In many developing countries like India, there is a widespread lack of general awareness about the importance of good oral health, which causes dental patients to neglect their oral hygiene, thus precipitating many long-term ailments. We developed an application that promotes the significance of regular dental checkups and oral health care by explaining to patients how these are intrinsic to overall health. Our application, in essence, extracts relevant health information from published scientific studies according to a patient’s medical history and shares it with the patient at the discretion of the supervising dentist, thereby empowering patients to make more informed decisions. We present a detailed overview of our semi- autonomous machine learning-based solution, along with the complex challenges involved in the design, development, and real-world deployment of our application. Finally, we conducted a randomized parallel-group study in India with 224 dental patients over two years to assess the utility of our proposed solution. Results show our application improved the patient recall rate from 21.1% to 37.8% (p-value = 0.024).
The mandible or lower jaw is one of the most commonly encountered maxillo facial fractures in our clinical practice. Un-displaced, single and closed fractures are usually managed by closed method with maxillo-mandibular fixation (MMF). Displaced open and multiple fractures are treated by open reduction and internal fixation (ORIF). Displaced, open and multiple fractures are treated by ORIF by various methods like intraosseus wiring, miniplate osteosynthesis and reconstruction plate with bone graft. Present study is done to evaluate these three methods of ORIF and their post-operative outcome. Our study material comprises 72 cases of isolated mandibular fracture of adult patient. All these patients are treated by ORIF by either of the three above methods. These patients are divided into three groups. Group A includes 18 cases treated by intra osseous wiring and post-operative maxillo mandibular immobilization for 4 weeks. Group B includes 38 cases treated with single or double miniplate and no post-operative immobilization. Group C includes 11 cases treated by reconstruction plate with bone graft along with miniplate in patients having bone loss or gap fractures. The surgical outcome based on parameters such as malocclusion, intra oral exposure of plate, wound dehiscence, delayed union, plate fracture, nonunion and sensory deficit are compared at 4-6 weeks after operation among the three groups and patients are followed up every three months interval. The results are statistically analysed and it is concluded that the use of miniplate gives better result in comparison to intra osseous wiring. In cases of bone loss or gap fracture reconstruction plate and bone graft along with miniplate are effective.
Auto-transplantation procedure, to replace lost tooth, where implant or fixed/ removable prosthetic option isn't recommended/ possible, as in child patients and young adults, have wide range of success rate. Not only proper case selection including transplantable tooth selection with ideal stages of root completion and root configuration, available bone and soft tissue; but also the surgical skill and post-surgical care play important roles for the success of the procedure. Here, 2 cases of auto-transplantation have been discussed.
