Introduction: Vitamin C is known as antioxidant and an important co-factor for endogenous adrenaline synthesis while vitamin B1 is known for its effect on lactate clearance which helps sepsis and septic shock condition. This study was conducted to investigate the effect of vitamin C and B1 combination on 28-day mortality in sepsis and septic shock patients. Methods: This is an open-label randomized controlled trial at tertiary hospital between July until September 2019. Inclusion criteria were patients aged 18-60 years diagnosed with sepsis and/or septic shock. Exclusion criteria were "do not resuscitate" status and on immunosuppressive medications. Thirty-one patients were divided into intervention group (n=15) and control group (n=16). Intervention group received 1.5 g vitamin C every 6 hours and 200 mg vitamin B1 every 12 hours through 3 days. Control group received 200 mg vitamin B1 every 12 hours for 3 days. The 28-day mortality was documented after the administration. This study used chi-square continuity correction as statistical analysis. Results: The relative risk for survival rate in 28-day mortality was 1.30 times higher for intervention group compared to control group (CI 95% 0.76 -2.21 p=0.53). The mortality risk in 28 days was also lower in intervention group compared to control group (RR=0.61 CI95% 0.22 -1.66 p=0.53). Conclusion: combination of vitamin C and B1 was able to lower the risk of 28-day mortality although the result wasnot statistically significant.
Vitrektomi adalah operasi mata untuk mengangkat bagian vitreous body. Hingga saat ini berbagai teknik anestesi tengah dikembangkan untuk mencapai kondisi teknik anestesi ideal. Teknik anestesi kombinasi general anesthesia dengan blok subtenon pada operasi vitrektomi memiliki potensi yang bagus untuk mengurangi nyeri pascaoperasi, mengurangi insiden refleks okulokardiak dan mengurangi jumlah penggunaan analgesia. Pada laporan ini, dilaporkan pasien laki-laki berusia 40 tahun dengan keluhan penglihatan kabur pada mata kanan sejak tiga bulan yang lalu. Pasien memiliki riwayat hipertensi, diabetes melitus tipe II, dan memiliki riwayat penggunaan heparin. Pasien didiagnosis non-closing vitreous hemorrhage. Pasien direncanakan dilakukan vitrektomi dengan general anesthesia yang dikombinasikan dengan blok subtenon. Selama operasi tidak terjadi perubahan hemodinamik yang bermakna. Keberhasilan teknik anestesi kombinasi ini ditunjukkan oleh hemodinamik yang stabil selama operasi. Teknik anestesi kombinasi pada pasien dengan komorbid yang menjalani vitrektomi seperti pada kasus ini dapat dipertimbangkan sebagai pilihan pada pengelolaan kasus sejenis di masa yang akan datang.
Background: NSAID (Non steroid anti inflammatory drug ) inhibit cyclooxygenase enzyme and also inhibition of proinflamation prostaglandin (PG). Ketorolac has recommended for a short time use on post operation, not longer than 2 days intravenous administration. So this drugs is good for acute analgesia. The research useful for looking an effect of ketorolac administration as a preventive analgesia concern to pupil in prevent myosis in eye's surgery within general anesthesia. Methods: it was a quasi experimental study done in Sumatera Eye Centre Hospital, Medan, Indonesia during January to June 2012. Sampling was taken using total sampling method with 44 patients.
Background & Objective: Novel Coronavirus-2019, the cause of COVID-19, was first discovered at the end of 2019 in Wuhan City. Analysis of confirmed cases in China states that 7%-26% of patients with severe disease required intensive care. The high number of cases of COVID-19 forced the ‘North Sumatra Indonesian Society of Anesthesiology & Intensive Therapy (NSISAIT) COVID-19 Task Force’ to manage and monitor appropriate intensive therapy at selected referral hospitals.
We conducted this study to know the comfort level of the NSISAIT COVID-19 Task Force during the management of intensive therapy at the COVID-19 referral hospitals.
Methodology: This study used a descriptive observational method with a cross sectional design. A questionnaire was designed and circulated by electronic means among the physicians involved in management of COVID-19 patients as a part of NSISAIT COVID-19 Task Force. The responses received were grouped and categorized using MS Excel software.
Results: The results of the study showed that the comfort level of the NSISAIT COVID-19 Task Force was categorized as satisfactory by almost half of the respondents. The COVID-19 handling was categorized as very good by 34 (65.4%) respondents; 25 (48.1%) felt a fairly good level of security, 21 (40.4%) answered that the facilities and infrastructure at the referral hospitals were quite good, 28 (53.8%) respondents received a very good award; and 27 respondents (51.9%) carried out intensive therapy management quite well.
Conclusion: The comfort level of the NSISAIT COVID-19 Task Force in the management of intensive therapy at the COVID-19 referral hospitals was in the comfortable category.
