Objetivos: O objetivo deste estudo foi avaliar, ex vivo, a porcentagem da obtenção de patência apical nos canais radiculares dos primeiros molares superiores permanentes e a influência da ampliação cervical nesta verificação. Método: Foram analisados 73 primeiros molares superiores permanentes, selecionados através de exame visual e radiográfico. Para a execução do estudo, os dentes foram previamente abordados e os canais radiculares localizados. A patência apical foi verificada utilizando-se instrumentos de aço inoxidável #08 ou #10, de acordo com o diâmetro de cada canal radicular. Quando não se conseguia a patência apical, o terço cervical foi ampliado, com brocas de Gates-Glidden #1 e #2, seguidas de brocas LA Axxess #20 e a tentativa de obtenção de patência, executada novamente. Resultados: Os resultados obtidos, com relação à presença de patência apical, foram de 79,45% para os canais radiculares mesiovestibulares, 29,62% para os canais radiculares mesiopalatinos, 72,60% para os canais radiculares distovestibulares e 84,93% para os canais radiculares palatinos. Conclusão: Pode-se concluir que a ampliação do terço cervical aumentou em 8,42% a obtenção de patência apical em todos os canais radiculares. IntroduçãoDurante o preparo do canal radicular, raspas de dentina geradas pela instrumentação, bem como fragmentos da polpa apical, tendem a ser compactados no forame, o que pode causar um bloqueio apical e, consequentemente, causar uma interferência no comprimento de trabalho (1).As raízes dos molares superiores apresentam anatomicamente canais radiculares que representam um desafio técnico à execução de tratamento endodôntico. Por apresentarem curvatura na porção cervical dos canais radiculares, o acesso em linha reta, tanto dos instrumentos endodônticos como das soluções irrigadoras e dos materiais obturadores à porção apical do canal radicular é dificultado (2). A realização do desgaste cervical em molares resulta em diferentes benefícios ao tratamento do canal radicular, tais como a melhor determinação do comprimento de trabalho (3), a determinação mais precisa do instrumento apical inicial (4,5) e diminuição do risco de ocorrência de desvio apical durante a instrumentação do canal radicular (6).Os primeiros molares superiores apresentam-se comumente com três raízes: mesiovestibular, distovestibular e palatina (7-8), posicionadas, em geral, de forma divergente. As raízes mesiovestibulares apresentam-se com menor volume e canais radiculares mais atrésicos, tendo, portanto, maior dificuldade de localização.
