Choledochocele is a rare form of choledochal cyst [1, 2]. Usually, it manifests clinically with epigastric pain of colic type, jaundice, and/or pancreatitis. Occasionally, a palpable mass may be found. We describe two cases of intestinal intussusception as a presenting feature of choledochocele.
Introduction: Papillary thyroid carcinoma (PTC) is the most common neoplasm of the endocrine system and the least aggressive, representing between 70% and 80% of thyroid neoplasms. This is characterized by slow growth and a low risk of progression to metastasis. Brain Metastases (BM) associated with PTC occur with a incidence rate of 0.15%. Tyrosine kinase inhibitors (TKI) are a very promising alternative for managing these patients. Here is described the case of a patient with PTC with brain and lung metastases, her respective management and evolution. Clinical Case Report: A 64-year-old patient with a history of controlled hypothyroidism, who was recently diagnosed with high cell variant PTC, positive for BRAF V600E mutation. Neck CT revealed a thyroid mass extending to the superior mediastinum, with associated lung metastases. We started by suppressing TSH with levothyroxine. Brain MRI was done due to persistent and as safety precaution before starting thyrotropin alfa This showed multiple intra-parenchymal lesions compatible with BM. Patient received management with gamma Knife radiosurgery for BM and with lenvatinib 24mg day. The patient presented a decrease in tumor size by more than 50% in the neck, with disappearance of the majority of BM at 90 days. After 18 months of treatment, she was still alive. Discussion and Conclusion: PTC associated with BM has a very poor life expectancy, and there are not guidelines for its management and the treatment is based on case reports and expert opinions. Radiosurgery plus ITK combined therapy could provide better outcomes to these patients
Introducción: el abordaje del paciente con diabetes mellitus tipo 2 ha cambiado a lo largo del tiempo. Años anteriores el enfoque terapéutico giraba en torno al nivel de hemoglobina glicada (HbA1c); ahora, con el advenimiento de nuevas moléculas, y luego de entender que la meta terapéutica no es sólo un punto de corte de HbA1c, se ha modificado el objetivo, para estar centrado en el paciente, en sus comorbilidades macro y microvasculares, riesgo cardiovascular y falla cardíaca. En esta revisión de la literatura se exponen las preguntas que se deben resolver al momento de definir una estrategia terapéutica en el paciente con diabetes mellitus tipo 2. Métodos: valoración de guías locales e internacionales de tratamiento de diabetes mellitus tipo 2. Resultados: se valoraron las guías del tratamiento de la diabetes mellitus tipo 2 realizadas por la Asociación Colombiana de Endocrinología, Asociación Colombiana de Medicina Interna, Asociación Colombiana de Nefrología, Sociedad Colombiana de Cardiología y Cirugía Cardiovascular, American Diabetes Association, American Association of Clinical Endocrinology, European Association for the Study of Diabetes y European Society of Cardiology. Además, se revisaron los ensayos clínicos que respaldan el uso de los diferentes grupos farmacológicos disponibles para el tratamiento de los pacientes con diabetes mellitus tipo 2. Conclusión: es menester del clínico realizar el abordaje terapéutico de los pacientes con diabetes mellitus tipo 2 de forma individualizada, centrado en el riesgo cardiovascular, comorbilidades asociadas, riesgo de hipoglucemia y costos de la intervención brindada.
Introducción: los inhibidores de punto de control inmunitario (IPCI) han revolucionado el tratamiento de los pacientes con cáncer, incrementando la supervivencia libre de enfermedad y son terapias con aval para el manejo de neoplasias sólidas. Entre los eventos adversos frecuentes se encuentran los que ocurren a nivel del sistema endocrino y se reporta el caso de una paciente tratada con anti CTLA-4 y anti PD-1 que desarrolló hipofisitis y adrenalitis. Objetivo: describir el caso de una paciente que presentó endocrinopatías (hipofisitis y adrenalitis) asociadas al uso de IPCI, en una clínica en la ciudad de Medellín, Colombia. Presentación del caso: mujer de 74 años con antecedente de carcinoma de células renales tratada con ipilimumab y nivolumab, suspendidos por neumonitis. Tres meses después acudió a urgencias por disminución de clase funcional y somnolencia, el examen físico mostró hipotensión y se evidenció hiponatremia, hipotónica y cortisol basal disminuido. Por ello, se solicitó perfil hipofisario, encontrando somatomedina y hormona folículo estimulante (FSH) bajas y hormona adrenocorticotropa ACTH elevada, además, se realizó resonancia de silla turca sin alteración estructural y se concluyó hipofisitis con imagen normal e insuficiencia adrenal primaria secundaria a adrenalitis. Discusión y conclusión: los inhibidores del punto de control inmunitario son medicamentos actualmente utilizados para diferentes tipos de neoplasias, estos medicamentos se asocian en un porcentaje significativo con endocrinopatías, sin embargo, encontrar la afectación de dos glándulas endocrinas es inusual, este es el segundo caso reportado en la literatura con hipofisitis y adrenalitis. Se deben reconocer las manifestaciones clínicas de las endocrinopatías asociadas a la inmunoterapia, teniendo en cuenta el uso creciente de estas terapias para diferentes neoplasias. El compromiso de más de una glándula endocrina es infrecuente, sin embargo, se debe sospechar y reconocer. Además, es importante establecer en cada centro un protocolo para el abordaje y el seguimiento de estos pacientes.
