Objective: Evaluate the intrathecal use of morphine-bupivacaine and bupivacaine in gynecological surgeries. Material and methods: An analytic longitudinal prospective study was perfomed at the Manuel Piti Fajardo Hospital in Florida, from February 2019 to March 2020. The universe was compound by 180 patients. Sample was select about 60 patients by probabilistic simple randon sampling distributed in two groups: A (Morphine -Bupivacaine) and B (Bupivacaine) according to inclusion criteria. The data were collected in questionnaire properly to investigation with baseline data in Statistical Package for Social Sciences. A univariate and multivariate analysis were conducted. Results: Complications and adverse reactions and their probability of presentation in the morphine=bupivacaine group was greater, pruritus (3.5 times), nausea and vomiting (2.8 times), hypotension (2.1 times) and urinary retention (2.1 times) corroborated by confidence intervals.This group had ten times more probability to have tolerable trouble or no pain (QR=10,2) that bupivacaine group, all of which showed moderate to several pain;10 times more probability of analgesia greater to 24 hours (QR=10,7) compared to bupivacaine and 8 times more likely to perceive good satisfaction. Conclusions: Evaluation of pain postoperative and analgesic time were better in patients managed with morphine =bupivacaine that resulted in analgesic of the greatest quality although prevailed pruritus, nausea and vomiting, hypotension and urinary retention like adverse reactions.
Introducción: Dos pandemias quedarían unidas en pleno siglo 21, el VIH-SIDA y la COVID-19. La primera con control en la mayoría de los países desarrollados y en vías de desarrollo, con tendencia a la disminución de las muertes; y la COVI-19 con rebrotes con incremento de contagios y muertes. Objetivo: Describir el comportamiento clínico-epidemiológico de la COVID-19 en los pacientes viviendo con el virus en Florida, 2021. Métodos: Estudio descriptivo y retrospectivo realizado a 21 pacientes que viven con VIH-SIDA que sufrieron infección de COVID-19 evaluando las variables socio-demográficas de la muestra: edad, sexo, procedencia; además formas de contagio epidemiológico, formas clínicas de presentación y evolución del cuadro clínico. Resultados: Predominaron los pacientes de 19 a 39 años 49,9%, el sexo masculino 57,1% y de procedencia urbana 71,4%; más de la mitad eran universitarios. La mayoría eran pacientes con control del VIH 61,9% y el 95,2% recibían tratamiento antirretroviral. La rinorrea (52,4%), obstrucción nasal y cefalea (33,3% cada uno), junto a la fiebre (28,6%) fueron los síntomas predominantes. En 42,9% el principal factor de riesgo fue ser contacto de un confirmado. Sólo 4,8% de los pacientes presentaron formas críticas, ninguno falleció. Discusión: La infección de VIH/SIDA asociada al contagio de COVID-19 no predispone a un incremento en la mortalidad y desarrolló un comportamiento similar a lo revisado en la bibliografía consultada. Conclusiones: El comportamiento clínico y epidemiológico de la infección de COVID-19 en pacientes que viven con el virus es similar a los que no portan esta condición.
Introducción: La epilepsia es una alteración del sistema nervioso central por incremento y sincronización anormal de la actividad eléctrica, que se puede manifestar por episodios recurrentes, espontáneos, intensos e impredecibles. Objetivo: Caracterizar a los niños epilépticos de difícil control según aspectos clínicos, epidemiológicos y terapéuticos. Material y métodos: Se realizó un estudio descriptivo transversal con niños epilépticos de difícil control vistos en la consulta neuro-pediátrica del centro ambulatorio en el hospital pediátrico provincial de Camagüey entre septiembre de 2019 y febrero de 2020. La muestra quedó formada por 69 niños. Se evaluó el comportamiento por edad, sexo, edad de comienzo, antecedentes prenatales, perinatales y posnatales, antecedentes familiares, tipo de crisis, manifestaciones electro-encefalográficas y drogas empleadas. Resultados: Prevaleció el sexo masculino en los pacientes con epilepsia de dificil control y la etapa de lactante fue la más representada en cuanto edad de inicio. La etiología más identificada fue la perinatal, predominando la hipoxia al nacer. Las crisis generalizadas fueron mayoría en dichos pacientes. La droga más utilizada fue el valproato de sodio, encontrándose el mayor por ciento de los pacientes en el estado de vivos controlados. Conclusiones: Los varones y el grupo de 6 a 9 años fueron los más representativos con un predominio de inicio en la lactancia. De los factores causales fueron los perinatales y la asfixia perinatal los más relevantes siendo las crisis generalizadas las más identificadas. Casi la totalidad de los pacientes presentaron manifestaciones electro-encefalográficas. El medicamento más usado fue el valproato de sodio.
