BACKGROUND: Traditional cosmetic prosthetic designs for a long shoulder stump in children have several disadvantages, such as a rigid uncomfortable receiving socket, the need to use a strap fastening, and the non-cosmetic product due to the disproportionate shoulder and forearm length compared with the preserved limb. AIM: This study aimed to design, manufacture, and test a prototype of the shoulder stump liner-socket. MATERIALS AND METHODS: A new liner-socket of a shoulder cosmetic prosthesis has been developed, confirmed by a Russian Federation patent for an invention, which for the first time, in the practice of domestic prosthetics, is made of two types of silicone and has been successfully used in cosmetic prosthetics for 17 children, of whom 10 previously used traditional long stump prosthesis. Satisfaction with new liner-socket prosthesis was analyzed using a psychometric scale compared with a conventional prosthesis. RESULTS: The developed design combines both the inner socket function, which allows the elbow hinge to be attached to it and the liner properties, which ensures a comfortable stay of the stump in the receiving socket due to the properties of silicone and excludes the use of traditional bandages. CONCLUSIONS: The presented medical and technical solution ensures the simplicity and reliability of fastening the cosmetic shoulder stump prosthesis, including on the long shoulder stump, without disturbing the relatively healthy limb proportions, as well as wearing comfort.

Санкт-Петербург, Россия Цель. Разработать технологию изготовления силиконовой приемной гильзы функционального протеза кисти. Материалы и методы. Разработана и впервые в практике отечественного протезирования апробирована технология изготовления вкладной приемной гильзы функционального протеза кисти из высокотемпературного (HTV) силикона, включающая традиционные этапы изготовления: слепок, позитив, изготовление приемной гильзы. Однако в каждом из этапов присутствуют технологические особенности, которые позволяют изготовить комфортную индивидуальную вкладную полноконтактную гильзу протеза кисти из силикона, которая успешно использована при изготовлении активных, рабочих и протезов с внешним источником энергии для пациентов с врожденными и приобретенными дефектами кисти. Результаты. Разработанная технология обладает целым рядом преимуществ перед традиционными, так как позволяет обеспечить комфортные условия полного контакта и частичного нагружения остаточного сегмента кисти булавовидной формы во вкладной приемной гильзе протеза. Эти свойства вкладной гильзы дают возможность изготовления максимально скелетированной несущей гильзы из композитных материалов для надежного крепления к ней комплектующих элементов функциональных протезов кисти. Заключение. Успешная апробация приемных гильз протеза кисти из HTV силикона создала условия для более широкого применения этой марки силикона при изготовлении нового поколения вкладных гильз протезов верхних и нижних конечностей. Background. Evaluation of the result of functional prosthetics in patients with different upper limb defects is a topical problem of modern prosthetics. Providing patients with non-functional hand stumps with functional prostheses is not wide-scale and refers to atypical and experimental prosthetics. While new functional prosthetic hands appear, there is no algorithm for evaluating the results of prosthetics. Aim. This study aimed to evaluate the result of functional prosthetics in children with congenital defects of the hand and fingers by active prostheses. Materials and methods. We observed 67 children with congenital hand defects, of which 22 were included in the experimental prosthetics group. Bench test station was used for an objective assessment of the residual function of the hand. The booth imitated the tasks of an international competition for the users of Cybathlon rehabilitation equipment and allowed users to perform a series of tests, for each of which a certain number of evaluation points were awarded. Samples at the stand were supplemented with a subjective assessment of the function of the hand using the DASH questionnaire validated in Russia. Results. The best subjective assessment of the supply of an active prosthetic hand was determined in the patients with underdeveloped hand similar to the truncation of the hand proximal to the metacarpophalangeal joints. The lowest functionality score of the active prosthetic hand was obtained in cases of ectrodactyly and hypoplasia of 1–5 fingers, which was associated with a high residual functionality of the hand. Conclusions. In addition to training functions, the developed bench test station serves a diagnostic function as it evaluates the results of functional prosthetics in patients with upper limb defects in different levels, including defects on the hand and fingers. The results of the study on the station correlate with the results of the DASH questionnaire.
