Latar Belakang, hipertensi merupakan penyakit yang menjadi persoalan dunia, hal ini dikarenakan prevalensi yang terus menerus mengalami peningkatan. Hampir 8 juta orang meninggal pertahun di seluruh dunia akibat penyakit hipertensi, sedangkan pada Kawasan Asia Tenggara sendiri hampir 1,5 juta orang tiap tahunnya. Penyakit hipertensi di Indonesia merupakan salah satu pencetus penting terjadinya mortalitas dan morbiditas. Data Kabupaten Kebumen penyakit hipertensi sebesar 23.735 kasus dan merupakan tiga teratas penyakit tidak menular. Penyakit hipertensi di RSU Purbowangi merupakan penyakit terbanyak nomor 2 setelah Diabetes Melitus. Tujuan penelitian, untuk mengetahui tingkat kepatuhan dan hubungan karakteristik pasien dengan kepatuhan minum obat. Metode penelitian, menggunakan metode crossectional dengan rancangan penelitian secara deskriptif menggunakan kuesioner MMAS-8. Hasil penelitian, tingkat kepatuhan penggunaan obat pada pasien hipertensi di Instalasi Rawat Jalan RSU Purbowangi pada kategori tinggi sebanyak 30 responden (46,2%), kategori sedang sebanyak 29 responden (44,6%), kategori rendah sebanyak 6 responden (9,2%) dan adanya hubungan yang signifikan antara pendidikan, pekerjaan dan penyakit penyerta responden dengan tingkat kepatuhan responden di Instalasi Rawat Jalan RSU Purbowangi (p-value< 0,05). Kesimpulan, kepatuhan penggunaan obat pada pasien hipertensi di Instalasi Rawat Jalan RSU Purbowangi berada pada kategori tinggi sebanyak 30 responden (46,2%) dan adanya hubungan yang signifikan antara pendidikan, pekerjaan dan penyakit penyerta responden dengan tingkat kepatuhan. Rekomendasi, perlu adanya penambahan jumlah responden yang lebih banyak dan mengendalikan terhadap variabel perancu.
