Background Perinatal arterial ischemic stroke (PAIS) is a neurologic disorder leading to long-term complications. Mesenchymal stem cells (MSCs) have emerged as a novel therapeutic agent. This systematic review aims to determine the effects of stem cell-based interventions for the treatment of PAIS in preclinical studies. Methods We included all controlled studies on MSCs in neonatal animals with PAIS. Functional outcome was the primary outcome. The literature search was performed in February 2021. Results In the 20 included studies, MSCs were most frequently delivered via intracerebral injection (n = 9), 3 days after the induction of PAIS (n = 8), at a dose ranging from 5 × 104 to 5 × 106 cells. The meta-analysis showed an improvement on the cylinder rearing test (MD: −10.62; 95% CI: −14.38 to −6.86) and on the water maze test (MD: 1.31 MD; 95% CI: 0.80 to 1.81) in animals treated with MSCs compared to the control group animals. Conclusion MSCs appear to improve sensorimotor and cognitive performance in PAIS-injured animals; however, the certainty of the evidence is low. Registration of the protocol of preclinical studies, appropriate sample size calculation, rigorous randomization, and reporting of the data on animal sex and survival are warranted. PROSPERO registration number: CRD42021239642. Impact This is the first systematic review and meta-analysis of preclinical studies investigating the effects of MSCs in an experimental model of PAIS. MSCs appear to improve sensorimotor and cognitive performance in PAIS-injured neonatal animals. The certainty of the evidence is low due to high or unclear risk of bias in most domains.
Prior studies have shown that self- and manual massage (SMM) increases flexibility in non-adjacent body areas. It is unclear whether this also influences performance in terms of force generation. Therefore, this study investigated the effect of SMM on the plantar surface on performance in the dorsal kinetic chain. Seventeen young participants took part in this within-subject non-randomized controlled study. SMM was applied on the plantar surface of the dominant leg, but not on the non-dominant leg. A functional performance test of the dorsal kinetic chain, the Bunkie Test, was conducted before and after the intervention. We measured the performance in seconds for the so-called posterior power line (PPL) and the posterior stabilizing line (PSL). The performance of the dominant leg in the Bunkie Test decreased significantly by 17.2% from (mean ± SD) 33.1 ± 9.9 s to 27.4 ± 11.1 s for the PPL and by 16.3% from 27.6 ± 9.8 s to 23.1 ± 11.7 s for the PSL. This is in contrast to the non-dominant leg where performance increased significantly by 5.1% from 29.7 ± 9.6 s to 31.1 ± 8.9 s for the PPL and by 3.1% from 25.7 ± 1.5 s to 26.5 ± 1.7 s for the PSL. SMM interventions on the plantar surface might influence the performance in the dorsal kinetic chain.
Zusammenfassung Hintergrund Es ist unklar, in welchem Umfang Beratung zu lebensstilbezogener Verhaltensänderung (BzlV) in Deutschland bereits in die physiotherapeutische Arbeit integriert ist und ob Physiotherapeut*innen die erforderlichen Kompetenzen aufweisen. Ziel Erfassung der Einstellung von Physiotherapeut*innen zur Gesundheitsförderung sowie zu Praxis, Kompetenzen und möglichen Barrieren für die Umsetzung von BzlV. Methode Mittels Online-Umfrage wurden in Deutschland tätige Physiotherapeut*innen zu ihren allgemeinen Einstellungen zur Gesundheitsförderung sowie zu Praxis, Kompetenzen und möglichen Barrieren für die Umsetzung von BzlV befragt. Neben einer deskriptiven Analyse der Angaben wurden Unterschiede zwischen akademisch und nicht akademisch ausgebildeten Physiotherapeut*innen mithilfe des Mann-Whitney-U-Tests ermittelt. Ergebnisse 511 Physiotherapeut*innen nahmen an der Umfrage teil, davon konnten 413 Fragebögen in die deskriptive Analyse eingeschlossen werden (Alter: 36,4 (± 10,88 SD) Jahre; w/m: 281/132). 92,7 % der Befragten hielten Gesundheitsförderung für einen fundamentalen Bestandteil der physiotherapeutischen Arbeit und 93,4 % stimmten zu, dass die Förderung eines gesunden Lebensstils zur Arbeit von Physiotherapeut*innen gehöre. Die Befragten erhoben (a) und berieten (b) am häufigsten zum Risikofaktor körperliche Inaktivität (a: 89,8 %; b: 91,3 %) und gaben an, über geringe bis mittlere Kompetenzen für die Beratung zur Verhaltensänderung zu verfügen. Akademisch ausgebildete Physiotherapeut*innen schätzten ihre Kompetenzen besser als nicht akademisch ausgebildete Physiotherapeut*innen (W = 8253,5, p = 0,001, r = 0,182) ein. Als häufigste Barrieren bei der Umsetzung von BzlV wurden strukturelle Ursachen, wie Zeitmangel (64,9 %) und fehlende Vergütung (70,5 %), genannt. Schlussfolgerung Physiotherapeut*innen haben überwiegend eine positive Einstellung zur Gesundheitsförderung und erachten BzlV als sehr wichtig. Derzeit bestehen aber noch viele Barrieren, die Physiotherapeut*innen an der Umsetzung hindern. Damit diese in Deutschland dennoch einen Beitrag zur Gesundheitsförderung leisten können, sollten Rahmenbedingen und Ausbildungsinhalte angepasst werden.
