Background: Many patients with several concurrent medical conditions (multimorbidity) are seen in the primary care setting. A thorough understanding of outcomes associated with multimorbidity would benefit primary care workers of all disciplines. The purpose of this systematic review was to clarify the relationship between the presence of multimorbidity and the quality of life (QOL) or health-related quality of life (HRQOL) of patients seen, or likely to be seen, in the primary care setting.
BackgroundAmong the strategies used to reform primary care, the participation of nurses in primary care practices appears to offer a promising avenue to better meet the needs of vulnerable patients. The present study explores the perceptions and expectations of patients with multimorbidity regarding nurses' presence in primary care practices.Methods18 primary (health) care patients with multimorbidity participated in semi-directed interviews, in order to explore their perceptions and expectations in regard to the involvement of nurses in primary care practices. Interviews were audio-recorded and transcribed. After reviewing the transcripts, the principal investigator and research assistants performed thematic analysis independently and reached consensus on the retained themes.ResultsPatients with multimorbidity were open to the participation of nurses in primary care practices. They expected greater accessibility, for both themselves and for new patients. However, the issue of shared roles between nurses and doctors was a source of concern. Many patients held the traditional view of the nurse's role as an assistant to the doctor in his or her various duties. In general, participants said they were confident about nurses' competency but expressed concern about nurses performing certain acts that their doctor used to, notwithstanding a close collaboration between the two professionals.ConclusionPatients with multimorbidity are open to the involvement of nurses in primary care practices. However, they expect this participation to be established using clear definitions of professional roles and fields of practice.
On the eve of the 1978, the World Health Organization has had a conference marked by the Alma Ata Declaration regarding primary health care. From this conference, the Cameroonian government took upon itself to guarantee equity on health care within the framework of a development policy oriented towards attaining social justice and assuring "health for all by the year 2000". It is from this perspective that, in 2002, a research program based on health in the urban settings was conducted in order to examine the status of health of the population. The objective of this study was to identify diarrhea risk factors in the city and to measure their prevalence and establish their spatial distribution in order to be able to evaluate the degree of progress achieved in the area of infant health since 1978. Microbiological tests revealed a diarrhea prevalence of 14.4%. Amongst all risk factors examined, the standard of living of households was closely linked to the incidence of diarrhea in Yaoundé. In addition, it was established that the incidence of diarrhea varied considerably from individual and area level.
L’hôpital est un milieu qui accueille les personnes âgées et dans lequel ces dernières ont à faire des apprentissages à un moment où elles ne sont pas en pleine possession de toutes leurs capacités physiques, psychologiques et cognitives. Elles peuvent alors y vivre des situations humiliantes qui génèrent des sentiments d’inconfort, de gêne et de honte. La présence d’intervenants peu chaleureux, manquant de compassion et empreints de préjugés négatifs envers les personnes âgées, est un autre facteur qui s’ajoute pour les amener à ne pas se sentir à l’aise, entraînant, entre autres conséquences, une baisse de leur estime de soi. Or, le toucher affectif est une stratégie qui aurait le potentiel d’agir sur la valeur personnelle des personnes âgées et d’améliorer ainsi leur estime de soi. C’est dans la perspective de vulgariser l’utilisation du toucher affectif dans la pratique infirmière qu’une étude a été réalisée afin de vérifier ses effets sur l’estime de soi des personnes âgées. Les résultats obtenus confirment que le toucher affectif influence positivement l’estime de soi des personnes âgées. Les auteurs de l’étude recommandent donc la systématisation du toucher affectif dans la pratique infirmière.
