le discrepanze Sensibile-Resistente.
INTRODUZIONELa crescente diffusione di microrganismi ESBLproduttori rappresenta un problema sia in ambito nosocomiale che comunitario (15). Le scelte terapeutiche, nel caso di infezioni gravi provocate da tali batteri, risultano limitate ed il rischio di mortalità elevato (6). Le ESBL possono conferire resistenza ai monobattami, alle cefalosporine a spettro esteso come cefotaxime, ceftriaxone e ceftazidime ed, in alcuni casi, anche a cefepime e cefpirome (1). I geni che codificano per le ESBL sono generalmente localizzati a livello plasmidico e vengono acquisiti mediante trasferimento orizzontale. oirel, et al (13) hanno dimostrato un'associazione tra la produzione di ESBL e l'espressione di resistenza qnrA-mediata ai fluorochinoloni. Ciò è attribuibile alla frequente co-presenza, a livello plasmidico, di geni qnr e di geni codificanti per vari tipi di ESBL, AmpC (5) o metallo-β-lattamasi (10). Originatisi da mutazioni degli enzimi progenitori TEM e SHV, ad oggi sono stati descritti numerosi tipi di ESBL, quali le CTX-M, divenute predominanti nell'ultimo decennio (8).Il rilievo della produzione di ESBL negli enterobatteri rimane una sfida per i microbiologi. Sebbene recenti cambiamenti nei breakpoint delle cefalosporine di terza generazione abbiano diminuito la possibilità di refertare i microrganismi ESBL-produttori come sensibili a queste molecole (CLSI Giugno 2010, EUCAST Version 1.1 April 2010), l'identificazione delle ESBL risulta fondamentale al fine sia di prevenire la disseminazione mediantre cross-trasmissione degli ESBLproduttori sia a scopo epidemiologico. Uno studio condotto nel 2008 (17) ha inoltre chiaramente dimostrato l'importanza della rapidità nell'impostazione di una terapia mirata, in caso di infezione causata da batteri ESBL-produttori. Risulta pertanto indispensabile disporre di metodi diagnostici rapidi, sensibili e che non necessitino di ulteriori test di conferma nel permettere la corretta identificazione dei microrganismi produttori di tali enzimi idrolizzanti. Diversi sono i test utilizzabili allo scopo di rilevare i batteri ESBL-produttori.
